materials. The documentation submitted has been reviewed and accepted by Miami &� a Cp0 R R«0
Product Control Section to be used in Miami Dade County and other areas where allooad by�he Autllq�ty
Having Jurisdiction (AHJ).

This NOA shall not be valid after the expiration date stated below. The Miami Dade County Product
Control Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County)
reserve the right to have this product or material tested for quality assurance purposes. If this product or
material fails to perform in the accepted manner, the manufacturer will incur the expense of such testing
and the AHJ may immediately revoke, modify, or suspend the use of such product or material within their
jurisdiction. RER reserves the right to revoke this acceptance, if it is determined by Miami Dade County
Product Control Section that this product or material fails to meet the requirements of the applicable
building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.
DESCRIPTION: GAF Conventional Built -Up Roof Systems for Wood Decks.
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami Dade County Product Control Approved", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERNMATION of this NOA will occur after the expiration date or if there has been a revision or change in
the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then
it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.
This NOA renews and revises NOA No. 13-0424.09 and consists of pages 1 through 16.

1 A slip sheet is required with GAFGLAS® Ply 4 and GAFGLASo Flex Ply"' 6 when used as a mechanically fastened
base or anchor sheet.

Fastener spacing for insulation attachment is based on a Minimum Characteristic Force (F) value of 275 lbf., as
tested in compliance with Testing Application Standard TAS 105. If the fastener value, as field-tested, are below
2751bf. insulation attachment shall not be acceptable.
Fastener spacing for mechanical attachment of anchor/base sheet or membrane attachment is based on a minimum
fastener resistance value in conjunction with the maximum design value listed within a specific system. Should the
fastener resistance be less than that required, as determined by the Building Official, a revised fastener spacing,
prepared, signed and sealed by a Florida Registered Professional Engineer, Registered Architect, or Registered
Roof Consultant may be submitted. Said revised fastener spacing shall utilize the withdrawal resistance value
taken from Testing Application Standards TAS 105 and calculations in compliance with Roofing Application
Standard RAS 117.
Perimeter and corner areas shall comply with the enhanced uplift pressure requirements of these areas. Fastener
densities shall be increased for both insulation and base sheet as calculated in compliance with Roofing Application
Standard RAS 117. Calculations prepared, signed and sealed by a Florida registered Professional Engineer,
Registered Architect, or Registered Roof Consultant (When this limitation is specifically referred within

All attachment and sizing of perimeter nailers, metal profile, and/or flashing termination designs shall conform to
Roofing Application Standard RAS 111 and applicable wind load requirements.

2. Renailing wood decks: When replacing roofing, the existing wood roof deck may haft'M be
renailed in accordance with the current provisions of Chapter 16 (High Velocity Hurricane Zones) of the Florida
BuildingCode. (The roof deck is usually concealed prior to removing the existing roof system).

3. Common roofs: Common roofs are those which have no visible delineation between neighboring
units (i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing contractor and/or
owner should notify the occupants of adjacent units of roofing work to be performed.
4. Exposed ceilings: Exposed, open beam ceilings are where the underside of the roof decking can be
viewed from below. The owner may wish to maintain the architectural appearance; therefore, roofing nail
penetrations of the underside of the decking may not be acceptable. The owner provides the option of
maintaining this appearance.
A5. Ponding water: The current roof system and/or deck of the building may not drain well and may
cause water to pond (accumulate) in low-lying areas of the roof. Ponding can be an indication of structural
distress and may require the review of a professional structural engineer. Ponding may shorten the life
expectancy and performance of the new roofing system. Ponding conditions may not be evident until the original

uppers (wall outlets): It is required that rainwater flow off so that the roof is not
overloaded from a build up of water. Perimeter/edge walls or other roof extensions may block this discharge if

Notice to Owner - Workers' Compensation Insurance Exemption
Florida Law requires Workers' Compensation insurance coverage under Chapter 440. of the Florida Statutes. Fla. Stat. § 440.05
allows corporate officers in the construction industry to exempt themselves from this requirement for any construction project
prior to obtaining a building permit. Pursuant to the Florida Division of Workers' Compensation Employer Facts Brochure:
An employer in the construction industry who employs one or more part-time or full-time
employees, including the owner, must obtain workers' compensation coverage. Corporate
officers or members of a limited liability company (LLC) in the construction industry may
elect to be exempt if:
1. The officer owns at least 10 percent of the stock of the corporation, or in the case
of an LLC, a statement attesting to the minimum 10 percent ownership;
2. The officer is listed as an officer of the corporation in the records of the Florida
Department of State, Division of Corporations; and
3. The corporation is registered and listed as active with the Florida Department of
State, Division of Corporations.
No more than three corporate officers per corporation or limited liability company members
are allowed to be exempt. Construction exemptions are valid for a period of two years or until
a voluntary revocation is filed or the exemption is revoked by the Division.
Your contractor is requesting a permit under this workers' compensation exemption. In these circumstances, Miami Shores Village
does not require verification of workers' compensation insurance coverage from the contractor's company. Therefore, you may be
personallyliable for the worker com enation in'uries of an person allowed to work under this ermit. Please check with your
insurance carrier since most property insurance policies DO NOT cover this type of liability.
