NOTICE OF ACCEPTANCE• SPECIFIC CONDITIONS
1. This approves an aluminum (alloy 3003 -H16) or steel (F = 40 ksi min.) storm panel designed
for the following locations:
2. The design will be dependent on the design load /span panel performance chart outlined on
drawing No. 93 -51 Sheet 2 of 2 and the anchor selection charts and corresponding spacing
charts cbntained in drawing No. 93 -51 Sheets 1 and 2 of 2.
3. The storm panels shall be installed as indicated on approved drawing No. 93 -51 Sheets 1 and 2
of 2.
Components shall be 6063 -T6 aluminum alloys as indicated on drawing.
The panels shall be constructed and installed in accordance with approved detailed drawing
No. 93 -51 Sheets 1 and 2, bearing the Dade County Product Control stamp.
4. Each unit shall bear a permanent label with the manufacturer's name or logo.
5. Application for building permit shall be accompanied by two copies of the following:
a. Completely dimensioned drawing showing : ize and location, including height above
grade, of openings to receive shutters.
b. Duplicate prints of approved drawings No. 93 -51, Sheets 1 and 2, bearing the approval
stamp, Notice of Acceptance number and date by Dade County Product Control
Section, clearly marked to show the components selected for the proposed installation.

6. Anchorage of the shutters shall be to concrete, masonry and /or wood framing and shall con ±ist
of only the anchors specified on drawing No. 93 -51, Sheet 1 and the corresponding spacing
charts on Sheet 2. Substitution of anchor type or indicated embedment is unacceptable and
requires to be engineered separately.
7. Permanent'set fastener components, embedded anchor bolts, threaded cones or metal shields,
not in use, must be protected against corrosion, contamination and damage at all time.

NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1. Extension of Acceptance may be considered after a new application has been filed and the
supporting data, test report no older than ten (10) years, have been re- evaluated.
All reports of re- testing shall bear the seal, signature and date of an engineer registered in the
State of Florida.
2. Any revision or change in the materials, use, or manufacture of the product or process shall
automatically be cause for termination, unless prior approval is granted for revisions or
change. 4
3. Any unsatisfactory performance of this product or process or a change in Code provisions shall
be ground for re- evaluation.
4. This acceptance shall not be used as an endorsement of any product for sales or advertising
purposes.

5. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the Notice
of Acceptance is displayed, then it shall be done in its entirety.
6. A copy of this approval as well as approved drawings and other documents, where it applies,
shall be provided to the applicant by the manufacturer or his distributors and shall be available
for inspection at the job site at all time. The prints need not be re- sealed by the engineer.
7. Failure to comply with Standard Conditions shall be cause for termination of Approval.

In conformance with Ordinance # 93 -141 and effective for all permits obtained on or after May
30, 1994:
• Windload Standards
The requirements of Section 6 of Standard 7 -88 of the Ame;'ican Society of Civil Engineers
(ASCE 7 -88) entitled "Minimum Design Loads for Buildings and other structures" are hereby
adopted as the minimum standard for the South Florida Building Code.
• Jmpact Lba$ Test Requirement
In conformance with Ordinances #93 -14 and 1193 -141, stating that unless exterior walls
components including but not limited to structural glazing, doors and windows of enclosed
buildings are specifically designed and constructed to preserve the enclosed building envelope
against wind pressures as set forth in Section 2309, and impact loads as set forth in Section 2315
of the South Florida Building Code, all such components shall be protected by an engineered
systems such as storm shutters, panels, etc. which shall comply with both ASCE 7 -88 and the
Impact Test Loads per Protocol PA 201 -94 followed by Cyclic Wind Pressure Loading per
Protocol PA 203 -94.
Manufacturers of hurricane protection systems (storm panels, etc.) holding a current approval
under ASCE 7 -88 must resubmit as a revision, along with a fee of $300.00, a satisfactory test
report indicating compliance with the Impact Test Loads pe Protocol PA 201 -94 followed by the
Cyclic Wind Pressure Loading per Protocol PA 203 -94, to continue their current approval
through its recorded expiration date.
Those applications currently in process, without the impact and cyclic tests performed will be
approved for a period ending May 29, 1994, but may be extended for the full three (3) year
period from the approval date upon completion and passing of the impact and cyclic tests.
