commencement of or during said work caused by strikes, war, acts of nature, sudden rains, wind storms or any event beyond our control.
4 OASIS BUILDERS LLC does not guarantee against leakage due to footwear, building construction, fire, hail, hurricanes, termites or other
pest and animals, etc or any event beyond our control, or to defects in materials
5 In the event leakage does occur in work completed by OASIS BUILDERS LLC, you the owner or owner's agents must notify OASIS .
BUILDERS LLC, in a timely manner. OASIS BUILDERS LLC will then repair said leak at no charge to the owner, providing article #4 of this
section does not apply and said job has been paid for-in full and the guarantee is in affect
6 Work done or attempted to be done do this roof, other than by OASIS BUILDERS LLC or their authorized agents, relieves OASIS BUILDERS
LLC from any future obligafion hereunder.
7 OASIS BUILDERS LLC will not be held responsible for damages to sidewalks, driveways, landscaping, fences, sprinklers, or gutters, etc
where same must be be crossed to gain access to the building.
8 Due to the nature of roofing, OASIS BUILDERS LLC, shah not be liable for damages to walls, ceilings, interior furniture, fixtures, decorations, .
stock or equipment, air conditioners, solar heaters, screening, swimming pools and patios etc, either before, during or after construction or
otherwise.
9 This contract is not assignable and is not subject to alteration, except by written agreement executed by an officer of OASIS BUILDERS LLC.
10 In the event payment is not made as stated above, cost of collection including all attorney fees, court cost, collection agency and any other
fees associated with collecting the debt are due and paid by the owner(s) or owner's agent.
11 Any interruption in work which is the fault of the owner.or owner's agents, will be billed according to the time and materials lost and such cost
shall be due and payable upon receipt of a bill.
12 Due to the nature of roofing work, we strongly recommend that the following be completed by the owner or owner's agents prior to
commencing work:
Cover furniture with a drop cloth. Remove any lamps, art objects or other'breakables from rooms with exposed beam ceilings. Remove any
fixtures, paintings, pictures, art objects or other breakables from walls and ceilings.
Although we take all due care, OASIS BUILDERS LLC can not be held responsible for any dirt or debris that might fall through the sheathing
or decking into the attic or through exposed beam ceiling type roofs. Therefore some clean up may be required by the •owner or owner's
agents . after each phase of construction.
13 In the event that your area sustains a hurricane or hurricane strength winds as recorded by the National Weather Bureau, all guarantees by
OASIS BUILDERS LLC are null in void.

HIGH VELOCITY HURRICANE ZONES — REQUIRED OWNERS NOTIFICATION
FOR ROOFING CONSIDERATIONS
R4402.13.1 Scope. As it pertains to the section, it is the responsibility of roofing contractor to provide the owner
with the required roofing permit, and to explain to the owner the content of the section. The provisions of Section
R4402 govern the minimum requirements and standards of the industry for roofing system installations.
Additionally, the following items should be addressed as part of the agreement between the owner ant the
contractor. The owner's initial in the designated space indicates•that the item has been explained.
Aesthetics - Workmanship: the workmanship provisions of Section R4402 are
for the purpose of providing that the roof system meets the wind resistance and water
instruction performance standards. Aesthetics (appearance) are not a consideratior,witk'
respect to workmanship provisions. Aesthetic issues such as color orGhltectural • • • •

3 5'1 Common roofs: Common roofs. are those which have no ti41514 delinee io
between neighboring units (i.e., townhouses, condominiums, etc.) In buildings with•''
common roofs, the roofing contractor and/or owner should notify the occupants of adjacent
units of roofing to be performed.
Exposed Ceiling: Exposed; open beam ceilings are where the underside of the
roof decking can be viewed from below. The owner may wish to maintain the architectural
appearance; therefore, roofing nail penetration of the underside of the decking may not be
acceptable. The provides the option of maintaining the appearance.
Ponding water: The current roof system and/or deck of the building may not
drain well and may cause water to pond (accumulate) in low -lying areas of the roof.
Ponding can be an indication of structural distress and may require the review of a
professional structural engineer. Ponding may shorten the life expectancy and performance
of the new roofing system. Ponding conditions may not be evident until the original roofing
system is removed. Ponding conditions should be corrected.
6. a� Overflow scuppers (wall outlets): It is required that rainwater flow off so that
the roof is not overloaded from a buildup of water. Perimeter /edge wall or other roof
extension may block this discharge if overflow scuppers (wall outlets) are not provided. It
may be necessary to install overflow scuppers in accordance with the requirements of
Sections R4402, R4403 and R4413.
7. a .�P Ventilation: Most roof structures should have some ability to vent natural
airflow through the interior of the structure assembly (the building itself). The existing
amount of attic ventilation shall not be reduced. It may be b , "tct to consider additional

LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami Dade County Product Control Approved", unless otherwise noted herein
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall
be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.

5. Fastener spacing for insulation attainment is based on a Nfinimum characteristic Force (F) value of
275 lb£, as tested in compliance with Testing Application Standard TAS 105. If the fastener value,
as field- tested, are below 275 M. insulation attachment shall not be acceptable.
6. Fastener spacing for mechanical attachment of an c sheet or membnine attachment is based
on a fastener mistime value in conjunction with the maxim= design value listed within
a specific system. Skald the fastener resistance a be less than that required, as determined by the
Building Official, a revised fastener spacing, prepaid, signed and sealed by a Florida Registered
Engineer, Architect, or Registeied Roof Consultant may be submitted. Said revised fastener spacing
shall utilize the withdrawal resistance value taken from Testing Application Stamdar s TAS 105 and
calculations in compliance with Roofing Application Standard RAS 117.
7. Perimeter and corner areas shall comply with the affianced uplift pressure requirements of these
areas. F shall be increased for both insulation and base sheet as calculated in
compliance with Roofing Application Standard RAS 117. (When Ibis limitation is spedfically
referred this NOA, General Limitation #9 will not be applcsble.)
8. All attachment and sizing of perimeter milers, metal profile, and/or flashing termination designs
shall conform with Roofing Application Standard RAS 111 and applicable wind load requirements.
9. The maximum desi piessure limitation listed shall be applicable to all roof pressure zones (i.e.
field, perimeters, and corners). Neither rational analysis, nor extrapolation shall be permitted for
enhanced fastening zones (Le. perimeters, extended corners and corners).

Shores, Fl. The perlite insulation had deteriorated in areas from previous roof leaks and
was not salvageable during the re- roofing process. We are aware that the 1" perlite
insulation has an R -value of R -2.78 and that it is more cost effective to install new
insulation above the ceiling with a minimum R -value of R -19 rather than installing new
1" perlite insulation on the roof deck.
