I have inspected the installation of the anchorage for the roof rafters at the above
referenced single family residence. The anchors are in compliance with the specifications
provided to Anthony Wilson Roofing and meet or exceed the up -lift requirements of the
code.
In order to avoid any possible misunderstanding, we wish to specifically advise that
nothing in this report is to be construed, either directly or indirectly, as a guarantee or
warranty of any portion of the work. To the best of our knowledge and professional
ability, this report represents an accurate appraisal of the present physical condition,
based on a visual examination of the observed conditions to the extent reasonably

wind uplift as required by the Florida rule for Retrofiting roof members for wind uplift.

CHANGE ORDER # 2
To perform extra - contractual work to the flat roof as follows:
A) East Side Edge:
Install the following material/system to the first 4 rafters at the North and South ends
of the East side and on every other rafter in between ( approx 40 -45 rafters):
18 gauge x 6" wide stainless steel angle/strap,
Fastened with 4 screws into sides of rafter,
- Fastened with 4 — 5/16" x 24 grade 8 bolts screwed into tapped holes in top flange
of existing I -beam.
Also
Install at every location where a rafter crosses the top plate at the wood wall, and
at the rafters not addressed with stainless steel strap over the I-beam, install 1
Simpson HGA10 with 8 fasteners.
B) North & South Sides
- Fasten 1 new hurricane strap to the face of the existing straps with (3) 12 — 14
Hex Tec screws. Bend straps over top of rafter and fasten with 4 # 12 screws.
C) West Side
- Install a custom designed and built steel angle -iron strap assembly that will secure
the 3 member girder rafter just inside the west fascia and the first full rafter east of
the girder assembly.
- This assembly will be bolted to the existing W-4 I -Beam that runs between the
girder and the rafter with one (1) 3/8" bolt, secured with 2 nuts (see attached
drawing).
There will be a minimum of 20 of these brackets installed approximately every 3
linear feet.
