I. PERMITTED PROJECT:
A. WORK & DIMENSIONS:
• Removal of 1 dock pile.
• Installation of a 282.5 square foot T-shaped wood dock consisting of a 5 foot wide by
17.5 foot long (87.5 square foot) access walkway located between 39 and 44 linear feet
from the north property line and a 10 foot wide by 19.5 foot long (195 square foot)
terminal platform located between 31.5 and 51 linear feet from the north property line.
The dock will be supported by 1 new wood pile and 8 existing wood piles.
B. REQUIRED MITIGATION:
• N/A
II. COMPLETED PROJECT:
A. WORK & DIMENSIONS:
• The 4 existing unauthorized cleats and the existing submerged dock debris required to be
removed have been removed.
• All other work as per final approved plans with minor deviations.

III. NOTES/RECOMMENDATIONS:
The authorized structures have been installed in substantial compliance with the above
referenced Class I permit. Recommend closure of the file. See attached photo card ?nd
