NEW WINDOW AND DOOR INSTALLATION CERTIFICATION.
Dear Behar Windows:
Pursuant to your request we performed an inspection on, Monday, June 20, 2016, to the above mentioned
address in order to verify the installation of the new impact windows and doors. The inspection consisted of(9)
horizontal roller windows (NOA#14-0515.05), (5) single hung windows (NOA#14-1212.03), (2) casement
windows(NOA#15-0406.10), (1)six panel(unglazed) impact French door(FL#11136.2),and(1)impact(glazed
French door(NOA#12-0402.03).
The windows installed into the precast concrete bucked openings have been installed per the onetime NOA
issued under the permit (WS-6-16-1600). The (2) above mentioned doors were installed per their respective
NOA's into typical concrete/masonry openings without precast concrete bucks.
We find that the windows have been installed into the precast concrete openings with the onetime NOA
specified anchor spacing, size, and locations and that the doors/windows without the precast buck condition
have been installed per the their respective product approvals.
The installation is in accordance with the approved plans,NOA's,and the 2014 Florida Building Code.

Building Department.Inspections performed by the Special Inspector hired by the owner are in addition to
the mandatory inspections performed by the department. Further,upon completion of work under each
Building Permit,I will submit to the Building Inspector at the time of the final inspection the completed
inspection log form and a sealed statement indicating that,to the best of my knowledge,belief and
professional judgment those portions of the project outlined above meet the intent of the Florida Building
