This door is approved for a maximum design pressure of +-42psf & -48psf and its use is
restricted to this pressure. The door passed the impact & cyclic test, therefore it does not
require hurricane protection devices. The door shall meet the burglary security
requirement of Section 3603.2(c)2(aa) of the South Florida Building Code. The door
operating mechanism (not included in this approval) shall be certified by Underwriters
Laboratories or other recognize agency.
3. Installation:
The door shall be installed as shown on drawings #S0020015. Wood in contact with
concrete or cement shall be treated with an approved preservative as set forth in
paragraph 2913.2(a) 1 or shall be of a durable species as defined in subparagraph
2913.2(a)6(aa) of the South Florida Building Code. Installation of 2" x 6" wood jam
requires a' /," dia dynabolt sleeve anchor and 1" 0'1 thick washer @a 18" o/c with a
minimum embedment of 1' /a' into concrete or grout filled concrete blocks.

4. Conditions_of_Use;
Application for building permit shall be accompanied by duplicate prints of the approved
drawings #S0020015 bearing the approval stamp, Notice of Acceptance number and date
by the Dade County Product Control Section along with computations prepared by a
qualified Florida Professional engineer or architect verifying the adequacy of the door at
each site the shop drawings shall bear the signature of the architect or engineer with the
following statement: "The supporting structure has been designed for the forces shown on
the approved manufacturer's drawing."
Application for building permit for replacement doors shall be accompanied by duplicate
prints of the approved drawing #S0020015 bearing the approval stamp, Notice of
Acceptance number. Also the Building Official must be satisfied that the propose
installation meets the use listed on Section 2 above.
5. Identification:
All approved products shall be permanently labeled with the manufacturer's name, city,
state and the following statement: "Dade County Product Control Approved."

NOTICE OF ACCEtTANCE: STANDARD CONDITIONS
1. Extension of Acceptance may be considered after a new application has been filed and
the supporting data, test report no older than ten (10) years, have been re- evaluated.
All reports of re- testing shalt bear the seal, signature and date of an engineer registered in
the State of Florida.
Any revision or change in the materials, use, or manufacture of the product or process
shall automatically be cause for termination, unless prior approval is granted for
revisions or change.
3. Any unsatisfactory performance of this product or process or a change in Code provisions
shall be grounds for re- evaluation.
4. This acceptance shall not be used as an endorsement of any product for sales or
advertising purposes.
5. The Notice of Acceptance number preceded by the words Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. if any portion
of the Notice of Acceptance is displayed, then it shalt be done in its entirety.
6_ A copy of this approval as well as approved drawings and other documents, where it
applies, shall be provided to the applicant by the manufacturer of his distributors and
shall be available for inspection at the job site at all time. The prints need not be
- re- sealed by the engineer.
7. Failure to comply with Standard Conditions shall be cause for termination of Approval.
