Property Location: 161 NE 105th St.Miami Shores,FL 33138
Folio Number: 11-2136-005-0140
Gentlemen:
In reference to the project located in the address mentioned above, I have visited the site and
based on a destructive test, a visual inspection and a rational analysis of the structure I hereby
attest that, to the best of my knowledge and professional judgment, the tie beam repair is in
compliance with the Florida Building Code 2014 edition and in accordance with the as-built
plans.
This certification verifies the following items:
• It was found a#5 rebar inserted approximately 3 inches deep into the existing tie beam
with a typical lap splice of approximately 36 inches. #3 ties were use at the splice. The
rebars and ties were found in good condition and in compliance with the Florida Building
Code 2014 edition.
• It was found a#5 rebar with a 90-degree angle inserted approximately 3 inches deep into
the existing concrete column with approximately 36 inches lap splice into the existing tie
beam. #3 ties were use at the splice section. The rebars and ties were found in good
condition and in compliance with the Florida Building Code 2014 edition.
• It was found that high strength epoxy grout was used to fill all gaps and holes. All dust,
debris, loose and spalled concrete was removed thoroughly by blast cleaning prior to
epoxy application.
Thank you in advance for your help in this process. If you require any additional information,
please contact me at my office.
Sincerely,
' `i0 .
