SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by Miami -Dade County Product Control Division and accepted
by the Board of Rules and Appeals (BORA) to be used in Miami Dade.County and other areas where allowed by
the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami Dade County Product Control
° Division (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted mamier, the manufacturer will incur the expense of such testing and the AHJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami -Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the High Velocity Hurricane
Zone of the Florida Building Code. ,

APPROVAL DOCUMENT: Drawing No. W03 -38, dated 06/07/03, titled "Series -230 Alum Out -Swing Door
W/ Sidelites (N.L) ", sheets 1 through 7, prepared by 'Al Farooq Corporation, signed and sealed by Humayoun •
Farooq, P.E., bearing the Miami -Dade County Product Control Approval stamp with the Notice of Acceptance
number and approval date by the Miami -Dade County Product Control Division.

LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and •
following statement: "Miami -Dade County Product Control Approved", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall
be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.
This NOA revises and renews NOA # 99- 0902.02 and consists of this page 1 as well as approval document
mentioned above.

Extension of Acceptance may be considered after a new application has been tiled and the
supporting data, test report no older than ten (10) years, have been re- evaluated.
All reports of re- testing shall bear the seal, signature and date of an engineer registered in the
State of Florida.
Any revision or change in the materials, use, or manufacture of the product or process shall
automatically be cause for termination, unless prior approval is granted for revisions or change.
3. Any unsatisfactory performance of this product or process or a change in Code provisions shall
be ground for re- evaluation.
4. This acceptance shall not be used as an endorsement of any product for sales or advertising
purposes.
5. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the Notice of

1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documentation, including test supporting data, engineering documents, are no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Dade County Product Control Approved ", or as specifically stated in the specific conditions of
this Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of
this product and the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation
initially submitted , is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be
cause for termination of this Acceptance, unless prior written approval has been requested (through the filing of a
revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:

10. Unless specifically indicated in the Acceptance (approval), this unit-is approved as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is required from the Product Control Section.
11. The spacing of fasteners at window sills shall be as tested. The spacing of fasteners in all other parts of the frame,
shall be as tested, but in no case shall exceed 24" on center. The first fastener shall be located at a maximum of 6"
from each corner and mullion or stile. Fastener shall fully penetrate the buck, which shall be the same size as the one
tested with the unit. No wood or plastic shields or pins shall be used for anchoring. See section four (4) of this
approval for sizes of fasteners used in testing.
12. Hardware for all windows and doors shall conform to Security and Forced Entry Prevention, Chapter 36 of the South
Florida Building Code.

1. DESCRIPTION
1.1 This approves an 18 ga. galvanized steel storm panel designed to comply with the South Florida
Building Code Impact Test Requirement and ASCE 7 -88 "Minimum Design Loads for Buildings
and Other Structures ", for the following locations:
1.1.1 Non- coastal building zone.
1.1.2 Coastal building zone.
2. MATERIAL CHARACTERISTICS
2.1. The storm panels shall be constructed as indicated on approved drawing No. 94 -152, Sheets 1 of
4 using 18 ga. steel (.045" thick) structural quality grade C, conforming to ASTM A446.
2.2 Panels are rolled formed, have a nominal width of 12.00" and a total width of 13.625 ", forming
2" deep ribs
2.3 Components shall be aluminum alloys 6063 -T6, with shapes and thicknesses as shown on
approved drawing.
2.4 Tested specimen:
- Dimensions: 3 panels totaling 37 1/2" wide x 101" long overall, corresponding to 96" span.
Dimensions: 3 panels totaling 37 1/2" wide x 137" long overall, corresponding to 132" span.
3. LIMITATIONS
3.1 The design will be dependent on the maximum panel height of 101" and 137 ", to be installed
only as outlined on drawing No. 94-152 Sheets 1 thru 4 of 4.
3.2 Minimum separation to existing glass to protect shall be
3.2.1 For panels uD to 101" high: 1 7/8 ".
3.2.2 For panels over 101" high and UD to 137" high: 2 7/8 ".
3.3 Maximum clearance between tqp of panel and inside of header shall be 1/4 ".
3.4 Special Requirement:
All panels with spans from 137" to 101" high, require 1/4 " -20 x 1" sidewalk bolts w /die cast
aluminum washered wingnuts spaced at 1/3" of the span along all overlaps.
All panels with spans from 101" to 33" high, require a 1/4 " -20 x 1" sidewalk bolt w /die cast
aluminum washered wingnut at midspan at all overlaps.
Panels less than or equal to 33" high do not require fasteners at overlaps.
See Drawing No.94 -152 for detail of requirements.
4. INSTALLATION
4.1 The panels shall be installed only in the following condition:
4.1.1 Wall Mount (Direct Mount)
The panels are secured to an "h" header (Part 2) or bolted, using 1/4 " -20 machine bolt &
wingnut at 6" o.c., to a continuous 2" x 2" x .125" studded aluminum angle (Part 4) at the head;
and using this same angle at the sill; all are anchored to the wall, using the tension anchors as
shown on Drawing No. 94 -152, Sections A , D and E "Wall Mount" Sheet 2 of 4, bearing the
Dade County Product Control approval stamp.

4 l 2 Buildout Mount
The panels are fastened to the continuous 2" x 2" x .125" studded aluminum angle (Part 4) at the
head and sill, using 1/4 " -20 machine bolt and wingnut spaced at 6.0" o.c., this angle is secured
with 1/4 -20 machine bolt and wingnut @ 6" o.c. to a 2.25" x 5.25" max. x .125" aluminum
angle (Part 5) used to provide the buildout and which is anchored to the wall in accordance to
the fastener schedule, as shown on Drawing No. 94 - 152, Section C "Build - Out Mount" Sheet 2
of 4, bearing the Dade County Product Control approval stamp.
4.1.3 Ceilina/Inside Mount
The panels are secured inside a top track "U" header (Part 3) at the head, which is anchored to
the inside of the opening, and fastened at the sill, using 1/4 " -20 studs w /washered wingnut
spaced at 6.0" o.c. to the continuous 2" x 2" x .125" studded aluminum angle (Part 4) which is
anchored to the slab or bulkhead below, as shown on Drawing No. 94 -152, Section B,
"Ceiling/Inside Mount" Sheet 2 of 4, bearing the Dade County Product Control approval stamp.
4.1.4 Anv combination of the above.
4 .2. Anchorage of the aluminum angles shall be to concrete, masonry and/or wood framing and shall
consist of only the anchors, embedment and spacing specified on "Anchor Schedules" and
Details on Drawing No. 94 -152, Sheets 1, thru 4 of 4. Substitution of anchor type or indicated
embedment is unacceptable and requires to be engineered separately.' Fasteners must have their
own Notice of Acceptance.
4.3 Permanent set fastener components, embedded anchor bolts, threaded cones or metal shields, not
in use, must be protected against corrosion, contamination and damage at all time.
5. IDENTIFICATION
5.1 Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Dade County Product Control Approved ".
Also a Warning Note shall be permanently stamped in each panel advising the home owner or
tenant that the storm panels will not offer hurricane protection unless all reinforcing bolts are
properly installed.
6. USE
6.1 Application for building permit shall be accompanied by two copies of the following:
6.1.1 This Notice of Acceptance.
6.1.2 Completely . dimensioned drawing showing size and location, including height above
grade, of openings to receive shutters; mean roof height, length and width of building.
6.1.3 Duplicate prints of approved drawings No. 94 -152 , Sheets 1, 2, 3, and 4, bearing the
approval stamp, Notice of Acceptance number and date by Dade County
Product Control Section, clearly marked to show the components selected for the
proposed installation.
6.2 The Building Official shall ensure the adequacy of shutter to meet the pressure requirement of
the opening in which it is to be installed.

NOTICE OF ACCEPTANCE STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documentation, including test supporting data, engineering documents, are no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Dade County Product Control Approved ", or as specifically stated in the specific conditions of
this Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of
this product and the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation
initially submitted , is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be -
cause for termination of this Acceptance, unless prior written approval has been requested (through the filing of a
revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
other purposes.
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the expiration date
may be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed, then it shall be
done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be provided to
the user by the manufacturer or its distributors and shall be available for inspection at the job site at all times. The
copies need not be resealed by the engineer.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.
9. This Acceptance contains pages 1, 2, 2(a) through 2(h) and this last page 3.
Items 10,11 & 12 listed below only apply to glazed products and doors
10. Unless specifically indicated in the Acceptance (approval), this unit-is approved as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is required from the Product Control Section.
11. The spacing of fasteners at window sills shall be as tested. The spacing of fasteners in all other parts of the frame,
shall be as tested, but in no case shall exceed 24" on center. The first fastener shall be located at a maximum of 6"
from each corner and mullion or stile. Fastener shall fully penetrate the buck, which shall be the same size as the one
tested with the unit. No wood or plastic shields or pins shall be used for anchoring. See section four (4) of this
approval for sizes of fasteners used in testing.
12. Hardware for all windows and doors shall conform to Security and Forced Entry Prevention, Chapter 36 of the South
Florida Building Code.

Code, 1994 Edition for Dade County, for the locations where the pressure requirements, as determined by
ASCE 7 -88 "Minimum Design Loads for Building and Other Structures ", do not exceed the Design
Pressure Rating values in Section 7 and within the limitations contained in Section 3.

1.3 Overall Size: 15'- 5" wide by 7' -11 7/8" (95 7/8 ") high x 4" deep.

at sill.
2.3.4 Corner construction: Cornersare butt joined, frame sill corners are secured with three #10
x 1" SMS, Frame head corners are secured with four #10 x 1" SMS and each fixed light corner is
secured with four #10 x 1" SMS.
2.4 Panel Construction:
2.4.1 Stiles: Consist of 1 3/4" deep x 4 1/8" face x .078" wall thickness extruded aluminum tube
with a 1/2" x 1/2" pocket for glazing and a built -in door stop is used at stiles
2.4.2 Rails: A 1 3/4" deep x 4 1/8" face x .078" wall thickness extruded aluminum tube with a
1/2" x 1/2" pocket is used at top and bottom rails.

1. DESCRIPTION
1.1 This approves an 18 ga. galvanized steel storm panel designed to comply with the South Florida
Building Code Impact Test Requirement and .SCE 7 -88 "Minimum Design Loads for Buildings
and Other Structures ", for the following locations:
1.1.1 Non- coastal building zone.
1.1.2 Coastal building zone.
2. MAATERLAL CHARACTERISTICS
2.1. The storm panels shall be constructed as indicated on approved drawing No. 94 -152, Sheets 1 of
4 using 18 ga. steel (.045" thick) structural quality grade C, conforming to ASTM A446.
Panels are rolled formed, have a nominal width of 12.00" and a total width of 13.625 ", forming
2" deep ribs
2.3 Components shall be aluminum alloys 6063 -T6, with shapes and thicknesses as shown on
approved drawing.
2.4 Tested specimen:
- Dimensions: 3 panels totaling 37 1/2" wide x 101" long overall, corresponding to 96" span.
Dimensions: 3 panels totaling 37 1/2" wide x 137" long overall, corresponding to 132" span.
3. LIMITATIONS
3.1 The design will be dependent on the maximum panel height of 101" and 137 ", to be installed
only as outlined on drawing No. 94 -152 Sheets 1 thru 4 of 4.
3.2 Minimum separation to existing glass to protect shall be:
3.2.1 For panels up to 101" high: 1 7/8 ".
3.2.2 For panels over 101" high and up to 137" high: 2 7/8 ".
3.3 Maximum clearance between top of panel and inside of header shall be 1/4 ".
3.4 Special Requirement:
All panels with spans from 137" to 101" high, require 1/4 " -20 x 1" sidewalk bolts w /die cast
aluminum washered wingnuts spaced at 1/3" of the span along all overlaps.
All panels with spans from 101" to 33" high, require a 1/4 " -20 x 1" sidewalk bolt w /die cast
aluminum washered wingnut at midspan at all overlaps.
Panels less than or equal to 33" high do not require fasteners at overlaps.
See Drawing No.94 -152 for detail of requirements.
4. INSTALLATION
4.1 The panels shall be installed only in the following condition:
4.1.1 Wall Mount (Direct Mount)
The panels are secured to an "h" header (Part 2) or bolted, using 1/4 " -20 machine bolt &
wingnut at 6" o.c., to a continuous 2" x 2" x .125" studded aluminum angle (Part 4) at the head;
and using this same angle at the sill; all are anchored to the wall, using the tension anchors as
shown on Drawing No. 94 -152, Sections A , D and E "Wall Mount" Sheet 2 of 4, bearing the
Dade County Product Control approval stamp.

4 1.2 Buildout Mount
The panels are fastened to the continuous 2" x 2" x .125" studded aluminum angle (Part 4) at the
head and s111, using 1/4" -20 machine bolt and wingnut spaced at 6.0" o.c., this angle is secured
with 1/4 -20 machine bolt and wingnut @ 6" o.c. to a 2.25" x 5.25" max. x .125" aluminum
angle (Part 5) used to provide the buildout and which is anchored to the wall in accordance to
the fastener schedule, as shown on Drawing No. 94 -152, Section C "Build -Out Mount" Sheet 2
of 4, bearing the Dade County Product Control approval stamp.
4.1.3 Ceiling/Inside Mount
The panels are secured inside a top track "U" header (Part 3) at the head, which is anchored to
the inside of the opening, and fastened at the sill, using 1/4 " -20 studs w /washered wingnut
spaced at 6.0" o.c. to the continuous 2" x 2" x .125" studded aluminum angle (Part 4) which is
anchored to the slab or bulkhead below, as shown on Drawing No. 94 -152, Section B,
"Ceiling/Inside Mount" Sheet 2 of 4, bearing the Dade County Product Control approval stamp.
4.1.4 Anv combination of the above.
4.2 Anchorage of the aluminum angles shall be to concrete, masonry and/or wood framing and shall
consist of only the anchors, embedment and spacing specified on "Anchor Schedules" and
Details on Drawing No. 94 -152, Sheets 1, thru 4 of 4. Substitution of anchor type or indicated
embedment is unacceptable and requires to be engineered separately. Fasteners must have their
own Notice of Acceptance.
4.3 Permanent set fastener components, embedded anchor bolts, threaded cones or metal shields, not
in use, must be protected against corrosion, contamination and damage at all time.
5. IDENTIFICATION
5.1 Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Dade County Product Control Approved ".
Also a Warning Note shall be permanently stamped in each panel advising the home owner or
tenant that the storm panels will not offer hurricane protection unless all reinforcing bolts are
properly installed.
6. USE
6.1 Application for building permit shall be accompanied by two copies of the following:
6.1.1 This Notice of Acceptance.
6.1.2 Completely . dimensioned drawing showing size and location, including height above
grade, of openings to receive shutters; mean roof height, length and width of building.
6.1.3 Duplicate prints of approved drawings No. 94 -152 , Sheets 1, 2, 3, and 4, bearing the
approval stamp, Notice of Acceptance number and date by Dade County
Product Control Section, clearly marked to show the components selected for the
proposed installation.
6.2 The Building Official shall ensure the adequacy of shutter to meet the pressure requirement of
the opening in which it is to be installed.

Extension of Acceptance may be considered after a new application has been tiled and the
supporting data, test report no older than ten (10) years, have been re- evaluated.
All reports of re- testing shall bear the seal, signature and date of an engineer registered in the
State of Florida.
2. Any revision or change in the materials, use, or manufacture of the product or process shall
automatically be cause for termination, unless prior approval is granted for revisions or change.
3. Any unsatisfactory performance of this product or process or a change in Code provisions shall
be ground for re- evaluation.
