Please be advised that I am under contract with Superior Stamp Concrete, President
Raymond Perez as of 'March 15th, 2006: He was to replace my driveway and extend the
back patio. The job was to be completed 1 -112 weeks later as_per the verbal agreement
of Mr. Perez to myself.
My driveway was excavated on March 31st, 2006. On April 3rd I spoke with Mr. Perez to
inquire about the inspection. He rushed me off the phone and subsequently did not return
any of my calls or messages for 2 weeks.
After a failed inspection on April 14th, 2006, Mr. Perez has not returned to fix the problem
or proceed- with any work.
!have filed a formal complaint and spoke to Mr. Peter Villa, Investigator at the Miami -Dade
Building Code & Compliance office on Thursday, April 27th, 2006.
As per Mr. Villa, Mr. Perez from Superior Stamp Concrete agreed to continue with the work
and complete the job with a final approved inspection date of May 8th, 2006.
On Friday, April 28th, 2006 Mr. Perez had his employee remove all papers from my
property. These papers included: Original Permit, Survey Plans, Notice of
Commencement, Failed Inspection from April 14th, 2006.
Besides the obvious concern that Mr. Perez will not complete the job, I also have 2 other
concerns at this point
*Mr. Perez has removed all permit papers from my property without my agreement
and he may not return them.
*lam in fear of getting sited and having additional monetary loss by having to
replace the permits, re- inspection fees and having to hire another contractor.
I wanted you to be aware of the events occurring on my property. I also plan to send a
more detailed complaint letter against Superior Stamp Concrete in the efforts to prevent
any of - my. neighbors .in.Miami Shores from. having the same negative-experience that I am
having with this company.

I. To maintain and repair, when necessary, the above- mentioned item(s) installed within the dedicated right
of way. if it becomes necessary for Miami Shores Village or Dade County to make repairs or maintain said
items within public right of way including restoration of street by reason of the Owner's failure to do so,
such expense shall be paid by the Owner or shall constitute a Tien against the above described property
