524.1 Scope. As it pertains to this section, it is the responsibility of the roofing contractor to provide the owner
ith the required roofing permit, and to explain to the owner the content of this section. The provisions of
hapter 15 of the Florida Building Code, Building govern the minimum requirements and standards of the
tdustry for roofing system installations. Additionally, the following items should be addressed as part of the
reement between the owner and the contractor. The owner's initial in the designated space indicates that the

1. Aesthetics - workmanship: The workmanship provisions of Chapter 15 (High Velocity Hurricane
nS e for the purpose of providing that the roofing system meets the wind resistance and water intrusion
erformance standards. Aesthetics (appearance) are not a consideration with respect to workmanship provisions.

2. Renailing wood decks: When replacing roofing, the existing wood roof deck may have to be
ed in accordance with the current provisions of Chapter 16 (High Velocity Hurricane Zones) of the Florida
ng Code. (The roof deck is usually concealed prior to removing the existing roof system).
3. Common roofs: Common roofs are those which have no visible delineation between neighboring
i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing contractor and/or
wne,, should notify the occupants of adjacent units of roofing work to be performed.
4. Exposed ceilings: Exposed, open beam ceilings are where the underside of the roof decking can be
ed from below. The owner may wish to maintain the architectural appearance; therefore, roofmg nail
:,netrations of the underside of the decking may not be acceptable. The owner provides the option of
lain ining this appearance.
5. Ponding water: The current roof system and/or deck of the building may not drain well and may
e water to pond (accumulate) in low -lying areas of the roof. Ponding can be an indication of structural
istress and may require the review of a professional structural engineer. Ponding may shorten the life
+cpectancy and performance of the new roofmg system. Ponding conditions may not be evident until the origina
iofi t system is removed. Ponding conditions should be corrected.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed by the BCCO and accepted by the Building
Code and Product Review Committee to be used in Miami Dade County and other areas where allowed
by the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product
Control Division (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County)
reserve the right to have this product or material tested for quality assurance purposes. If this product or
material fails to perform in the accepted manner, the manufacturer will incur the expense of such testing
and the AHJ may immediately revoke, modify, or suspend the use of such product or material within
their jurisdiction. BORA reserves the right to revoke this acceptance, if it is determined by Miami -Dade
County Product Control Division that this product or material fails to meet the requirements of the
applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code and the High Velocity Hurricane Zone of the Florida Building Code.
DESCRIPTION: GAF Conventional Built-Up Roof System for Wood Decks.
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state
and following statement: "Miami-Dade County Product Control Approved ", unless otherwise noted
herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has
been no change in the applicable building code negatively affecting the performance of this product
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or
change in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an
endorsement of any product, for sales, advertising or any other purposes shall automatically terminate
this NOA. Failure to comply with any section of this NOA shall be cause for termination and removal of
NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is
displayed, then it shall be done in its entirety.

WOOD DECK SYSTEM LIMITATIONS:
1 A slip sheet is required with Ply 4 and Flex PIyTM 6 when used as a mechanically fastened base or anchor
sheet.
2. Minimum '/" Dens DeckTM or ' /2" Type X gypsum board is acceptable to be installed directly over the
wood deck.
GENERAL LIMITATIONS:
1. Fire classification is not part of this acceptance, refer to a current Approved Roofing Materials Directory
for fire ratings of this product.
2. Insulation may be installed in multiple layers. The first layer shall be attached hi compliance with
Product Control Approval guidelines. AlI other layers shall be adhered in a full mopping of approved
asphalt applied within the EVT range and at a rate of 20-40 lbsfsq., or mechanically attached using the
fastening pattern of the top layer
3. All standard panel sizes are acceptable for mechanical attachment. When applied in approved asphalt,
panel size shall be 4' x 4' maximum.
4. An overlay and/or recovery board insulation panel is required on all applications over closed cell foam
insulations when the base sheet is fully mopped. If no recovery board is used the base sheet shall be
applied using spot mopping with approved asphalt, 12" diameter circles, 24" o.c.; or strip mopped r
ribbons in three rows, one at each sidelap and one down the center of the sheet allowing a continuous
area of ventilation. Encircling of the strips is not acceptable. A 6" break shall be placed every 12' in
each ribbon to allow cross ventilation. Asphalt application of either system shall be at a minimum rate of
12 lbs sq. Note: Spot attached systems shall be limited to a maximum design pressure of -45 psf.
5. Fastener spacing for insulation attachment is based on a Minimum Characteristic Force (F) value of 275
lbf., as tested in compliance with Testing Application Standard TAS 105. If the fastener value, as field -
tested, are below 275 lb£ insulation attachment shall not be acceptable.
6. Fastener spacing for mechanical attachment of anchor/base sheet or membrane attachment is based on a
minimum fastener resistance value in conjunction with the maximum design value listed within a specific
system. Should the fastener resistance be less than that required, as determined by the Building Official,
a revised fastener spacing, prepared, signed and sealed by a Florida Registered Engineer, Architect, or
Registered Roof Consultant may be submitted. Said revised fastener spacing shall utilize the withdrawal
resistance value taken from Testing Application Standards TAS 105 and calculations in compliance with
Roofing Application Standard RAS 117.
7. Perimeter and corner areas shall comply with the enhanced uplift pressure requirements of these areas.
Fastener densities shall be increased for both insulation and base sheet as calculated in compliance with
Roofing Application Standard RAS 117. Calculations prepared, signed and sealed by a Florida registered
Professional Engineer, Registered Architect, or Registered Roof Consultant (When this limitation is
specifically referred within this NOA, General Limitation #9 will not be applicable.)
8. All attachment and sizing of perimeter milers, metal profile, and/or flashing termination designs shall
conform with Roofing Application Standard RAS 111 and applicable wind load requirements.
9. The maximum designed pressure limitation listed shall be applicable to all roof pressure zones (i.e. field,
perimeters, and corners). Neither rational analysis, nor extrapolation shall be permitted for enhanced
fastening at enhanced pressure zones (i.e. perimeters, extended corners and corners). (When this
limitation is specifically referred within this NO.A, General Limitation #7 will not be applicable.)
10.. All products listed herein shall have a quality assurance audit in accordance with the Florida Building
Code and Rule 9B-72 of the Florida Administrative Code.

ASPHALT FELT SYSTEMS WITH HOT ROOFING ASPHALT
Type G2 asphalt glass mat base sheet ( "GAFGLAS® #75 Base Sheet" or "Tri-Ply® #75 Base Sheet ", or "GAFGLAS® #80 ULTIMA") is a
suitable altemate for Type G1 asphalt glass fiber ply sheet ( "GAFGLAS® Ply 4" or "Tri-Ply® Ply 4 ", or "GAFGLAS® Ply 6 °) in the Class A, B

The roof deck may first be covered with one ply Type G2 asphalt saturated glass mat base sheet "GAF Stratavent® Eliminatorr' Venting Base
Sheet (Perforated)" or "GAF Stratavent® Eliminator" Venting Base Sheet ( Nailable)." Perforated to be loose laid or mopped and nailable to
be mechanically attached granule side down.
As an option Type G2 asphalt glass mat base sheet ( "GAFGLAS® #75 Base Sheet° or "Tri-Ply® #75 Base Sheet" or " GAFGLAS® #80 ULTIMA"
or "GAF Stratavent® Eliminator" Venting Base Sheet ( Nailable)" may be substituted for Type G1 asphalt glass fiber ply sheet ( "GAFGLAS®

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed and accepted by Miami -Dade County BNC - Product Control
Section to be used in Miami Dade County and other areas where allowed by the Authority Having Jurisdiction
(AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section (In Miami Dade County) and/or the AHJ (m areas other than Miami Dade County) reserve the right
to have this product or material tested for quality assurance purposes. If this product or material fails to
perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may
immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. BNC
reserves the right to revoke this acceptance, if it is determined by Miami -Dade County Product Control
Section that this product or material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.

RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall

3. LIMITATIONS:
3.1 Fire classification is not part of this acceptance.
3.2 For mortar or adhesive set tile applications, a static field uplift test in accordance with TAS
106 may required, refer to applicable building code.
3.3 Applicant shall retain the services of a Miami -Dade County Certified Laboratory to perform
quarterly test in accordance with TAS 112, appendix `A'. Such testing shall be submitted to
the Building Code Compliance Office for review.
3.4 Minimum underlayment shall be in compliance with the applicable Roofing Applications
Standards listed section 4.1 herein.
3.5 30/90 hot mopped underlayment applications may be installed perpendicular to the roof slope
unless stetted otherwise by the underlayment materiahnanufacturers published literature. -
3.6 This acceptance is for wood deck applications. Minimum deck requirements shall be in
compliance with applicable building code.
4. INSTALLATION
4.1 Eagle Roofing Products LLC Bel Air, Bel Air Double Eagle, Ponderosa, Golden Eagle,
Ponderosa Double Eagle Flat Concrete Roof Tiles and its components shall be installed in
strict compliance with Roofing Application Standard RAS 118, RAS 119 and RAS 120.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials. The
documentation submitted has been reviewed and accepted by Miami -Dade County PERA - Product Control Section to
be used in Miami Dade County and other areas where allowed by the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. PERA reserves the right
to revoke this acceptance, if it is determined by Miami -Dade County Product Control Section that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.
DESCRIPTION: 3M' ' 2- Component Foam Roof Tile Adhesive AH -160
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following
statement: "Miami -Dade County Product Control Approved ", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change
in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product, for
sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any section of
this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall be done
in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors and
shall be available for inspection at the job site at the request of the Building Official.
This renews and revises NOA# 11- 0124.04 and consists of pages 1 through 7.

LIMITATIONS:

INSTALLATION:
1. 3MTM 2- Component Foam Roof Tile Adhesive AH -160 may be used with any roof tile assembly having a current
NOA that lists uplift resistance values with the use of 3MTM 2- Component Foam Roof Tile Adhesive AH -160.
2. 3MTM 2- Component Foam Roof Tile Adhesive AH -160 shall be applied in compliance with the Component
Application section and the corresponding Placement Details noted herein. The roof tile assembly's adhesive
attachment with the use of 3MTM 2- Component Foam Roof Tile Adhesive AH -160 shall provide sufficient
attachment resistance, expressed as an uplift based system, to meet or exceed the uplift resistance determined in
compliance with Miami -Dade County Roofing Application Standards RAS 127. The adhesive attachment data is
noted in the roof tile assembly NOA.
3. 3MTM 2- Component Foam Roof Tile Adhesive AH -160 and its components shall be installed in accordance with
Roofing Application Standard RAS 120, and 3M Company's 3MTM 2- Component Foam Roof Tile Adhesive AH-
160 Operating Instruction and Maintenance Booklet.
4. Installation must be by a Factory Trained 'Qualified Applicator' approved and licensed by 3M Company. 3M
Company shall supply a list of approved applicators to the authority having jurisdiction.
5. Calibration of the Foam Dispenser RTF1000 dispensing equipment is required before application of any adhesive.
The mix ratio between the "A" component and the "B" component shall be maintained between 1.0 -1.15 (A): 1.0
(B). The dispense timer shall be set to deliver 0.0175 to 0.15 pounds per tile as determined at calibration. No
other settings shall be approved.
6. 3MTM 2- Component Foam Roof Tile Adhesive AH -160 shall be applied with Foam Dispenser RTF1000 or
ProPack® 30 & 100 dispensing equipment only.
7. 3MTM 2- Component Foam Roof Tile Adhesive AH -160 shall not be exposed permanently to sunlight.
8. Tiles must be adhered in freshly applied adhesive. Tile must be set within 2 to 3 minutes after 3MTM 2-
Component Foam Roof Tile Adhesive AH -160 has been dispensed.
9. 3MTM 2- Component Foam Roof Tile Adhesive AH -160 placement and minimum patty weight shall be in
accordance with the Placement Details' herein. Each generic tile profile requires the specific placement noted
herein.

I recently received a price quotation from MJK Construction; they are the firm we use to handle all of our
strap work. According Michael Kulikowski, the owner of MJK Construction, the total cost (including
engineering) of installing the new straps around the perimeter of the Hollers' home would be $10,995.00.
The cost of the reroofing that we are performing is $51,000.00. Fifteen percent of that number is $7,650.00 -
well below what it would cost to redo the straps.
Mr. Heller would prefer not to spend the money to install the straps. My understanding, based on our
conversation and my reading of the statute, is that he is not required to do so.
