Kawalek Roofing & Painting, Inc. 54 NE 95 Street The principle parties to this contract, Ms. Williams, owner and Mr. Kawalek, roofer, recognize this roofing job began on June 29, 2005, as Miami Shores Permit # BP05 -803. $8000.00 was paid to Mr. Kawalek on July 01,2005 ($4000 distributed for each h ®MGM). the permits were allowed to expire on Dec. 28,2005 for House 64 and on Dec. 18,2005 for House 86. This contract is essentially an agreement to finish said job in a timely and professional manner. The property owner, Judith Williams, is, currently charged with violations of Sec.4, Sec.12 -128, Case # 9 -07 -2818, MiAbi Shores Village Code Enforcement. On May 22,200t7, the Miami ShOtes Budding Dept. notified Ms. Williams, and presumably Mr. Kawalek, roofer (CC B800007, rc 0066925), that a complaint will be filed with Miami Dade .Code CoMplience Office for possible disclipinary action if requirements were complied. with.
