State Law requires construction to be done by a licensed contractor. You have applied for a permit under an exception to the law.The exemption allows you,as the owner of your property,to act as your own contractor even though you do not have a license. You must supervise the construction yourself. You may build or improve a one-family or two-family residence. You may also build or improve a commercial building at a cost of$25,000.00 or less(The new form states 75,000).The building must be for your own use and occupancy. It may not be built for sale or lease. If you sell or lease a building you have built yourself within one year after the construction is complete, the law will presume that you built for sale or lease, which is a violation of this exemption. You may not hire an unlicensed person as a contractor. It is your responsibility to make sure the people employed by you have licenses required by state law and by county or municipal licensing ordinances.Any person working on your building who is not licensed must work under your supervision and must be employed by you, which means that you must deduct F.I.C.A and with-holdings tax and provide workers'compensation for that employee, all as prescribed by law.Your construction must comply with all applicable laws, ordinances, buildings codes and zoning regulations. Please read and initial each paragraph. 1. 1 understand that state law requires construction to be done by a licensed contractor and have applied for an owner-builder permit under an exemption from the law.The exemption specifies that I,as the owner of the property listed, may act as my own contractor with certain restrictions even though I do not have a license. Initial 2. 1 understand that building permits are not required to be signed by a property owner unless he or she is responsible for the construction and is not hiring a licensed contractor to assume responsibility. Initial 3. 1 understand that,as an owner builder,I am the responsible party of record on a permit.I understand that I may protect myself from potential financial risk by hiring a licensed contractor and having the permit filed in his or her name instead of my own name.I also understand that the contractor is required by law to be licensed in Florida and to list his or license numbers on permits and contracts. Initial 4. 1 understand that I may build or improve a one family or two-family residence or a farm outbuilding.I may also build or improve a commercial building if the costs do not exceed$75,000.The building or residence must be for my use or occupancy. It may not be built or substantially improved for sale or lease. If a building or residence that I have built or substantially improved myself is sold or leased within 1 year after the construction is complete, the law will presume that I built or substantially improved it for sale or lease, which violates the exemption. Initial 11 S. I understand that,as the owner-builder,I must provide direct, onsite supervision of the construction. Initial 6. 1 understand that I may not hire an unlicensed person to act as my contractor or to supervise persons working on my building or residence. It is my responsibility to ensure that the persons whom I employ have the license required by law and by county or municipal ordinance. Initial_ 7. 1 understand that it is frequent practices of unlicensed persons to have the property owner obtain an owner-builder permit that erroneously implies that the property owner is providing his or her own labor and materials.I,as an owner-builder,may be held liable and subjected to serious financial risk for any injuries sustained by an unlicensed person or his or employees while working on my property. My homeowner's insurance may not provide coverage for those injuries. I am willfully acting as an owner-builder and am aware of the limits of my insurance coverage for injuries to workers on my property. Initial 8. 1 understand that I may not delegate the responsibility for supervising work to be a licensed contractor who is not licenses to perform the work being done.Any person working on my building who is not licensed must work under my direct supervision and must be employed by me,which means that I must comply with laws requiring the withholding of federal income tax and social security contributions under the Federal Insurance Contributions Act(FICA)and must provide workers compensation for the employee.I understand that my failure to follow these may subject to serious financial risk. Initial 9. 1 agree that,as the party legally and financially responsible for this proposed Construction activity,I will abide by all applicable laws and requirement that govern owner-builders as well as employers.I also understand that the Construction must comply withal[applicable laws, ordinances, building codes, and zoning regulations. Initial 10. 1 understand that I may obtain more information regarding my obligations as an employer from the Internal Revenue Service, the United States Small Business Administration, and the Florida Department of Revenues.I also understand that I may contact the Florida Construction Industry Licensing Board at 850.487.1395 or http://www.myfloridalicense.com/dbpr/pro/cilb/index.htmi Initial 11. 1 am aware of,and consent to;an owner-builder building permit applied for in my name and understands that I am the party legally and financially responsible for the proposed construction activity at the following address: 110 J%J ► AVJE4 pArA&4l gk3yi2$ n` Initial,[ 12. 1 agree to notify Miami Shores Village immediately of any additions, deletions,or changes to any of the information that I have provided on this disclosure. The diagrams and instructions outlined in this guide are for illustration purposes only and are not meant or implied to replace a licensed professional.Any construction or use of Pravol Dura-Shield must be in accordance with all local zoning and/or building codes. The ■ consumer assumes all risks and liability associated with the construction and use of this product. Safety: When dealing with any type of construction project it is necessary to wear appropriate safety equipment to avoid any risk of injuries. Pravol Dura-Shield recommends but is not limited to the following safety equipment when handling, cutting, and installing Pravol Dura-Shield:gloves, a respiratory protection, long sleeves, pants, and safety glasses. Tools: Standard woodworking tools may be used. It is recommended that all blades have a carbide tip.Standard stainless steel or acceptable coated deck screws and nails are recommended. Environment: A clean, smooth, flat, and strong surface is needed to install Dura-Shield Composite Decking correctly. Please check with local building codes before ever installing any type of decking. If installation does not occur immediately Dura-Shield Composite Decking needs to be put on a flat surface at all times. Never ever should it be put on a surface that isn't flat. Planning: Plan a layout for your deck before starting it to ensure the best possible looking deck for your project. Building codes and zoning ordinances generally apply to permanent structures, meaning anything that is anchored to the ground or attached to the house. So nearly every kind of deck requires permits and inspections from a local building department. We recommend drawing out a site plan of your proposed project that you intend to do to minimize errors and make your perfect deck. - Construction: Pravol Dura-Shield Composite Decking is NOT intended for use as columns, support posts, beams, joist stringers or other primary load- bearing members. Pravol Dura-Shield must be supported by a code-compliant substructure. While Pravol Dura-Shield pVfte%are great for deck re-planking (removing old deck surface planks and installing Pravol Dura-Shield on code-complia4substructure), Pr*avbl Dura-11IT18 V boards CANNOT be installed on existing deck boards. • • • Routing: .. . . .. ..... It must be installed onto a substructure, so there is adequate and unobstructed air flow under the deck to prevent excessive water absorption.A minimum of 6"of continuous net free area under the deck surface is required for adequate ventilation on all decking so air can circulate between adjacent members to promote drainage and drying. Heat and Fire Excessive heat on the surface of Pravol Dura-Shield Composite Decking from external sources such as but not limited to fire or reflection of sunlight from energy efficient window products, Low-emissivity (Low-E) glass can potentially harm Pravol Dura-Shield Composite Decking products. Low-E glass is designed to prevent passive heat gain within a structure and can cause unusual heat build-up on exterior surfaces. This extreme elevation of surface temperatures, which exceeds that of normal exposure, can possibly cause Pravol Dura-Shield Composite Decking products to melt, sag, warp, discolor, increase expansion/contraction,and accelerate weathering. Current or potential Pravol Dura-Shield Composite Decking customers that have concerns about passible damage by Low-E glass should contact BuildDirect if the product which contains Low-E glass for a solution to reduce or eliminate the effects of reflected sunlight. Fasteners When fastening Dura-Shield Composite Decking all screws that are face fastened should always be driven in at a 90 degree angle to the decking surface. Toe nailing/screwing should never been down to Dura-Shield Composite Decking. An extra joist should be added if a 90 degree angle cannot be driven into the board. Use white chalk, straight boards,or string lines as templates for straight lines.NEVER USE COLORED CHALK.Colored chalk will permanently stain Dura-Shield Composite Decking and is highly not recommended. Face fastening should follow the guidelines set out on page#4 of this installation guide. All nails/screws that are face fixed should always be stainless steel. Depending on the screws that you use when face fixing there could be potential bulging or mushrooming. It is recommended to take care of these mushrooms/bulges by taking a rubber mallet and patting them down to give your deck a better look. - When choosing which screws/nails to use always check first with your local home centers and hardware stores to see if they have screws that are engineered specifically for composite decking. These screws/nails will always work and give Dura-Shield Composite Decking the best looking outcome, using other screws/nails that are not recommended for composite could potentially damage/harm time deck. If you are 6 6 unsure which screw/nail to use contact your manufacturer for more information. ; 6•• •••• ••••0• ••66 000 • 6 Board Spacing • 666 •• 66 660006 Two boards should NEVER be installed on the same joist, when butting up the ends of the board they haveto to install"on separate joist's 6 60 66660 with separate clips. 6666 6 6 666606 000000 6666• 66 6• 66 6 6666•• 66.666 • 6 • 0 • • 0000•• 666.66 6 6 • 6 6 • • •66666 60 6 6 006 6 6 00.0 6 6 6 • TM For more information on this product or 19 BuildOl10111 M C 161- to order samples call 1-877-631-2845 or visit our website at buiiddirect.com Page 2 INSTALLATION P--- R A- VO!
