Workmen's Compensation insurance. Unless otherwise specified, Contractor's work carries a 5 year limited workmanship
warranty. If there are any persons, other than our employee(s), on the roof during the project or after the job is completed, any
warranty issued for the project will immediately become null and void without exception. The Contract Documents consist of this
proposal, Scope of Work Exhibit W, the terms and conditions, all documents referenced therein and the Limited Workmanship
Warranty and are incorporated herein by reference.

Permitting & Testing
■ Permitting and testing that is required to secure the roofing permit is included in our bid proposal.
■ Miami -Dade Product Notice of Acceptance for the proposed system detailing testing and
approval for all materials.
■ Notice of Commencement filing with the county and fees before the start of the project.
■ Notice of Termination will be filed at the end of the project upon final payment.
Low Slope Roof
• Cover and make reasonable effort to protect driveway, air conditioner, screen patio, and other
obstructions.
• Remove existing roof to a clean workable surface and dispose of debris.
• Replace rotten decking using CDX 19/32 plywood decking. Three sheet of decking (96 sq feet)
included in this proposal at no cost. Additional decking installed at $2.50 per square foot including
carpentry.
• Re -nail all decking on roof with 2.5 inch galvanized ring shank nails for homeowners insurance
discount included in this quote.
• Any rotten fascia board to be replaced with like kind and species. Fifty feet of fascia board
included at no additional charge. Additional fascia board installed at $8 per linear foot including
carpentry.
• Install new lead flashing around all plumbing vents with shop fabricated screens to keep rodents
out of building. Vents with be mechanically fastened and sealed with asphalt cement and fabric
membrane.
• Install new galvanized steel gooseneck vents with vermin screen over existing bath and kitchen
exhaust. Vents with be mechanically fastened and sealed with asphalt cement and fabric
membrane.
• Install 75# base sheet fastened to High Velocity Wind Zone requirements.
• Install 3° eave drip 26 gauge galvanized s_tpl. This metal available prefinished in white or brown

• All metal components and flashing that come in contact with asphalt will be primed for proper
adhesion of roofing felts.
• Install Ply 4 Fiberglass interply, (2) plies in full mopping of hot asphalt.
• Install Modified Cap Sheet set in full mopping of hot asphalt.
• Remove and reinstall roof tiles around flat roof and reseal with roof cement and underlayment.
• Hand broadcast white ceramic stones at all penetrations and asphalt bleed outs for neat
appearance and additional UV protection.
• Repair Drywall damage inside of the house by roof leak in comer flat roof, repair area and install
new drywall, reseal with drywall bond and install (1) coat of primer paint to repair area only.

Maintenance Agreement
• Having a maintenance plan in place is the best way to keep your roof looking its best and
performing over the long term. Regular maintenance can identify and eliminate issues before they
become problems. Well documented maintenance keeps your warranty in place and is invaluable
when making an insurance claim after a storm or other loss.
• Apply a sealer coating to all lead vent stacks, all-purpose vents, curb and wall base flashing laps
and drain sump areas.
• Apply a polyurethane sealant to the following sheet metal items as necessary: valley metal and
flashing.
• Visually inspect roof tiles for any loose or missing fasteners. If any fasteners are loose, they will
be tightened or replaced as necessary. Additional fasteners will be added where missing
fasteners are located.
• Provide owner with written inspection report and digital photos of deficiencies and corrective
action taken.
• Annual service charge $850.00 with pressure clean file roof included, First payment due when

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami -Dade County RER -
Product Control Section to be used in Miami Dade County and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product
Control Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County)
reserve the right to have this product or material tested for quality assurance purposes. If this product or
material fails to perform in the accepted manner, the manufacturer will incur the expense of such testing
and the AHJ may immediately revoke, modify, or suspend the use of such product or material within their
jurisdiction. RER reserves the right to revoke this acceptance, if it is determined by Miami -Dade County
Product Control Section that this product or material fails to meet the requirements of the applicable
building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.
DESCRIPTION: GAF Conventional Built -Up Roof Systems for Wood Decks.
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in
the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then
it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.

WOOD DECK SYSTEM LIlVIITATIONS.
1 A slip sheet is required with GAFGLAS® Ply 4 and GAFGLAS® Flex Ply' 6 when used as a mechanically fastened
base or anchor sheet.
2. Minimum 1/4" DensDeck7 Roof Board or %2" Type X gypsum board is acceptable to be installed directly over the
wood deck
GENERAL LmTATIONS:
1. Fire classification is not part of this acceptance; refer to a current Approved Roofing Materials Directory for fire
ratings of this product.
2. Insulation may be installed in multiple layers. The first layer shall be attached in compliance with Product Control
Approval guidelines. All other layers shall be adhered in a full mopping of approved asphalt applied within the
EVT range and at a rate of 20-40 lbs./sq., or mechanically attached using the fastening pattern of the top layer
3. All standard panel sizes are acceptable for mechanical attachment. When applied in approved asphalt, panel size
shall be 4'x 4' maximum.
4. An overlay and/or recovery board insulation panel is required on all applications over closed cell foam insulations
when the base sheet is fully mopped. If no recovery board is used the base sheet shall be applied using spot
mopping with approved asphalt, 12" diameter circles, 24" o.c.; or strip mopped 8" ribbons in three rows, one at
each sidelap and one down the center of the sheet allowing a continuous area of ventilation. Encircling of the
strips is not acceptable. A 6" break shall be placed every 12' in each ribbon to allow cross ventilation. Asphalt
application of either system shall be at a minimum rate of 12 lbs./sq. Note: Spot attached systems shall be
limited to a maximum design pressure of 45 psf.
5. Fastener spacing for insulation attachment is based on a Minimum Characteristic Force (F') value of 275 lbf., as
tested in compliance with Testing Application Standard TAS 105. If the fastener value, as field-tested, are below
275 lbf insulation attachment shall not be acceptable.
6. Fastener spacing for mechanical attachment of anchor/base sheet or membrane attachment is based on a minimum
fastener resistance value in conjunction with the maximum design value listed within a specific system. Should the
fastener resistance be less than that required, as determined by the Building Official, a revised fastener spacing,
prepared, signed and sealed by a Florida Registered Professional Engineer, Registered Architect, or Registered
Roof Consultant may be submitted. Said revised fastener spacing shall utilize the withdrawal resistance value
taken from Testing Application Standards TAS 105 and calculations in compliance with Roofing Application
Standard RAS 117.
7. Perimeter and corner areas shall comply with the enhanced uplift pressure requirements of these areas. Fastener
densities shall be increased for both insulation and base sheet as calculated in compliance with Roofing Application
Standard RAS 117. Calculations prepared, signed and sealed by a Florida registered Professional Engineer,
Registered Architect, or Registered Roof Consultant (When this limitation is specifically referred within
this NOA, General Limitation #9 will not be applicable.)
8. All attachment and sizing of perimeter nailers, metal profile, and/or flashing termination designs shall conform to
Roofing Application Standard RAS 111 and applicable wind load requirements.
9. The maximum designed pressure limitation listed shall be applicable to all roof pressure zones (i.e. field,
perimeters, and corners). Neither rational analysis, nor extrapolation shall be permitted for enhanced fastening at
enhanced pressure zones (i.e. perimeters, extended corners and corners). (When this limitation is specifically
referred within this NOA, General Limitation #7 will not be applicable.)
10. All products listed herein shall have a quality assurance audit in accordance with the Florida Building Code and
Rule 61 G20-3 of the Florida Administrative Code.

HIGH VELOCITY HURRICANE ZONES REQUIRED OWNERS NOTIFICATION FOR ROOFING
CONSIDERATIONS
1524.1 As it pertains to this section, it is the responsibility of the roofing contractor to provide the owner with the
required roofing permit, and -to explain to the owner the content of this section.. The provisions of Chapter 15 of
the Florida Building Code, Building govern the minimum requirements and standards of the industry for roofing
system installations. Additionally, the following items should be addressed as part of the agreement between the
owner and the contractor. The owner's initial in the adjacent box indicates that the item has been explained.
1. Aesthetics -Workmanship: The workmanship provisions of Chapter 15 (High Velocity Hurricane Zone)
are for the purpose of providing that the roofing system meets the wind resistance and water intrusion
performance standards. Aesthetics (appearance) issues are not a consideration with respect to workmanship
provisions. Aesthetic issues such as color or architectural appearance, that are not part of a zoning code, should
be addressed as part of the agreement between the owner and the contractor.
$tLL2. Renailing Wood Decks: When replacing roofing, the existing wood roof deck may have to be renailed
in accordance with the current provisions of Chapter 16 (High Velocity Hurricane Zones) of the Florida Building
Code. (The roof deck is usually concealed prior to removing the existing roof system).
<. Common Roofs: Common roofs are those which have no visible delineation between neighboring
units (i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing contractor and/or owner
should notify the occupants of adjacent units of roofing work to be performed.
4. Exposed Ceilings: Exposed, open beam ceilings are where the underside of the roof decking can be
vi ed from below. The owner may wish to maintain the architectural appearance, therefore, roofing nail
penetrations of the underside of the decking may not be acceptable. The Florida Building Code provides the
option of maintaining this appearance.
5. Ponding Water: The current roof system and/or deck of the building may not drain well and may
cause water to pond (accumulate) in low-lying areas of the roof. Ponding can be an indication of structural
distress and may require the review of a professional structural engineer. Ponding may shorlign'the life
expectancy and performance of the new roofing system. Ponding conditions may not be:qvid8rit until the original ••,
