In accordance with your request and authorization, a representative of LG Engineering Services,
Inc. completed the Roof Tile Uplift Test for the rear addition at the above referenced project.
This testing was performed in general accordance with Roofing Application Standard RAS No.106
-- Standard procedure for field verification of the bonding of mortar or adhesive set tile system
and mechanically attached, rigid, discontinuous roof systems.
The total of the tested roof surface area was less than 5000 square feet, and the mean height of
the roof is less than 20 feet above ground surface. The type of tile used for this project was
reported to be Santa Fe Roof Tile. This tile was reported to have been foamed in place.
At the time of this testing, one test for every corner, one test for every 100 square feet of roof tile
in the perimeter area and one test for every 200 square feet of roof tile in the field area were
tested. Initially the entire area of the roof was examine for loose tile. More than 10,% of the roof
tile located in the field area and more than 20 % of the roof tile at the roof perimeter= and. Corner
were physically examined.

As it pertains to this Appendix "F', it is the responsibility of the roofing contractor to provide the owner with the required
roofing permit, to provide the owner with this appendix and to explain to the owner the content of this form. The provisions
of Chapter 34 of the South Florida Building Code (SFBC) govern all the minimum requirements and standards of the indus-
try for roofing system installations. Additionally, the following items should be addressed as part of the agreement between
the owner and the contractor. The owner's initial in the adjacent box indicates that the item has been explained.
gCommon Roofs: Common roofs are those which have no visible delineation between neighboring units (i.e. town-
�
houses, condominiums, etc.). In buildings with common roofs, the roofing contractor and/or owner should notify the
occupants of adjacent units of roofing work to be performed.
APPENDIX "F"
REQUIRED OWNERS NOTIFICATION FOR ROOFING CONSIDERATIONS
. Aesthetics - Workmanship: The workmanship provisions of Chapter 34 are for the purpose of providing that the roof-
ing system meets the wind resistance and water intrusion performance standards. Aesthetics (appearance) issues are
not a consideration with respect to workmanship provisions. Aesthetic issues such as color or architectural appearance,
that are not part of a zoning code, should be addressed as part of the agreement between the owner and the contractor.
. Renailing Wood Decks: When replacing roofing, the existing wood roof deck may have to be renailed in accordance
with the current provisions of Chapter 29 of the SFBC. (The roof deck is usually concealed prior to removing the
existing roof system.)
Exposed ceilings: Exposed, open beam ceilings are where the underside of the roof decking can be viewed from
below. The owner may wish to maintain the architectural appearance, therefore, roofing nail penetrations of the under-
side of the decking may not be acceptable. The SFBC provides the option of maintaining this appearance.

. Ponding Water: The current roof system and/or deck of the building may not drain well and may cause water to pond
(accumulate) in low -lying areas of the roof. Ponding can be an indication of structural distress and may require the
review of a professional structural engineer. Ponding may shorten the life expectancy and performance of the new roof-
ing system. Ponding conditions may not be evident until the original roofing system is removed. Ponding conditions
should be corrected.
. Overflow Scuppers (wall outlets): It is required that rainwater flow off so that the roof is not overloaded from a build
up of water. Perimeter /edge walls or other roof extensions may block this discharge if overflow scuppers (wall outlets)
are not provided. It may be necessary to install overflow scuppers in accordance with Chapter 23 of the SFBC.
. Ventilation: Most roof structures should have some ability to vent natural airflow through the interior of the structur-
al assembly (the building itself). The existing amount of attic ventilation shall not be reduced. It may be beneficial to
consider venting which can result in extending the service life for the roof.

2. For nail -on applications, fasteners for mechanical attachment of tiles shall have a head diames—

3. System installation shall be in compliance with the system specifications outlined in this Prod's=
Control Approval. The method of attachment utilized shall provide a minimum characteristic
resistance load greater than or equal to the required uplift resistance determined in compliance" .rr
Metro-Dade County Protocol PA 115 or PA 127. The Ladrillera Santafe "Santafe S' tile proftt`
been tested for both wind characteristics and static uplift performance, therefore, attachment
calculations for installation in compliance with PA 115 or PA 127 shall be done as an 'Moment
Based System'
4. For mortar and adhesive set tile applications, a field static uplift test by a Metro-Dade Counr-
accredited testing agency, in compliance with Metro-Dade County Protocol PA 106, shall be
performed.
5. All tiles shall bear the imprint or identifiable marking of the manufacturer's name or logo for
identification in the field.
6. For mortar or adhesive set tile applications, 30/90 hot mopped underlayment applications masa
installed perpendicular to the roof pitch prior to June 1, 1995 unless stated otherwise by the mats` —.am
manufacturer. Thereafter, 30/90 underlayment systems shall be installed parallel to the roof pit= -
compliance with Appendix 'A' of Dade County Protocol PA 120.
7. The applicant shall retain the services of a Metro-Dade County certified testing laboratory to
maintain quality control in compliance with the South Florida Building Code and related prote_

NOTICE OF ACCEPTANCE STANDARD CONDITIONS
Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed
and the original submitted documentation, including test supporting data, engineering documents, are
no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city,
state, and the following statement: "Dade County Product Control Approved ". or as specifically
stated in the specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this
product and the product is not in compliance with the code changes:
b) The product is no longer the same product (identical) as the one originally approved:
c) If the Acceptance holder has not complied with all the requirements of this acceptance.
including the correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially
submitted , is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been
requested (through the filing of a revision application with appropriate fee) and granted by this
office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process:
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
other purposes.
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance
is displayed, then it shall be done in its entirety.
A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall
be provided to the user by the manufacturer or its distributors and shall be available for inspection at
the job site at all times. The copies need not be resealed by the engineer.

Polyfoam Products, Inc., produces two - component expanding polyurethane adhesive for the attachment
of clay and concrete roof tile. Polypro® AH160 is specially designed for application of concrete and
clay roof tile over a variety of roof deck substrates and underlayment materials.
Polypro® adhesive is applied with proprietary Foampro® RTFI000 dispensing equipment. Approval of
this product is specific to the two component mix and Foampro® RTFI000 dispensing equipment.
Alternative methods of dispensing will require a revision to this approval.
Polypro® adhesive shall be applied only by 'Qualified Applicators', licensed by Polyfoam Products, Inc.
Certificates and identification cards are issued to applicators upon successful completion of a application
training program. Approval of this adhesive system is contingent upon application by a 'Qualified
Applicator.

COMPONENT APPLICATION
1. Polypro®D AH160 tile adhesive shall only be applied over an underlaymcnt installed in compliance
with application instructions detailed in its NOA.. Underlayment may he any underlayments or
underlayment systems listed Miami -Dade County Roofing Application Standard PA 120.
2. Adhesive application shall be in accordance with the instructions provided herein. Tthe
manufacturer may provide additional instructions subsequent to this approval date which may
prevail. Installation must be by a Factory Trained 'Qualified Applicator' approved and licensed by
Polyfoam Products, Inc.
3. Calibration of the FoamprolD dispensing equipment is required before application of any adhesive.
The mix ratio between the "A" component and the "B" component shall he maintained between 1.0-
1.15 (A) : 1.0 (B). The dispense timer shall he set to deliver 0.08 to 0.15 pounds per tile as
determined at calibration. No other settings shall be approved without revision of NOA.
4. Adhesive shall be dispensed in a 17 in' to 23 in pad at time of application. The cured pad shall be
approximately 5" x 12 ". Foam placement shall be in accordance with the 'Placement Details'
herein. Each generic tile profile requires a specific placement, as noted in Table 1, below
5. Adhere tile directly in freshly applied adhesive. Tile must be set within 2 to 3 minutes aftcr foam
has been dispensed.

2. Polypro® A11160 adhesive shall be installed only by 'Qualified Applicators' approved and licensed
by Polyfoam Products, Inc. •
3. For adhesive set tile applications, a field static uplift test by a Miami -Dade County accredited
testing agency, in compliance with Miami -Dade County Roofing Application Standard PA 106,
shall be required not less than 72 hours after application to confirm tile adhesion.
4. Refer to application drawings published by the National Tile Manufacturers Association for
penetrations and terminations, as well as the 'Placement Details' included with NOA.
5. Refer to tile Roof Tile Assembly Product Control NOA for system acceptance with this Component.
6. Refer to the Prepared Roof Tile Assembly for fire rating.
7. Polypro® A1 shall not be exposed permanently to sunlight.
8. Polypro0 AH160 shall be applied with Foampro RTF 1000 dispensing equipment only.
9. Polyfoam Products Inc. shall supply Building Official with a list of 'Qualified Applicators' approved

I Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and thc
original submitted documentation, including test supporting data, engineering documents, are no older than
eight (8) years.
2 Any and all approved products shall be permanently labeled with thc manufacturer's name, city, state, and the
following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3 Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance,, including the
correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted,
is no longer practicing the engineering profession.
4 . Any revision or change in the materials, usc, and/or manufacture of the product or process shall automatically
be cause for termination of this Acceptance, unless prior written approval has been requested (through the
filing of a revision application with appropriate fee) and granted by this office.
S Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6 The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and followed by thc
expiration date may be displayed in advertising literature. if any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7 A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the job site
at all times. The copies need not be resealed by the engineer.
8 Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.

In accordance with your request and authorization, a representative of LG Engineering Services,
Inc. completed the Roof Tile Uplift Test at the above referenced project. This testing was
performed in general accordance with Protocol PA 106 -94 -- Field static uplift test for mortar or
adhesive set tile systems and mechanically attached, rigid, discontinuous roof systems.
The total of the tested roof surface area was less than 5000 square feet, and the mean height of
the roof is less than 15 feet above ground surface. The type of tile used for this project was
reported to be Santa Fe Roof Tile. This tile was reported to have been foamed in place.
At the time of this testing, a total of forty -five (45) tests were conducted; ten (10) at the roof
perimeter and ten (10) at the roof corner and twenty -five (25) in the field area. More than 5%
of the roof tile located in the field area and more than 10% of the roof tile at the roof perimeter
were physically examined.

Lap base sheet 4" and attach to the wood deck with approved nails and tin caps 9"
o.c. at the sidelap and two staggered rows 12" o.c. centered on the sheet.
Ply 4 and Ply 6® applications require sheathing paper over wood decks before
application of anchor sheet.

6. In no case shall the fastener requirments for recover be less than the .
requirements for the deck type inthe: new construction approvals.
7. GAF Materials Corporation requires .tearoff of the existing roof over
lightweight insulating concrete dec4 except as determined on a job - to-job
basis.

1. All asphalt shall comply with ASTM D 312 type :or, :type° P/ requirements. All
proposed asphalt producers must be approved by.the'applicant.
2. Fire ratings are determined by a combination of slope,' ;deck ;type; and assembly. • Refer to current Underwriters Laboratories Roofing: Materials Directory'or other fire
testing data listed by an approved listing laboratory. agency. Fire ratios shall be in
compliance with ' Sections 3401.5 and 3401.6 of the South Florida • Building • Code.
3. Maximum slope range shall vary for each system; consult. current Underwriters
Laboratories Roofing Materials Directory and manufacturer's � `spedifcafions for
compliance with design criteria for each project.
4. An overlay /recovery board insulation panel is required on all applications over
closed cell foam insulations when the base sheet is fully mopped. If no recovery
board is used the base sheet may be applied using spot mopping: with. approved
asphalt, 12" diameter circles, 24" o.c.; or strip or spot mopped 8" ribbons in three
rows, one at each sidelap and one down the center of the sheet allowing a continuous
area of ventilation. Encircling of the strips is not acceptable. A 6" break shall be
placed every 12' in each ribbon to allow cross ventilation. Asphalt application of
either system shall be at a minimum rate of 12.lbs.sq.. Note maximum design
pressure restrictions on spot attached systems.
5. Any amendments to these provisions shall be in compliance with Sections 203 and
204 of the South Florida Building Code.
6. All work shall be performed by a Contractor licensed to do roofing work in Dade
County. Contractor shall be familiar with the details and specifications published by
the manufacturer.
7. In re- roofing applications, moisture content in an existing built -up roof must be in
compliance with Section 3401.9(c) of the South Florida Building Code.
8. Prior to application, all existing roof surfaces used as a bonding substrate shall* be
tested for uplift resistance in compliance with Dade County Protocol PA 124 to the
calculated design pressure of the roof deck.
If required, any Factory Mutual Approved vapor barrier in .conjunction with
proprietary or approved adhesives may - - be — used = - prior ' to = the = application of -the -
insulation layer.
10. Perimeter and corner areas shall have an increase . in fastener density: -for both
insulation and base sheet in compliance with Dade County .PA-
South Florida Building Code.
11. All attachment and sizing of perimeter nailers and metal *file' designs shall
conform with Dade County Protocol PA 111 of the South Florida Buil ;; g;C. de.

12. Flashings shall be installed according to the manufacturers standard: details, and may
be applied in cold application adhesive, approved asphalt or may be apphed in
conjunction with an approved torch applied modified. bitumen membrane. Specific .
details, approved by the manufacturer, shall be submitted with the Section II Permit
Application.
13. Roll good materials shall be stored in strict compliance with GAF Materials
Corporation requirements.
14. Consult current Underwriters Laboratories Directory: forth appropriate = coating for
each roofing assembly to obtain the required fire , rating ° :The assembly shall be
installed in strict compliance with Chapter 3401.5 and 3401..6 of the 'South Florida
Building Code for maximum fire classification.
15. Asphalt moppings shall be with applied with asphalt approved by : the .applicant and
shall be approved in compliance with equiviscous temperature (EVT) methods of
asphalt application. Asphalt containers or bulk tickets shall indicate an . EVT,
finished blowing temperature (FBT) and flash point. Asphalt types and temperature

16. Insulation may be installed in multiple layers. The first layer shall be attached in
compliance with Product Control Approval guidelines. All other layers shall be

17. Pre - assembled cap nails may only be used• for termination of fully bonded surfaces
such as mechanical termination at wood:sblocking, head and side laps, and base
flashing terminations. Cap nails shall not be used for the mechanical attachment of a
base sheet. Tin caps are required by the South Florida Building Code.
18. The submission of system specifications and details shall accompany the Section II
Building Code Permit. This information is available in the publications listed in the
'System Description' of this Product Control Approval. The submission of these
documents, as well as the proper application and installation of all 'materials shall be
the sole responsibility of the contractor.
19. All standard panel sizes are acceptable for_mechanical_attachment.When- applied -in—
approved asphalt, panel size shall be 4' x 4' maximum.
20. Fastener spacing for base sheet attachment is based on a mininitun withdrawal
resistance value of 250 lbf as tested in compliance with Dade County Protocol PA
105. If the fastener values as tested are below 250 lbf a professional engineer may
submit a revised fastener spacing utilizing the withdrawal resistance. value taken
from Dade County Protocol PA 105. Calculations shall be based on a rtipture value
