LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state,

noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been
no change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change
in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement
of any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure
to comply with any section of this NOA shall be cause for termination and removal of NOA.

followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is

Here's information YOU need to know about the "! Rule".
If your home or business is below the 100-year flood elevation, also known as the Base
Flood Elevation (BFE), FEMA has flood damage prevention regulations that may affect how
you remodel, renovate, or add on to your building. If your home or business sustained
structural and/or interior damage, these regulations may affect how you rebuild.' These laws
are required by the National Flood Insurance Program to protect lives and investment from
future ° # a a damages, Miami Shores Village is required to adopt and enforce these laws in
order for federally - backed flood insurance to be made available to we residents and
property owners.

PLEASE READ THE FOLLOWING INFORMATION:
SUBSTANTIAL DAMAGE means damage of any origin sustained by a structure whereby the
cost of restoring the structure to it's before damage .• ition would equal or exceed 50
percent of the market value or replacement cost of the structure before the damage
occurred. (Note: The cost of the repairs must include all costs necessary to fully repair the
structure to its before damage" condition.)
SU TANTIAL IMPROVEMENT means any reconstruction, rehabilitation, addition, or other
improvement of a structure, the cost of which equals or exceeds 50 percent of the market
value of the structure before the "start of construction" of the improvement. If a building is
"substantially damaged" or "substantially improved", it must be brought into compliance with
the flood damage prevention regulations, including elevating the building to or above the
100 -year flood elevation.
Miami S =. Village, follows the National Flood Insurance Program requirements, and has
the responsibility to determine "substantial damage" and "substantial improvement ", and
has implemented the procedures on the following pages to do so.

NOTICE TO PROPER T' OWNERS
Miami Shores Village will use the assessed value of your structure (excluding the land) recorded by the
Property Appraiser's Office. If you disagree with the Property Appraiser's valuation of the structure, you
may engage a property appraiser licensed by the State of Florida to submit a comparable property
appraisal for the lmarket value of the structure.
You must obtain and submit to the Building Department a detailed and complete cost estimate for the
addition, remodeling, reconstruction or for re. = ^ of all the damages sustained by your home, prepared
and signed by a licensed general contractor. The contractor must sign an vit indicating
tiat
the cost
estimate submitted includes all damages or all improvements to your home, not just structure . The signed
contract document must be submitted with your application, If the owner is acting as his or he own
contractor, the owner is responsible for submitting the cost estimate, and providing documentation,
including subcontractor bids, to document the cost estimate.
