Dear Mr.Naranjo,
I refer to the above-referenced permit number.
I am writing to advise you that my contract with Casa Q(and its architect,Callum Gibb)has been terminated.
See the attached communication that I sent via certified mail to Casa Q on December 10, 2015 with a follow-up
Termination Letter on January 6, 2015. Each letter explains the basis for my terminating their services. Effective
January 21, 2015, 1 have not gotten any feedback from either Casa Q or Callum Gibb as it relates to them signing
MSV Change of Contractor/Architect Form.
With the assistance of Facemaker, I am now working with another architect to prepare the new plans and will
revert to Miami Shores Village upon completion.
To this end, please close this permit number 15-2595 referenced above and discontinue any further review of the
plans prepared by Callum Gibb and presented by Casa Q.

"Jackie" Gonzalez (hereinafter collectively referred to as"Pacemaker")to assist with permit,project coordination
and other related matters regarding the renovation of my home located at 1450 NE 103rd Street,Miami Shores,FL
33138("Property"). To this end,I authorize Facemaker to act on my behalf as it relates to interacting with Miami
Shores Village for permit related and other matters pertaining to the Property.
On a separate note, I also want to provide written notice that I plan to terminate my contract with Casa Q and the
architect, Callum Gibb Architect PA, that prepared the plans currently being reviewed by Miami Shores Village
under the above referenced permit number. I will submit the necessary paperwork for the Change of Contractor)
Architect over the next few days. In the meantime, however, I specifically request your office to put a hold any
further processing of the permit application and to discontinue any further review of the plans submitted under
