LABELING: Each unit shall bear a pednanent label with the manufacturer's name or logo, city, state and
following std "Miami -Dade County Product Control Approved", :tam otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable builctng code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as as endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for tarnmlnntion and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall
be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.
This NOA renews and revises NOA # 040528.05 and consists of this page 1 and evidence pages E-1 and 13-2, as
well as approval document mentioned above.

b. Misuse of this Acceptance as an endorsement of product; for sales, advertising or any
other purpose..
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the Notice of
Acceptance is displayed, then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies,
shall be provided to the user by the manufacturer or its distributors and shall be available fer
inspection at the job site at all times. The engineer does not need to reseal the copies.
B. Failure to comply with any section of this Acceptance shalt be cause for termination and removal of
Acceptance.

This NOA shall not be valid after the expiration date stated below. BCCO reserves the right to secure this
product or material at any time from a jobsitc or manufacturer's plant for quality control testing. If this
product or material fails to perform in the approved manner, BCCO may revoke, modify, or suspend the
use of such product or material immediately. BCCO reserves the right to revoke this approval, if it is
determined by BCCO that this product or material fails to meet the requirements of the South Florida
Building Code.

"French Door XX, X" Sheets 1 through 4 of 4, prepared by manufacturer, dated 02/16/98, with
revision C dated 1 -22 -99 and last revised on 04- 13 -01, signed and scaled by Robert I.. Clark, P.E;.,
bearing the Miami -Dade County Product Control approval stamp with the Notice of Acceptance
number and approval date by the Miami -Dade County Product Control Division. These documents
shall hereinalier be referred to as the approved drawings.
3. LIMITATIONS
3.1 This approval applies to single unit application of pair °I-doors and single door only, as shown in
approved drawings. Single door unit shall include described in the active leaf or this approval.
4. INSTALLATION
4.1 The outswing aluminum French doors and its components shall be installed in strict compliance
with the approved drawings.
4.2 Hurricane protection system (shutters): the installation of this unit will not require a hurricane
protection system.
5. LABELING

1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documentation, including test supporting data, engineering documents, are no older than
eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3. Renewals of Acccptancc will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) lithe Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation oldie product;
d) The engineer who originally prepared, signed and scaled the required documentation initially submitted is
no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and /or manufacture oldie product or process shall automatically
be cause for termination of this Acccptancc, unless prior written approval has been requested (through the
filing of a revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process.
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other purpose.
6. The Notice of Acccptancc number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acccptancc is
displayed, then it shalt be done in its entirety.
7. A copy of this Acccptancc as well as approved drawings and other documents, Nvhere it applies, shall be
provided to the user by the manufacturer or its distributors and shall he available for inspection at the job site
at all time. The engineer need not reseal the copies.
S. Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acccptancc.

SCOPE.
This NOA is being issued under the applicable rules and regulations governing the use of construction materials. The
documentation submitted has been reviewed by Miami -Dace County Product Control Division and accepted by the Board of
Rules and Appeals (BORA) to be used in Miami Dade County and other areas where allowed by the Authority Having
Jurisdiction (AIM.
This NOA shall not be valid after tie expiration date stated below. The Miami-Dade County Product Control
Division (Iii Mucci Dade County) and/or the /Iii (in antes other than Miami Dade County) reserve tlx right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the Azar may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described mein, and has been designed to comply with the High Velocity Hurricane
Zone of the Florida Building Code.
