Survey Report – "Library Fire Alarm Upgrade", Barry

Advanced Industrial Hygiene Services, Inc. is pleased to submit the following report for a pre -
renovation survey for asbestos -containing building materials (ACBM) conducted January 23,
2014 in the above -referenced facility. This survey was performed to meet the current NESHAP
regulations and Miami -Dade County Department of Environmental Resource Management
(DERM) prior to conducting the planned renovation activity. The name for this project is
"Library Fire Alarm Upgrade Renovation".
Facility Description & Project Scope
The subject facility consists of a four story institutional (library) building. Structural components
include a sub -grade concrete foundation, precast concrete "honey -comb” upper floor decks and
concrete masonry unit (CMU) wall systems. Interior floor finishes include institutional carpet
and vinyl composition tiles. Interior walls are composed of CMU and painted stucco. Ceiling
systems consist of exposed grid acoustic tiles. The subject facility has stacked HVAC
mechanical rooms on each of the four floors with a large fan coil unit in each with associated
insulated pipes and supply and return duct systems. The duct systems are located above the
suspended acoustic ceiling grid and are composed of sheet metal. Supply ducts have insulation
on the exterior. Return ducts are not insulated.
The scope of this project was to sample and identify any ACBM that may be present in the
facility that may require special treatment prior to or during the planned renovation.

Sampling Strategy

Bulk material sampling was conducted according to the following sampling plan:
Friable Surfacing Materials:
Friable surfacing materials, where encountered, are sampled based on the recommendations
found in the EPA "Pink Book" entitled "Asbestos in Buildings: Simplified Scheme for Friable
Surfacing Materials" (EPA document 560/5-85-030a). The number of samples collected is based
on the total square footage of the homogenous area of the material. If the area is between 100
and 1000 f, three samples are collected If the area is between 1000 and 5000 i, five samples
are collected. If the area is between 5000 fe and 10,000 fl , seven samples are collected. Nine
samples are collected if the area is greater and 10,000 IFO.
Thermal System Insulation (TSI):
If present, at least three samples are collected in a randomly distributed manner from each
homogeneous area of TSI not assumed to be ACM. At least two samples are collected from each
homogeneous area of patched TSI. Where cement or plaster is used on fittings such as tees,
elbows, or valves, samples were collected in a manner sufficient to determine whether the
material is ACM or not ACM.
Miscellaneous Materials.-
These
aterials:These include materials such as acoustical ceiling tiles, floor tiles and linoleum, wall board, wire
insulation, caulking and sealants, laboratory equipment, furniture, draperies, etc. (basically,
anything that does not fall into the first two categories). Suspect miscellaneous materials are
sampled in such a manner as to determine whether or not they contained asbestos. The number of
samples collected of a given miscellaneous material is left to the discretion of the inspector. As a
general rule however, the number of samples collected of most friable miscellaneous materials is
based on the same scheme as that used for friable surfacing materials.
All accessible areas of the renovation area were inspected Bulk sample specimens of materials
suspected of containing asbestos were collected and submitted for analysis by Polarized Light
Microscopy in conjunction with dispersion staining as outlined in 40 CFR, Part 63, Subpart F
dated January 1987. Analysis was conducted by Advanced Industrial Hygiene Services, Inc. who
is accredited for asbestos fiber analysis through successful participation in the National
Voluntary Laboratory Accreditation Program (NVLAP) and meets the requirements of section
206(d) of Title lI of the USC Chapter 15, TSCA as stated in 40 CFR 763 dated April 30, 1987.

report.

Those suspect asbestos -containing materials that were present and expected to be removed
and/or disturbed as a part of this project are listed below:
Flooring Materials:
Floor substrate consists of concrete. It is AIRS' understanding that locations of planned floor
penetrations do not have floor finish material(s). Representative samples were collected of the
concrete floor system material and all were reported by the laboratory to contain no detectable
quantities of asbestos.
Wall Materials:
Wall materials consist of concrete masonry units (CMU) finished with stucco/plaster and paint.
The wall system surrounding the elevator shaft is composed of firebrick and mortar.
Representative samples were collected of the wall system materials and all were reported by the
laboratory to contain no detectable quantities of asbestos.
Ceiling Materials:
Ceiling materials consist of two types of 2 X 2 fl exposed grid acoustic tiles. Representative
samples were collected of both types of the ceiling tiles and all were reported by the laboratory
to contain no detectable quantities of asbestos.
Heating, Ventilation and Air -Conditioning (HVAC) System:
HVAC system components in the renovation area consist of sheet metal ducts. Supply ducts have
exterior insulation wrap sealed with black mastic. Return ducts are not insulation but are sealed
with light tan mastic. Previous sampling has determined that the black mastic material on the
insulated ducts contains asbestos fibers.
Surfacing Materials:
No spray -applied surfacing materials were observed in areas affected by the renovation as
described.
Thermal System Insulation (TSI) Materials:
No TSI materials are scheduled affected by renovation as described However, previous
sampling has determined that the insulated pipe elbows and fittings in the HVAC mechanical
rooms contain asbestos fibers.
Additional Miscellaneous Materials:
Additional miscellaneous suspect asbestos -containing materials potentially affected by the
planned renovation consisted of insulation material on top of the fluorescent light fixtures
throughout the facility. Representative samples were collected of the insulation material and all
were reported by the laboratory to contain no detectable quantities of asbestos.

inspection report.
AIRS, Inc. is licensed to conduct inspections for asbestos -containing materials in buildings under
an Asbestos Consultants Business License (ZA 0000058) and is qualified by Bruce Marchette,
CIH (Asbestos Consultants License # IA 0000041).
Closing Remarks
This limited pre -renovation survey for ACBM is intended for the sole use of Barry University.
The scope of services performed in execution of this investigation may not be appropriate to
satisfy the needs of other users, and any use or re -use of this document or the findings,
conclusions, or recommendations presented herein is at the sole risk of said user. Opinions and
recommendations presented herein apply to site conditions existing at the time of our
investigation. They cannot necessarily apply to site changes of which this office is unaware and
has not yet had the opportunity to evaluate.
The contractor performing the renovation work must be notified of the presence of asbestos
containing materials (ACM) and presumed asbestos containing materials (PACM) in the facility

representative's office (The Department of Environmental Resource Management, 701 NW 1st

the onset of the project.
Disclaimer
This inspection report is the result of a diligent search of the renovation areas for asbestos -
containing building materials. The purpose of this inspection was to identify asbestos -containing
materials that may require special treatment prior to proceeding with the planned renovation
operation. Because this inspection was conducted prior to a planned renovation operation, only
those suspect asbestos -containing materials expected to be disturbed as a result of the renovation
activities were sampled and submitted to the laboratory for asbestos content. However
comprehensive this inspection appears, it does not claim to have identified all of the asbestos -
containing materials present in the facility.
