In accordance with your request and authorization, a representative of LG Engineering Services, Inc.
completed the Roof Tile Uplift Test at the above referenced project. This testing was performed in
general accordance with Roofing Application Standard TAS No.106 -- Standard procedure for field
verification of the bonding of mortar or adhesive set tile system and mechanically attached, rigid,
discontinuous roof systems.
The total of the tested roof surface area was less than 10000 square feet, and the mean height . of the
roof is less than 40 feet above ground surface. The type of tile used for this project was reported to
be Lifetile Roof Tile. This tile was reported to have been foamed in place.
At the time of this testing, one test for every corner, one test for every 100 square feet of roof
the perimeter area and one test for every 200 square feet of roof tile in the field area w
Initially the entire area of the roof was examine for loose tile. More than 10% of the
in the field area and more than 20% of the roof tile at the roof perimeter and corn
examined.

In accordance with your request and authorization, on January 20, 2005, a representative of LG
Engineering Services, Inc. performed a visual roof inspection for the flat roofs at the above
referenced project. This inspection was performed, by means of visual observations and conducting
roof field test cuts, to inspect the tin capping and hot mopping of the roofing felts.
At the time of our inspection, the installation of the roof surfacing material had been partially
completed. Three roof field test cuts were made. Based on our findings, the followings were found:
Flat Roof
1. The roof system consisted of a built -up roof system. The base sheet was noted to have been
mechanically attached to the underlaying wood deck. The nailing pattern for the field area
was between 6 and 7" o.c. at the side lap and two rows between the side laps between 6 and
8" o.c. The type of fastener used was 1 -1/4" long ring shank nails thru 1 -5/8" tin caps.
2. The nailing pattern for the perimeter /corner areas was between 5 and 7" o.c. at the side lap
and six rows between the side laps between 5 and 7" o.c. The type of fastener used was 1-
1/4" long ring shank nails thru 1 -5/8" tin caps.
3. On top of the base sheet, two plies of roofing felts were fully mopped. In some areas, the
mineral surfaced cap sheet was also had been applied. The amount of asphalt used was noted
to be adequate where the roofing felts were not touching.

1524.1 As it pertains to this section, it is the respond of the roofing contractor to provide the owner with the
required roofing permit, and to explain to the owner the content of this section. The provisions of Chapter 15 of the
Florida Building Code, Bulking govern the Wolin= requirements and standards of the industry for roofing system

RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the perthrrnance of this product_
TERMINATION of this NOA wiil occur after the expiration date or if there has been a revision or change in the
rnareaials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature_ If any portion of the NOA is displayed, then it shall
be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
