1. We have included fumishing and installing:
a. Main line safety switch with rejection Gips and RK5 fuse. (Size of disconnect is based on exsting Motor sizes)
b. New cab car light safety switches.
c. Code compliance machine room light fixtures with protective covers
d. GFI receptacles In machine room.
e. GFI receptacles in eacti'pit:
f. New hoist way lights in each pit.
g. Phone Conduit with wire only
h. Dedicated earth grounds per elevator machine room
I. Air Condition connection for elevator designated for elevator on emergency generator
j. Lowering relay contact for two (2) elevators
k. Permit provided with this proposal (please allow 4 weeks after the owner information is submitted for permitting)
I. Price is Base on re- using main line feeders. (Based on existing motor sizes)
m. Proposal is base on information to us from the building any work beyond the scope of this proposal will have
additional cost.
2. Not included in this proposal is the following:
a. fire alarm
b. shunt trip relays connected to the existing fire alarm
c. Not responsible for eAsting code violation.
3. All work is to be done on regular working hours between 8:00 AM to 4:30 PM excluding holidays -and overtime. included is one
(1) night shut down of elevators.
Price for the work or service performed:
Nine thousand eight hundred ninety seven Dollars and no Cent $ 9,897.00
Adder price for One (1) 1 ton mini splits A/C w /duct work from existing A/C duct $ 5,1.57.00
All additional work performed shall be paid for at the rate of $_83.04 per regular working hour. Additional
materials used in the work shall be paid for at our normal rates.

All payments shall be due in accordance with the terms described above. Customer agrees to pay all court costs and attorneys fees
