We have been retained by the owners of 1285 N.E. 101 Street, Miami Shores, to provide
special inspector services as defined in section 305.3 of the S.F.B.C. The repairs are to
be performed by Robbins Gunite Service under contract to the owner. The repairs will
consist of the following:
1. Gunite repair to approximately 265 lineal feet of concrete grade beam.
2. Gunite repair to approximately 100 lineal feet of concrete slab soffit.
1. Remove all loose and spalling concrete using chipping hammer.
2. Inspect exposed interior reinforcing steel and replace any steel damaged in excess of
10 %. Should laps be required, at least 48 bar diameters are required as per ACI 318-
95. Steel is to be 60 KSI. Concrete cover to be a minimum of 1.5 inches as per ACI
318- 99.7.7
3. Sandblast existing exposed rebar and coat with Armatec 100 or equivalent.
4. Gunite to original dimension.
In progress inspections will be performed and a final inspection letter will be forwarded
upon completion.

DO NOT BEGIN ANY WORK WITHOUT HAVING RECEIVED YOUR VALIDATED PERMIT AND PERMIT CARD. Applying for a permit does not grant
the right to begin construction. HOURS OF CONSTRUCTION are limited to: Monday through Friday from 7:30 a.m. to 6:00 p.m., and Saturday from 8:00 a.m.
to 5:00 p.m. No inspections will be conducted on weekends or holidays.
2. All construction of demolition areas MUST BE MAINTAINED IN A CLEAN, NEAT AND SANITARY CONDITION free from construction debris.
3. STREETS AND NEIGHBORING PROPERTIES SHALL BE KEPT FREE FROM DIRT AND DEBRIS.
4. SWALES MUST BE PROTECTED FROM BEING DAMAGED BY EQUIPMENT OR VEHICLES, AND MAY NOT BE USED FOR STORAGE. A bond is
required for work in or near the street/sidewalk.
5. CONSTRUCTION TRAILERS ARE PROHIBITED ON SINGLE FAMILY RESIDENTIAL CONSTRUCTION SITES. Other construction may have a trailer
which requires a separate permit.
6. PORTABLE TOILETS for a construction site require a separate permit.
7. DO NOT DISCHARGE WATER INTO THE RIGHT OF WAY OR STORM DRAINS without approval from the Building, Planning and Zoning Department.
8. EQUIPMENT AND MATERIALS SHALL BE STORED at least 10 feet from the edge of pavement.
9. Department of Health and Rehabilitative Services (HRS) approval is required for applications involving septic tanks. Department of Environmental Resources
Management (DERM) and/or Miami -Dade Water and Sewer Department (MDWASD) approval is required for applications involving sewers.

Application is hereby made to obtain a permit to do work and installation as indicated. I, the OWNER of the property, certify that all
work will be performed to meet the standards of all laws regulating construction in this jurisdiction. I understand that separate permits
are required for ELECTRICAL, PLUMBING, POOL, EXTERIOR DOOR, MECHANICAL, WINDOW, FENCE, DRIVEWAY,
ROOFING and SIGNS and there may be additional permits required from other governmental agencies.
I, the OWNER of the property, have disclosed all information related to any work at the property performed in the prior twelve
months to the Building Official. Further, I am fully aware that if the cumulative cost of work to my home or business under this and
any other permit equals or exceeds fifty percent (50 %) of the fair market value of the structure, the entire structure must meet the
present federal flood criteria for finished floor elevation. I am also fully aware that if the total cost of work to my home or business
under this and any other permit exceeds fifty percent (50 %) of the replacement cost of the structure, then the entire structure must
conform to the current code requirements of the Building Code.
WARNING TO OWNER: Your failure to record a NOTICE OF COMMENCEMENT may result in you paying twice for
improvements to your property. If you are spending more than $2,500 or intend to obtain financing, you may wish to consult with
your attorney or lender before recording your Notice of Commencement. The Notice of Commencement must be recorded at:

When Forms and Steel are in Place for the Pouring of any
concrete used in construction of any building.
4. Concrete.
A. The minimum amount of cement allowed per cubic yard of
concrete will be six sacks.
• B. The maximum amount of Hater per sack of cement will be six
gallons.
C. Forms must be used on all Foundation Trenches.
D. Forms on Foundations, Foundation Walls, Tie Beams - must not
be removed for twenty -four hours after finishing the pouring
of concrete. Shore Jacks and Forms under unsupported beams
must remain in place one day for each lineal foot of beam;
in no case can they be removed under ten days without
permission from Building Inspector.
Poured Slabs: Forms and Jacks must remain in place for a
period of not less than twenty -one days after pouring
concrete.
E. Mixing Concrete:
The mixing of concrete ingredients on the job must be done
by weight or measurement; shovel measure will not be
permitted.
F. Transit Mix Concrete:
If Transit Mix Concrete is usod, the Building Inspector must
be furnished with a report from a properly qualified testing
laboratory, certified in the presence of a Notary Public,
showing the results of tests of the product to be used,
Certificate to be furnished at time pouring is started.
G. Steel in all foundations must bo suspended in place by wires;
the use of rock, brick or other materials to support steel
will not be permitted. Steel must be at least 2" from
bottom of trench, and laps must be forty tines the diameter
of the steel. All steel must bo securely wired in place.
All steel must be covered by at least 2" of Concrete.

5. Continuous Spread Footings:
FOUNDATION WALLS
A. The bottom of all footings shall be at least six inches
below natural grado - Grade to be taken at the lowest point
of ground in foundation line.
Exterior 1 Story 10 x 24 Concrete 3 - 5/8 Bars

at right angles 3 ft. o.c.

No masonry or wood construction will be permitted on foundat-
ion for 36 hours after finishing the pouring of concrete.
Steel used in all concrote must be freo from rust and scale.

B. Sidewalks and Driveways:
All sidewalks outside of property line shall be of concrete
and shall not be less than 4" thick, and shall be of the
standard 5 ft. width, using concrete containing not less
than 6 sacks of cement per cu. yd. and no more than 6 gal.
of water per sack of cement. If topping is used, the slab
shall be 3" thick plus 1" of topping, topping to be not less
than one part cement and two parts of clean, sharp sand.
Driveway ribbons and slabs shall be a minimum of 6" thick,
ribbons over 10 ft. in length shall be reinforced with 2 -
lsi rods, rods to be set a maximum of 18" apart with 3/8"
bars at right angles every 18 ". Slab Driveways and garage
floors shall be reinforced by 6x6 mesh wire the entire width
and length of slab.
6. All two story masonry buildings shall havo concrete corner
columns 8x12x12 with 5 - 5/8 rods with hoops not less than
1/4" IS spaced not more than one foot on centers, with inter -
mediate columns not more than 16 ft. apart reinforced with
4 - 5/8 rods and hoops to be spaced not more than one foot
on centers. Columns shall be dowelled to the footing and
be continuous from footing to top of plate beam or parapet.
7. All buildings shall have continuous concrote wall tie beams
placed at second floor and roof lino with a depth of at least
12 ", reinforced with 4 - 5/8 rods, two placed at the top and
two at the bottom.
8. Run a continuous concrote girt at least eight inches high by
width of wall, reinforced with 2 - 1/2" rods on top of all
parapet walls.
9. All gables that have a span of 12 foot or over, or contain more
than 25 sq. ft., shall have a tic beam not loss than 8" high
by the width of wall, reinforced with 2 - 1/2" rods and anchored
to the tie beam with 2 - 1/2" dowels.
All gables are to be poured before the placing of any roof
sheathing.
10. Clear a space under joist, level off the high places in the
grade under the house and provide at least a 1'8" cloar space
between the bottom of joist and grade under the houso.
11. Vents in Foundation Wall:
Vents must be placed near the corners except on street elevat-
ions when registered architect plans otherwise. Vents includ-
ing access openings must be installed to provide a minimum of
2 sq. ft. of clear area for each 15 lin. ft. of outside wall.
Screen all vents and accesses with #16 copper screen wire.
All vents shall have 8x8 concrete lintel over it reinforced
with 2 - 1/2" bars. Openings must also be left in interior
bearing walls for cross ventilation.
12. Joists and Rafters:
All floor and roof framing, except ornamental rafters, must
be long leaf yellow pine, or equal. Exterior wall studs in
frame buildings must be long leaf yellow pine or equal and
shall not be less than 2x6's.
13. Floor Joists:
A. No wooden floor joists used on any building shall be less
than 2x8 nor spaced more than 16" on centers.
2x8 joists shall not exceed o. 10 ft. span.
2x10 not to exceed a 15' span.
2x12 not to exceed an 18' span on 16" centers.

Run rows of bridging not to exceed 8 ft. apart - Bridging
to be not less than 1x4.
B. Frame double joists under all partitions running in the
same direction as joists. Frame for hearth 20" out from
finished face of fireplace and at least 12" wider than
fireplace opening on each side. Frame from chimney and
other openings double header trimmers and spiking pieces.
Keep all timbcr2" off from face of chimney walls in all
cases. Use; metal lath and furring on chimney walls; no
wood will be permitted.
Heatilators may be used in lieu of fire brick.
14. Rafters shall not be less than 2x6's.
C. All Chimneys and fireplaces to be built of brick, using
a minimum of 8" red clay brick from bottom up to the flue
lining, with an additional 4" fire brick in the fire chamber.
A minimum of 8" cement brick may be used around the flue
lining; chimneys to be lined with fire clay flue lining
the full height.
Rafters for tile roofs:
2x6's shall not exceed a 10' clear span
2x8's shall not excced a 16' clear span
2x10's - 16' to 20' clear span - every 3rd rafter to have
collar ties and bracing. No braces to be fastened to
ceiling joist except over partitions.
Cathedral Ceilings - Rafters shall be not less than 4x6, placed
not more than 24" o.c.
15. Ceiling oists:
A. Ceiling Joists shall be not less than 2x6's. 2x6 shall not
exceed a 12' span. 2x8's shall not exceed 18'.
B. Furring strip% to be set at least 3/8" off all masonry walls.
The wood coming in contact with masonry must be treated
with a wood preservative before placing on the wall.
Furring strips must have fire stops at floor line.
16. A. Plates shall be anchored to the beam with 1/2'k 12" bolts.
For 2x4 plates, bolts to be 2' -8" o.c., for 2x6 plates 4'
o.c. Set approved anchors, properly placed in each of the
concrete beams to anchor each fourth floor joist, ceiling
joist and rafter.
B. All partitions coming against masonry walls shall be anchor-
ed at cap plate with joist anchor set in tie beam.
C. All studs must have fire stops half way between floor line
and ceiling joist. Openings 3' or over in all bearing
partitions must be trussed.
17. Placing sills, girders and plates - All sills, girders or plates
shall have a level uniform bearing on foundation walls where
necessary to provide a suitable bearing, a cement grout shall
be uniformly placed under the girders, plates or sills. The
use of wood wedges, shingles or small blocks of wood will not
be acceptable construction.
18. Concrete Slabs and Steps:
Where forms for slabs or steps are used, the slabs shall be of
the suspended type and all wood form work shall be removed.
The space under the slab shall be well ventilated and an open-
ing of ample size shall be provided to give access. Where a
slab is poured on wood joists in the bath room it shall be not
less than 3" thick reinforced with wire mesh, and shall be
poured before any fixtures are placed. No openings shall be
left under bath tub or other fixtures.
19. Wood sub - floors - The placing of a wood sub -floor for all wood
joist construction will be required. The 'sub-floor shall be
laid diagonally with diagonal cut and joints overbearing. Cut
in 2x4's to support sub -floor between joist at outside walls.
20. Roof Sheathing - All roof sheathing to be of 1x6 tongue and
grooved, left free of projecting nail heads, splinters etc.
All butt joints to be over rafters.
Storm Sheathing - Storm Sheathing must be tongue and grooved
1x6 or 1x8 applied diagonally parallel with wind bracing, laid
solid, from top plate to bottom plate, and must be well nailed
to both plates and studs.
21. Treated timbers:
Plates bearing on masonry and the ends or parts of all floor
joists, sills or girders or other wood coming in contact with
masonry shall bs treated with an approved wood preservative;
the brushing on of the preserver after these timbers are built
in place will not be acceptable treatment.
22. Access Opening and Scuttle
An opening of ample size shall be provided to give access to the
space under the house. Use a cement sill at this opening to
prevent surplus water from running under the house. A scuttle
or opening of ample size shall be provided in the ceiling of
a closet or hall to give access to the attic space.
23. All windowsills shall be poured concrete at least 6" deep by
width of wall reinforced wi t -2-1/2" rods. Sills to extend
6" on either side of opening..
24. When garage is attached to house, the wall between house and
garage shall be of fire proof construction of masonry or metal
lath and cement plaster; when metal lath and plaster are used
the studs must be not less than 2x6's.
25. A. Consult with both Building and Plumbing Inspectors prior
to framing bath room floors and thus avoid excessive cutting
on account of plumbing pipes. The running of plumbing pipes
through or into any concrete beams or lintels will not be
permitted without firstconsulting Building and Plumbing
Inspectors.
B. Plumbing - All cast iron plumbing pipe must be the extra
heavy. The use of standard grade pipe will not be permitted.
All Septic Tanks must be filled with water for twenty -four
hours before an inspection will be made.
26. Consult Building Inspector regarding arch and special beam
reinforcing rods.
27. All window and door frames must be properly anchored to masonry.
28. All gas, gasoline or kerosene heaters shall have a vent, the vent
to be carried through the roof.
29. No frame stucco buildings will be permitted. Use of frame stucco
as a portion of exterior wall as masonry construction, when
considered by Building Inspector to be unavoidable, will be
permitted. In this case storm sheeting, a 30# felt shall be
applied, then stucco is applied.
30. Roofing: Tile Roofs
A. Drying in - Cover all roof surface with 30# felt laid
according to manufacturer's specifications with 4" lap, tin
capping entire surface at 12" centers. Over this lay 90#
slate surface roofing, mop solid with 4" laps.
B. Drip Strip - Copper drips shall be required on evens and
rakes where sheathing does not project 6" from face of wall.
Omit on rake where barrel tile is rolled over edge or where
shingle tile starting pieces are used.
C. Barrel Tile - Pan tile shall be spaced so that covers will
lap pan tile at least 2" on each side at each end of tile.
Pan tile shall lap length -wise at least 2" and cover tile
shall lap 3 ". Special mortar shall be used to insure that
all tile are firmly embedded. A one part cement - two parts
clean, sharp sand is recommended.
D. Flat Shingle Tile. The third course of shingles shall lap
the first course of shingles 2 ".
E. Inter- locking tile to be laid according to manufacturer's
specifications.
F. Where concrete roofing tile is used, the Building Inspector
must be furnished certificate from approved testing laborat-
ory showing tile has passed the following tests:
Aggregates
The aggregates should be well graded and free from harmful
amounts of soluble salts, organic matter and other deleter-
ious material which might produce efflorescence. Particular
are should be taken in selecting the source of aggregate
Shingle type tile shall be subjected to the Modulus of
Rupture and Absorption tests, essentially as outlined in
A. S. T. M.
Standard Method of Testing Brick, Serial Designation
C 67 -37.

and its weathering qualities fully determined. All material
used in concrete tile should pass the 1 /4t screen. The mix
used should not be leaner than one part of Portland Cement
to three parts of well- graded, durable sand. If it is found
that a mix of one part of Portland Cement to throe parts of
sand produces a harsh mix, the proportion of sand shall be
reduced so that a workable mix can be obtained.
Mixing
Mixing shall be mechanical and continue for not less than
three minutes after all materials and water are in the mixer.
Curing
After manufacture, the tile shall be placed on racks and
permitted to harden before they are stacked. The tile shall
be stacked as soon as possible, but in no case shall they
be permitted to remain on the racks for a period longer
than twenty -four hours. After the tile are stacked, they
shall be covered with damp burlap, which shall be kept
continuously moist for a period of ninety -six hours. Tho
burlap can then be removed and the tile allowed to dry slow-
ly under cover, protected from the sun, for a poriod not
less than fourteen days (including curing period.) No tile
is to be shipped for a period of less than twenty -eight days
after manufacture (including curing period.) The tile may
be cured for the same periods of time by using a water spray
or steam curing in place of tho wet burlap. All curing shall
be done under a roof or shed where tile are protected from
tho sun and wind. Alternate wetting and drying of the tile
during curing period is prohibited. All curing shall be
done at a temperature not lower than 60 degrees Fahrenheit.
Testing of Tile
All roofing tile manufactured in accordance with those
specifications, when tested in the following manner, shall
meet the following requirements:
The average modulus of rupture shall be not less than
300 pounds per square inch, with no individual specimen
less than 250 pounds per square inch.
The absorption shall average not more than 12 %, with no
individual specimen exceec'ing 14%.
Mission Type Tile shall be tested for crushing strength and
absorption by a method modelled from A .S . T. M. Standard
Specification for Concrete Sower Pipe, Serial Designation
C. 14-35.
The Crushing strength shall average not loss than 350 pounds
por lineal foot of tile, with no individual specimen being
less than 250 pounds per lineal foot of tile.
The absorption shall average not more than 12%, with no
individual specimen exceeding 14 %.
All tests shall be made by a recognizes' testing laboratory.

33. Lathing
All corners and angles :oust have metal lath 4" each way.
A certificate of the results of tests for roofing tile for
all jobs using cement tile shall be furnished to the Build-
ing Inspector before the placing of any tile on roof.
Cement
Portland cement shall be used in the manufacture of concrete
roofing tile and shall meet the requirements of the current
Standard Specification for Portland Cement adopted by the
American Society for Testing Materials.
31. Sol -r Tanks - Permits must be obtained before any installation,
all work must be inspected. Before covering up, tank and coils
must have pressure turned on 24 hours before inspection.
A. Solar tank boxes may be wood or masonry. If wood is used the
studs must be not less than 2x4's - Sir',ing or Sheathing must
be tongued and grooved - Bottom of Box must be tongued and
grooved. Box to be mopped inside and outside with hot
asphalt and covered with 30# felt on the inside, outside
with a 90 lb. slate surface roofing; over paper use 1x2
furring strips and metal lath before plastering with cement
plaster.
B. Tank to have drain line in bottom, run to outside of build-
ing with sill cock or valve, for flushing purposes.
C. Coils must have drain line at lowest point of coil for flush-
ing or draining of coil.
32. All outside stucco work shall be applied in minimum of two
operations, first a scratch coat and then a finish coat. At
least one day must elapse between the coats. The finish coat
must contain integral wrterproofing material, used according
to manufacturer's directions. All walls to be thoroughly
wet down before any plastering is done.
Application is hereby made for the approval of the plans and
specifications herewith submitted for the building or other
structure herein described. This application is made in com-
pliance and conformation with the Building Ordinance of the
Miami Shores Village, Florida, and all provisions of the Laws
of the State of Florida, all Ordinances of Miami Shores
Village, and all rules and regulations of the building
Department of Miami Shores Village, shall be complied with,
whether specified or not.
