Do hereby petition the Village of Miami. Shores -to actas my own contractor pursuant to the laws
of the State of Florida, F.S 489.1.03(7). And I •have read and understood the following disclosure
statement, which entitles me to work as my own contractor; I further understand that I as the
owner must appear in person to complete. all applications.
State Law requires construction to be done by a licensed contractor. You have applied for a
permit under an exception to the law. The exemption allows you, as the owner of your property,
to act as your own contractor even though you do not have a license. You must supervise the
construction yourself. You may build or improve a one family or two- family residence. You may
also build or improve a commercial building at a cost of $25,000.00 or less. The building must be
for your own use and. occupancy. It may not be built for sale or lease: If you sell or lease a
building you have built yourself within one year after the.construction 'is complete, the law will
presume that you built for sale or lease, which is a violation of this exemption. You may not hire
an unlicensed person as a contractor. It is your responsibility to make sure the people employed
by you have licenses required by state law and by county or municipal licensing ordinances. Any
.person working on your building who is not licensed must work under your supervision and must
be employed by you, vvhich means.that.you must deduct F.I.C.A and with -holdings tax and •
provide workers' compensation for that employee,,all as prescribed by law. Your construction
must comply with all applicable laws, ordinances, buildings codes and zoning regulations.

SECTION 2 - EARTHWORK
2 -01 EXTENT OF WORK: The work includes all necessary earth and rook exoava-
tion, fills and back- filling, rough and finished grading necessary for
footing, fou�j ndation walls, terraces, etc., shown on the plans. The plumber
shall do all excavating and bank- filling necessary for his work.
2 -02 CLZARING OF SITE: The General Contractor shall clean the entire build-
ing site of all brush, rubbish and all other objectionable matter. Do not out
down any trees without first consulting the Arohiteot. All trees within the
area of building wall shall be transplanted on plot where directed by the
Architect.
2 -03 EXCAVAtING: The Contractor shall do all earth and rock excavating for
all footings and under all parts of the building wherever required for the
construction of building to a depth as shown or specified.
2 -04 FILL AND BACK -FILL: All earth used for filling and bank- filling shall
be Olean and free from rubbish, veketable or other objectionable matter. All
filling and back - filling when placed shall be puddled and tmaped. Fill and
bank -fill to proper levels for all tile end cement floors, sidewalks and run-
ways.
2 -05 GILDING: This Contraotor shall fill or excavate as necessary to bring
the finished grade to levels shown on the drawings at the building line. From
these lines the finished grade shall be worked to meet the grades as shown.
SECTION 3 - MISCELLANEOUS IRON, BRONZE & STEEL
3-01 GENERAL REQUUIR M IT8: This Contractor shall furnish all wrought iron
anchors, wrought iron pot hangers, wrought iron pipe vents, bolts, ties, clips,
etc., necessary for the entire building operation.

miscellaneous masonry necessary for the completion of the building as shown
on the drawings.
5-02 MATERIALS:
A. Cement - Shall be Florida Portland Cement conforming to the current
standard specifications of the ltmerioan Sooiety of Testing Materials.
B. Sand - Shall be clean, sharp and free from any foreign or deleter-
ious matter, and shall be well graded from coarse to fine.
C. Concrete Blocks - Shall be of approved mixture and texture, cured
at least eight days in the shade and kept well dampened; No oraoked blocks
shall be allowed. Blocks shall oomply with the requirements of the looal
building code, and shall be 8" x 8" x 16 ", bearing the underwriter's label.
D. Broken Rook - Shall be clean And hard of sizew shown on the drawings..
E. Common Brick - Shall be sound, thoroughly hard burned, of standard
size, free from defects and shall be whole and new.
F. Fire Brick - Shall be made sound, hard burned] fire clay brioks.
G. Cement Brick - Shall be well cured, hard, and of standard size.
H. Flue Linings - Shall be standard terra ootta flue linings of sizes
shown.
I. Water - Shall be from City Main.
d. Exposed Brick - Shall be old red brick, sound and thoroughly hard
burned.
5 -03 STRUCTURAL CONCRETE: Grade beams, walls columns, tie beams, etc.,
shall be reinforced oonorete as shown on the drawings. Conorete shall be
mixed to proportions of 1 cement, 2* sand, and 3i rook. .gigcregate shall be
clean and graded and free from all matter that, in the opinion of the Engne-
ews, is deleterious to concrete. Water from City Mains.
5 -04 REINVOACING STEEL: All steel for concrete shall be new, intermediate
grade, oorreotly fabrioated, placed and held rigid in place.
5 -0b SUB - FLOURS: All sub - floors shall be as called for on the structural draw-
ings or noted on the plans. Fhe finish of all sub - floors shall be sufficiently
low, to provide proper setting bed for finish floor palled for in plans and
specifications. Provide waterproof paper under all concrete slabs on fill.

5 -08 MASONRY "IORK: All cement blocks shall be laid up true and plumb in a sub -
stantisl workmanlike manner, using cement brick for filling, where neoessary

lines using clay mortar. Entire chimney shall be constructed of common cement
bricks except where oonorete beams comas.
5 -12 DRIVE'W'AY: Drigeway where shown shall be built of 8" rock rolled to 6"
with pea rock finish.
SECTION 6 - ROOTING & SBEETlETAL RK
6 -01 E1TEI T OF WORK: This part of the contract includes materials, labor
and equipment for all tile roofs, sheetme tal work and fleshings of all de-
scriptions for the house. All work shall be watertight at c omle ti on.
6.02 FELT: The entire roof shall be covered with 301 felt and A 190 slate
surfaced Genaoso roofing felt as manufaotured by Barber Asphalt Company,
being oareful to stagger all Joints. Felt mall be applied to have at it
3" over lap horizontally and vertically, upper edge, well nailed with large
head galvanized roofing nails. felt on hips and ridges shall lap 12" both
ways. Swab in all felt with pitch.
6-03 TILE: The entire roof shall be covered with cement tilt as selected.
Tile shall be laid in solid mortar bed. Under side of all eave tile to be
pointed up. No badly broken tile will be allowed and the entire roof must
be swept clean.
6-04 NAILS: All sails used in oonneotion with roofing shall be galvanized
nails.
6 -05 FLASHING: All intersections of roof with vertical surfaces, of every
nature:, shall be flashed and counter flashed with Toucan Metal. Flashing
shall be full length pieces looked and soldered joints. All pipes passing
through roof shall be caulked in the hubs. Rosen shall be used or the sold-
ering flux. Provide Tonoan metal drip strip.
6-06 GUARANTEE: This Contractor shall give a written guarantee against
all defects of workmanship and material in the roof for a period of five (5)
years after date of acceptance.
SECTION 7 - GLASS & GLAZING
7 -01 EXTENT 01' RZ : This Contractor shall furnish and install all glass
necessary far doors, oabinets, sash or vtherever else called for or required
by the drawings.
7 -02 GLASS: Glass for all doors and windows not specified otherwise &a11

11 -01 EXTENT OF YORK: This part of the contract includes all materials, lebor

wrought iron work, pipes, hinges switch and outlet plates, eto.
11 -02 MATERIALS: All paints, varnishes, lacquers and oils shall be Sherwin
Williams, DuPont, Benjamin Horre & Co., or Pittsburgh Plate Glass Co. All
paint shall be delivered on the job in their original packages. They shall
be used in strict accordance with the manufacturers directions and mast not
be thinned unless so directed to do so by the Architect. All colors to be
selected by the Architect.
11 -03 WORKMANSHIP: All dust, dirt, eto., shall be removed before Npplying
paint. Puttying shall be done after the priming coat has been applied. All
finished surfaces shall be smooth, even and free from defects. Sand papering
shall be done where necessary. Closets shall be the same r,s specified for
the adjoining room. See that the woodwork is properly prepared and in per-
fect condition before any painting is started. Ftiling to do so, the paint-
ing Contractor shall be responsible for any defective point.
11 -04 PRIMING & BACK - PAINTING: All woodwork to be painted shall be primed
with lead and oil paint. iteri or door frames and window bucks shall be back -
painted with a heavy ooat of creosote before erection. All trim shall be
primed xi before plastering. The backs of all cabinets shall be given e. heavy
ooat of shellac after fitting.
11-06 EXTERIOR WOODWORK: All exterior roadwork shall receive, in addition

SECTION 12 - PLUMBING
12 -01 SPECI .i oT.L: The "General Conditions of the Contreot ", issued by the
A.I.A. and the General Conditions of t ese specifications shall form a part
of these specifications on Plumbing.
12 -02 EXTENT OF WORK: The work included by this specification comprises the
necessary labor and materials to install complete in every detail the plumb-
ing system of Sanitary Drainage, hot and cold water supply system. The
installation shall be so made thr't the several oom:Jonent parts will function
together as a workable system oom)lete with all aocessories for its operation,
and finally left with all equipment properly adjusted and in working order.
12 -03 P: .ITS & CERTIFICATES: This Contractor shell promptly obtain and
pay for all permits necessary to carry on and complete the plumbing, unless
otherwise herein specified, and shall conform with all existing cotes, rules
and regulations in effect at the plaoe of building.
12 -04 CHARACTER OF WORK: The entire plumbing system must be so instelled and
perfected as to fully comply with the Florida :state Board of Health and all
governing bodies that have juriadiotion at the plane of building, oarefully
comply with their requirements and notwithstanding anything to the contrary
in these ` specifications.
12 -05 CUTTING & ALTERING WORK OF O ITERS: Immediately upon signing the contrast
for this work this Contractor shall inspect. the plans and apecifioations to
assure that the necessary provision for the proper installation of this work
has been made. This Contractor shall furnish to the other trades Contraotors
such information as is necessary to progide chases, pockets. and openings in the
structure as it is built, avdding as much cutting as possible in the rough
structure.
12 -06 WATER SERVICE CONNECTION: This Contr ctor shall arrange with the water
company for a tap to the we ter mein and the installation of a I" water meter
with a full size by -pass and gate valve. Meter and tap to be paid for by owner.
12 -07 EARTHWORK: All excavation and baok- filling required in the construction
of this work shall be done by this Contra who shall furnish such braving,
sheathing, etc., as may be required to properly protect foundation& or walls
of the building as directed. Baok- filling shall be done in a menner to prevent
undue settlement, and surplus earth removed as directed.
12 -08 MATERIALS: All materials furnished must be new and free from defects
and be the best of each grade specified or required for first class work.
All water pipe and fittings from meter throughout, or size shown shall be gal-
vanized steel, or equal as approved. All soil pipe shall be extra heavy oast
iron.
12 -09 SIZE OF WATER PIPE: This Contractor shall make an approved 1 -1/2" oon-
neotion to the 4" water meter and graduating the size of all branches therefrom
as they connect into more than one riser to a size aggregating the total area
of all risers so connected. Branch pipes connecting to e. single riser shall
be at least as large as the riser and never less that& i" pipe.
12•-10 SPECIAL REt UIREMATTS: All water piping passing through o onorets shall
be wrapped with t deadening felt £+ tar paper. Congrol valves for all tubs
and sinks shall be located in concealed, but aooeasible plane. Risers to each
group of fixtures shall be provided with vertioal air chambers at lear;t 24"
high, one pipe size larger than riser. All hot and cold risers shall be suf-
fioiently large to furnish a full supply of water to all fixtures oonneoted
thereto, and shall be at least k pipe. Approved aleanout plugs shall be in-
stalled wherever required. All vent stacks shall expend through the roof. All
pipes passing through roof shall have lead oollar of #4 lead turned into top
of pipes and flange 12" on roof around pipe.
12 -11 SEPTIC TASK: Provide and install oomplets a 550 gallon concrete septic
tank with 125 feet of drain tile. Drain tile shall be ]aid on a bed ofd'
crushed #z rook and z covered with 4" orushed rook. prop of rook to be 6" below
finish grade. See plot plan for location. Same shall be installed aoosrding
to manufvcturer's direotions.
12-12 SILL COCKS: Install where shown on plans or where instructed by the
Architect, 3/4" sill cooks.
12 -13 TESTS: Before any of the roughing -in work is covered, and prior to the
setting of the fixtures and equipment, all water pipes shall be subject to tests
for tightness of joints under pressure to whioh they may be subjeoted. Water
pressure to be mainained in pipes during the entire construction. Tests for
tightness of .soil, west vent lines shall be made in a000rdanoe with looal
regulations.
12 -14 GUARANTEE: This Contractor shall be responsible for the proper installs.

SECTION 13 - ELECTRIC
13-01 GENERAL: The "Articles of the General Conditions of the Contract" stand-
ard form of the A.I .A. , fourth edition, copyright 1935, together with tho
General Conditions of the apeoifications for the entire work as hereinbefore
set forth are to be considered as an integral part of the specifications for
electrioel work. By ,submitting a bid on these specifications and the acoompany-
ing plans, Contractors shall be held to have femiliarieed themselves with all
of the aforementioned articles, and ae successful Contractor to execute the
contract accordingly.
13-02 SCOPE OF WORK: The work under this o ontra.ot comprises furnishing all
labor amd materials neoessary for the complete installation of electric wiring
and lighting, power and heating devises, public and private telephone service
signal system, radio service, distributed to the various outlets as indicated
on the plans in Ell details from the solace of supply for each system. Eaoh
system is to be complete in itself for the service intended. The ocntraotor,
will be held to have carefully oompae.d the drawings and specifications, checked
conditions at the site under which this installation is to be made, the install.
ation conform to the requirements of the plans and specifications, irrespeo ivs
of whether each individual item is specifically mentionet or noi.
13--03 REGULATIONS, PERMITS & CERTIFICATES: All of the work covered in these
specifications shall conform to the National Electrical Code in effect at
the time, the rules and regulations of local and state bodies, the respective
service eompaines' supplying the services, and the Engineering Standards as re-

commended by the Assooiation of Electroligists. Workmanship and materials
which exceed the requirement of the above mentioned authorities and herein
specified, shall take precedence over such ordinances. The cost of obtaining
the necessary permits and of seouring the certificates of inspection and approval
for all work installed under this contract shall be borne by the Electrical Con-
traotor. Final certificates of approval shall accompany the final request for
payment of this work, as no final settlement will be made by the Architect un-
less this provision ie adhered to by the Contractor.
13-04 MATERIALS & WORKMANS1IIP: All materials used throughout the installation
shall be new and the best of their respective kind, installed in a neat workman -
like manner, and to the entire satisfaotion of the Architect and all governing
bodies. The Contractor shall upon request of the Arohitect, furnish samples of
the materials whichehe porposes to furnish and install, and obtain written ap-
proval of their use before proceeding with the work.
13 -05 ML+'TNOD OF WIRING: Unless otherwise specified, all wires are to be ruin
ooncealed in conduit from the point of service to the lest outlet, providing a
continuous and electrically bonded raceway for the wires.
13 -06 PLANS: The plans indicate the outlets and switches in their respective
locations by standard symbols, location of all special outlets shall be checked
with the Architect before installing same. Any outlets not located properly
shall be moved where directed at this Contractor's expense.
13-07 CUTTING & FITTING: Generally, all cutting and notching incidental o the
installation or eleotrlcel work will 'be done by the General Contractor at the
direction of this Contractor, provided the cutting is ordered prior to the cod -
pletion of such construction work, which may entail alterations in the finished
work to accomplish same. This Contrsctor's failure to advise of special require-
ments, prior to completed construction of parts affeoted will necessitate an or-
der by the Architect to effect same at the expsne of the Electrical Contractor.

1$-48 SERVICE & METERING: Electric service will be supplied by the power
oompany s t a point sand in a manner as indicated on the plane. Provide and
install the necessary oonneetions for overhead service into the building
where shown in strict conformity with the requirements of the power company
and their reoommendatione for this installation. Terminate the service lines
in a main switch supplied by this Contractor, and metering block furnished to
this Contractor by the power company for installations The neoessary meter
board and meter loops are to be provieded as required. When the number and
size of the service! wires are not shown on the plans, this Contractor will be
required to install, them of = capacity for the requirements.
13 -09 DISTRIBUTIONS Froni the main switch or sub - feeder panel, supply the re-
spective feeders, lighting panels, special outlets, and lighting outlets and
controls 'required, Furnishing the necessary wire, conduit and accessories for
their complete service. separate circuits shall be provided aeeording to their
individula capacity for electric refrigerator, ironing board receptacle, Ventila-
ting fan, water heater, electric range, electric, room heaters, etc.., as required.
1.., -10 CHANGING OUTLETS: Where it becomes necessary to obange the locations of
outlete to fit and center with panels or panelled wall spaces, this Contractor
shall, et his own expense, move the outlets as required.
13-11 SYSTEM GROUND: furnish and install a system and equipment ground of a
size and in a manner es presoribed.by the jurisdictional regulations. Same
shall be neatly and accessibly installed in a manner and location acceptable
by the L rchiteat. Where necessary, make 'connection to a water pipe and leave
same exposed, this Contractor shall make arrangements with the plumbing con-
tractor to make a short extension of his piping to accomodate the oonneotion at
the location desired, the cost of same to be borne by the Eleotrioal Contractor.
13-12 INSTALLAT1uN REQUIREMENTS: Lighting Outlets: All ceiling, wall or
switoh outlets shall be provided with a pressed steel galvanized outlet box
fitted with a plaster ring to finish flush with the finished plaster. Outlets
meant for mounting fixtures shall be provided with a 3/6" fixture stud rigidly
festened to the outlet box. U boxes shall be rigidly mounted with an ap-
proved outlet box support of metal, aoourate]y as the oenter lines or other
dimensions given. This Contractor shall ascertain from the Architect the de-
sired mounting heights of all wall outlets and switches before proceeding with
his work, unless such special notations are carried upon the plans.
13-13 FIXTURES & LAMPS: Unless otherwise specified, all lighting fixtures
and lamps will be furnished by others and hung by this Contractor.
13 -14 NUMBER JF CIRCUITS: The number oi circuits to be contained in branoh
lighting panel -board shall be such as to provide for one circuit for, eaoh 600
square feet of floor area. Allow at least one additional circuit in eaoh panel
for a spare and when required for balance of panels, . there being no unequal num-
ber of active circuits, two or more circuits shall be allowed. Where power oir.-
ouirts are taken from lighting panels or where power panels are provided, furnish
an added 30 amp. outlet as a spare power detail. Light and power feeders shall
be provided to properly serve the above requirements without overload or the
future necessity of increasing the respective feeders to serve same.
13-16 BRANCH PANEhBOARDS: Shall be solid neutral type employing moulder units
mounted on steel backs, es manufactured by Bull -Dog Pro. Co., Westinghouse,
Frank Adam Co., Square D. Co., or Cleveland Dwitahboard De.
13 -16 SAFETY SWITCHES: Shall be externally operated . type "C2 ", quiok make and
break sgitohes mounted in a steel cabinet. Same shall be as manufaotured by
Bull -Dog, Square C, Westinghouse, or Trumbull.
13-17 CONNECTIJNS TO . PI'LIANCES: Shall be made by this Contrpotor from a
heavy duty reoeptaale of the et paoity required and installed by this Contraotor
in a well adjacent to the appliance, with wiring in flexible conduit and with
provisions for a gro,tnd connection. Reaptables and plugs ehe].l be P. & S.,
Bryant, or G. E., for medium oapaeity, for heavier capaoity, same &all be as
manufactured by H & H., Hubbell, or Raloo.
13 -18 CONDUIT: Used throughout the installation s, all be thick wall steel
tube galvanized or sheradized, as manufactured by G. E., National Eleo. Prod.
Co., or Central Tube
13-19 WIRE: Shall be of National Electric Code requirements as manufactured
by American Steel and Wire Co., National Elec. Prod. Co., or G. E. Co. Wire f8

13-20 WALL RECEPTACLE: Shall be flush type duplex of like manufacture: P &S

13-23 SWITCH & RECEPTA, LE MOUNTING HEIGHTS: Generally, swithoes shall be
mounted at 4 feet, Phil receptacles shall b e 18 inches above finished floor.
Prior to starting the w rk, this Contractor shall ascertain if these dimensions
hold throughout and be overned by the Architect's recommendations unless

sentative. The final result shall indicate that all wiring shall test free
and be clear from any faults common to such instillations.
13-33 GUARANTEE: All Materials installed under this oontraot shall be guarante•
ed in writing to the Architect for a period. of 12 months after final scope tans•
of the work. Under this guarantee the Contractor shall repair or replace at
his own expense any parts which may become defeotive or due to pour workmanship
