The total depth of hole #5 was 27 ft., pile was tested with a 10000 psi pump machine but it did not move. On hole #3, east elevation a metal deck was found at 5 feet depth, that metal deck was cut in order to continue with the penetration of piles Pile Hole #3, was monitored by the installer. Pursuant to your authorization, All State Engineering & Testing Consultants, Inc. (ASETC) conducted a subsurface investigation at the above referenced project. The investigation was performed on May 24, 2010. The purpose of the investigation was to develop preliminary information about the site and the subsurface conditions existing in the vicinity of the proposed construction. To achieve the desired objective one (1) standard penetration test boring was performed and the log is enclosed in this report. TEST METHOD: The borings were conducted in accordance with procedures outlined for standard penetrations test and split spoon sampling of soils by ASTM Method D -1586. A two (2) foot long two (2) inches O.D. Split Spoon Sampler was driven into the ground by successive blows with 140 Ib. Hammer dropping thirty (30) inches. The soil sampler was driven two (2) feet at a time then extracted for visual examination and classification of the retained soil samples. The number of blows required for a one (1) foot penetration of the sampler is designated as "N" (known as the standard penetration resistance value). The "N" value provides an indication of the relative density of non - cohesive soils and the consistency of cohesive soils. Suitable corrections are applied to this number in order to include the effects of soil overburden pressure and other factors. A general evaluation of soils is made from the established correlation between "N ",and the relative density or consistency of soils. This dynamic method of soil testing has been widely accepted by foundation engineers and architects to conservatively evaluate the bearing capacity of soils. A continuous drilling and sampling procedures was used therefore, the Samples were taken at intervals of two (2) feet or at every change in soil characteristics. The types of foundation material encountered have been visually classified and are described in detail in the boring logs. The results of the field penetration tests are presented in the boring logs in numerical forms. The average ground water level at the site was found at five (5) feet six (6) Inches below the existing surface (see logs). Fluctuation in the observed ground water level should be expected due to seasonal climatic changes, rainfall variation, surface water run -off and other specific factors related to the site in question. 2380 West 78th Street ! It was observed that boring #1 shows loose strata in a form of cavity from 5' -0" to 6'-0 ", from 6' -O" to 14' -0 ", from 18'-0" to 24' -0" and from 26' -0" to 32' -0". It is suggested that the building needs to be underpinned by one of the following method: 1. UNDERPINNING BY PRESSURE GROUTING: It is our professional opinion that high pressure epoxy or grouting must be pumped through 2" diameter holes approximately 6' -0" in depth of the affected areas in order to strengthen the sub -soil condition. It Is also suggested that our representative should monitor the entire operation to determine the exact volume of grouting and the composition of water - cement ratio. ASETC will then issue a Certification of Grouting Operation upon completion. 2. UNDERPINNING BY HELICAL PIER: Helical piles are another feasible alternative. The capacity of these piles is essentially developed in tip bearing and side friction. When these piles are installed or socketed into place, they will lock into the rock and sand strata thereby providing adequ to bearing capacity. The analysis for this foundation alternative consisted of determining a pfe capacity for specific size and depth of installation. The entire pile driving (installation) operation should be properly monitored and perfomx:d in accordance with the relevant local and state requirements. CONCLUSION: According to the U.S. Department of Agriculture and Soil Conservation Service the soil survey of Miami -Dade County, Florida discloses that the soils at the Encamacion property are classified as Dade fine sand, Rockdale fine sandy loam (limestone complex), and limestone substratum -urban land complex. These soils formed in sandy marine sediment over limestone. Sinkholes originate beneath the surface when water migrates through the limestone and creates large voids or cavities in the bisdrock. The dissolving limestone is replaced by sand granules that fall into the depressions. The boring Togs show variations in the "N" values which disclose a downward migration of soils into voids and fractures within the carbonate rock. These solution holes (sinkhole activity) result in the settlement of the surficial soils. The sinkhole activity is causing surface depressions and subsequent damage to the building. Regardless of the thoroughness of our geotechnical exploration there is always a possibility that conditions on the subject property (site) may be different from those at the test locations. INTRODUCTION On April 8, 2010, Ms. Lori Mehall of Nationwide Insurance Company retained ProNet Group, Inc. to inspect the Encamacion property, Dated at 1125 NE 92nd Street in Miami Shores, Florida. The purpose of our assessment was to determine the origin and cause of cracks on the interior and exterior finishes of the residence. II. BASIS OF REPORT This report is based on the following: • Interview with the homeowner, Ms. Shirley Encamacion. • Information regarding the history of the residence obtained on the Miami -Dade County Property Appraisers website. • A relative elevation survey of the interior floor surface of the residence using a Technidea Zip Level ®. • Pertinent dimensions and photographs of the structure and reported damages in particular. • Review of the soil survey maps and data published by the United States Department of Agriculture's Natural Resources Conservation Service for shallow soil properties in the geographical area of the residence. PROPERTY DESCRIPTION The residence is a two -story structure constructed with CMU exterior walls covered with a painted stucco facade and brick veneer with a living /dining room and kitchen supported on a CMU stem -wall foundation with a structural, raised wood floor /beam system. There is a concrete slab -on -grade foundation at the attached garage, bedroom No. 1, hallway bathroom and den. The second floor areas of the residence are located above the garage, bedroom No.1 and hallway bathroom. The residence has a hip -type roof system covered with low- profile concrete roof tiles. An attached covered patio and in -ground swimming pool is at the back area of the structure. A storage building is situated at the right side back area of the property. The floor elevation of the residence is slightly higher than the street elevation and the primary drainage for the property is the street and side yards. For the purposes of this report, the front of the residence faces primarily to the south. IV. CONCLUSIONS Based on our findings and observations as noted in this report, it is our technical opinion that the horizontal /vertical /stepped cracks in the interior and exterior finishes resulted from differential foundation movement from the changes in the soil moisture content of the supporting soils that occurred under nominal environmental (sun and April 29, 2010 Page 4 Pro Net File No. 14142 rain) and seasonal (wet and dry cycles) factors occurring through the years, adverse soil behavior, improper yard drainage, and improper roof rainwater discharge in conjunction with the aging of the residence. V. DISCUSSION INTERVIEW The following information was obtained during our interview with homeowner: • The residence is approximately 51 years old. • They have lived in the residence for approximately 20 years. • The den and swimming pool were constructed before they purchased the residence. • Navigable waterway is located less than one - quarter mile west of the residence. • Mrs. Encamacion initially noticed exterior cracks in the east CMU wall and the south (front) brick veneer a few years ago and she noticed interior drywall cracks in the living /dining room walls and ceiling in early 2010. OBSERVATIONS On April 15, 2010, Brendan M. Larkin, P.E. of Pro Net Group, Inc., in the presence of the homeowner inspected the exterior, interior and crawlspace of the residence. General April 29, 2010 Page 5 ProNet Ale No. 14142 dimensions and photographs were taken to document our observations. This report identifies and documents the representative damages and conditions at the residence and reflects typical conditions observed during our inspection. Also, a single photograph may be included to depict the damage in a particular area. All photographs are available for review and some of the photographs taken are included in this report. Following are some of the pertinent observations made during our inspection at the Encamacion residence: Exterior • No gutter /downspout system installed at the residence and roof rainwater discharges at the perimeter of the residence; • Several horizontal /vertical /stepped cracks in the east CMU wall and the south brick veneer /CMU wall; and, • Five air ventilation openings for the crawlspace area approximately sixteen inches (16 ") long by eight inches (8 ") high all located on the east wall, at or slightly above exterior grade level. Two ventilation openings have a CMU block place in front of it, blocking air flow and the other ventilation openings have screen /fabric over the openings. One ventilation opening located on the south stem wall is completely sealed off. Interior • The garage floor, bedroom No. 1 and hall bathroom are constructed on a slab - on -grade foundation; and, • Several drywall horizontal /vertical /stepped cracks located on the living room east and south walls and ceiling. Crawlspace • Soil consists of sand and small /medium rocks and remnants of concrete debris and concrete over pour; April 29, 2010 Page 6 Pro Net File No. 14142 • Crawlspace soil level is approximately twelve inches (12 ") lower than the exterior grade level; • No vapor retarder installed over the soil; • Soil damp to the touch; • An approximately four foot by four foot by one foot (4'x4'x1') deep cavity at the southeast comer of the crawlspace area with two voids traveling approximately one foot (1') beneath the east and south stem walls at the southeast comer area of the foundation of the residence; • Approximately one inch to three inches (1 " -3 ") of voids and /or soil subsidence between soil and concrete debris /over foundation pour at north and northeast stem wall areas; and, • Water staining and evidence of water intrusion, soil erosion and soil subsidence at the CMU stem walls at the south and east stem wall areas. REVIEW OF USDA — NRCS SOIL MAPS The shallow soils in the area of the Encamacion residence have been mapped by the U.S. Department of Agriculture — Natural Resources Conservation Service. USDA soil maps are based upon aerial surveys and shallow test pit excavations/hand auger borings and are not necessarily an exact representation of in situ soil conditions on a specific site. The Encamacion residence is located in an area mapped as soil type: Udorthents, limestone substratum -Urban land complex (42), located in areas with 0 to 5 percent slopes, moderately well drained, and generally consisting of gravelly sand (SP, SP -SM) to a depth of thirty inches (30 ") over un- weathered bedrock to the interpreted depth of thirty -four inches (34 ") below grade. The depth to groundwater is listed as twenty -four inches to forty-eight (24" to 48 ") below grade. April 29, 2010 Page 7 Pro Net File No. 14142 ELEVATION SURVEY A micro - elevation survey was performed on the floors within the garage, bedroom No. 1, hallway, hall bathroom, living /dining room and kitchen area of the Encamadon residence using a Technidea Zp Level ® in order to evaluate the magnitude, location and general direction of differential floor slopes that may have occurred since original construction. The data obtained is used to determine the amount of relative differential movement from one location to another within the areas surveyed. The elevations taken are corrected for different floor coverings in order to evaluate the relative elevation differences using a single datum. A benchmark elevation of zero inches (0") was established outside the main entrance at the concrete sidewalk. The results of the elevation survey of the kitchen and living /dining room indicate that the high area of the residence floor slopes approximately one and one -half inches (11/2 ") from the slab -on -grade foundation, west kitchen /living floor area to approximately a twenty foot (20') long, low area at the CMU pier and wood beam supported area of the living /dining room, approximately five feet to twenty -five feet (5' to 25') away. This low area is located under the shared south kitchen wall /north firing room wall in the vicinity of the CMU stem -wall that travels in an east/west direction and supports the wood floor beams that travel in a north /south direction. The results of the elevation survey in the living room indicate that the high area of the residence floor slopes approximately one inch (1 ") from the slab -on -grade foundation, west kitchen /living floor areas to a low area at the southeast living room comer area approximately twenty-five feet (25') away. The garage, bedroom No. 1, hall and hall bathroom are supported on a slab -on -grade foundation. The results of the elevation survey indicate that the high area of the residence floor slopes approximately one -half inch (1/2 ") from the east slab -on -grade April 29, 2010 Page 8 Pro Net File No. 14142 foundation hall and hall bathroom areas and generally slopes down toward the northwest area of bedroom No. 1 approximately twenty -five feet (25') away. A separate relative elevation survey was performed on the exterior area of the Encamadon residence using a Technidea Tip Level® in order to evaluate the slope and direction of rain water flow at the front and east side yards of the residence. A benchmark elevation of zero inches (0") was established outside the main entrance at the concrete sidewalk. The results of the exterior elevation survey indicate that the high area of the north half of the front yard, east of the driveway, slopes approximately twelve inches (12 ") to a low area at the exterior northeast living /dining room area approximately forty-five feet (45') away. The rain water flow passes at or near the five air ventilation openings for the crawlspace that are located at or slightly above exterior grade level. ANALYSIS Soil consolidation (compaction) is a natural reaction of the soil to the addition of load. Soil consolidation is rarely completely uniform beneath a structure or soil supported floor slabs and a certain amount of differential movement is considered normal. The magnitude of floor slope measured in the garage of the Encamacion residence, however, exceeds what is considered normal settlement. The fact that the central area of the living /dining room has settled approximately one and one -half inches (11/2 ") and the southeast perimeter of the living room has settled approximately one inch (1 ") indicate that the overburden soils are subsiding. The four foot by four foot by one foot (4'x4'x1') deep cavity at the southeast comer of the crawlspace area with two voids traveling beneath the stem wall foundation at the south and east areas of the foundation of the residence indicate that consolidation /erosion of the natural soils and surface subsidence has been a long -term condition. April 29, 2010 Page 9 Pro Net File No. 14142 There are five air ventilation openings for the crawlspace area at or slightly above the exterior grade level. The CMU stem wall and soil in the vicinity of the five air ventilation openings evidence water intrusion, soil erosion, soil consolidation and soil subsidence. These conditions are the result of lack of gutters at the Encamacion residence; the rain water not being directed away from the south and east foundation area, the crawlspace soil level approximately twelve inches (12 ") lower than the exterior grade level and air - ventilation openings on the east wall at or near grade level. The measured floor slopes and cracks in the CMU /brick facade walls are consistent with stem wall foundation settlement relative to the slab -on -grade foundation. Evidence of erosion and subsidence in the crawlspace along the north, east and south stem walls indicate that settlement of the residence has been a long -term condition and is not the result of any one -time event. The cracks in the CMU /brick veneer walls have been Tong -term conditions that have been occurring for years. The ventilation openings located at the east perimeter do not meet the Florida Building Code requirements for crawlspace ventilation of one square foot of vent openings for each one - hundred fifty square feet (150 sf) of crawlspace area. The ventilation around the perimeter of the residence is approximately forty percent (40 %) of what is required by code; however, all of the ventilation openings are located on the east face of the residence, which does not allow for cross ventilation. Shallow groundwater conditions in the Miami area result in a constant source of moisture and moist soil/high relative humidity conditions beneath an elevated residence. In addition, the high water table characteristics of the soil, the relatively flat site and poor drainage conditions, in combination with the residence located near a navigable water way, and rainwater discharging onto the north and east sides of the residence also contribute to elevated moisture and water intrusion in the soil and high April 29, 2010 Page 10 ProNet File No. 14142 relative humidity conditions within the crawlspace resulting in the observed differential foundation movement. In summary, the horizontal /vertical /stepped cracks in the interior and exterior finishes resulted from differential foundation movement from the changes in the soil moisture content of the supporting soils that occurred under nominal environmental (sun and rain) and seasonal (wet and dry cycles) factors occurring through the years, adverse soil behavior, improper yard drainage, and improper roof rainwater discharge in conjunction with the aging of the residence. The above submitted opinions are based upon sdentific methods, our inspection and observations, generally accepted engineering criteria and the professional knowledge and experience of the engineer in the forensic analysis of residential and commercial buildings and components. Such an inspection cannot detect all existing or potential defects and it should, therefore, be understood that future conditions affecting items discussed in this report cannot be predicted since they are all subject to change.
