This testing was performed in general accordance with Roofmg Application Standard TAS No.106 -- Standard procedure for field verification of the bonding of mortar or adhesive set tile system and mechanically attached, rigid, discontinuous roof systems. The total of the tested roof surface area was less than 10000 square feet, and the mean height of the roof is less than 40 feet above ground surface. ' The type of tile used for this project was reported to be Barrel Roof Tile. This tile was reported to have been foamed in place. At the time of this testing, one test for every corner, one test for every 100 square feet of roof tile in the perimeter area and one test for every 200 square feet of roof tile in the field area were tested. Initially the entire area of the roof was examine for loose tile. More than 10% of the roof tile located m the field area and more than 20% of the roof tile at the roof perimeter and corner were physically examined. The request is to allow the construction of a pnelesse sewage treatment plant that would serve a proposed shopping center. The subject property is located at 9450 N.E. 2"' Avenue, Miami Shores, Florida and is more particularly described in Exhibit "A" attached hereto and incorporated hciciu by reference. Section 24- 30(1) of the Code provides that package sewage ant plants may not be approved by the Director of the Miami-Dade County Department of Environmental 1tesources Management (DERM), unless so directed by the Miami -Dade County Environmental Quality Control Board. The Board finds that the available information indicates that the Petitioner proposes to build a 12,434 square foot, shopping center on the subject property that presently is occupied by a restaurant that is served by a septic tank. The existing restaurant would be demolished and the proposed shopping center would then be constructed on the site. Public water abuts the site lathe form of a 12 -inch main. The closest public sanitary sewer main is a 16 -inch force main located west of the 1-95 expressway, along N.W. 7111 Avenue, approximately one mile west of the proper The Board finds that the proposed interim package sewage treannent plant will consist of a five - thousand (5,000) gallons per day (GPD) activated sludge system with an aeration/de-nitrification basin, rapid sand filters and disinfection. The Board also finds that the treated effluent will be discharged to the proposed rapid infiltration basin or "soakage pit". The Board further finds that the Petitioner plans to follow design parameters governed by Chapters 62 -302 (Surface Water Quality Standards), 62 -520 (Groiiadwateir Standards, Classes and Exemptions), 62 -600 Domestic Water Facilities), 62 -610 (Reuse of Reclaimed Water and Land Application), and 62 -620 (Wastewater Facility Permitting) of the Florida Administrative Code (FAC). The Board also finds that a monitoring well down gradient from the soakage pit will be required to monitor certain water quality parameters depending upon the characteristics of the commercial domestic waste. The Board further finds no industrial waste will be discharged. The Board finds that the proposed interim sewage treatment plant is needed, because there are no sewage facilities in the area and it is not economically feasible to connect to the closest public sanitary sewer main. The Board also finds that the aforesaid treatment plant will be designed to meet the required high treatment standards. Additionally, the Board finds that the discharge of tertiary treated, highly disinfected effluent as required by County and State regulations presents a lower environmental risk to the groundwater quality of the area than the existing septic tank effluent discharge. Finally, the Board finds that, based upon the evidence and testimony presented and the foregoing findings, granting the requested approval pursuant to the provisions of Section 24-30 of the Code will not be detrimental to the public health, welfare and safety, will not create a nuisance, and will not materially increase the level of pollution in this County. ACCORDINGLY, IT IS ORDERED AND ADJUDGED that, based upon the evidence and testimony presented and the foregoing findings, Petitioner's request for approval of the subject petition pursuant to the provisions of Section 24-30 of the Code be and the same is hereby granted, subject to the following conditions: 1. The interim package sewage treatment plant shall be required to cease operations in accordance with the requirements of Section 24 -13(9) of the Code of Mlami- Dade County, when public sanitary sewer becomes available as provided therein. 2. RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change in the applicable building code negatively affecting the performance of this product. TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any section of this NOA shall be cause for termination and removal of NOA.
