In accordance with your request and authorization, a representative of LG Engineering Services,
Inc. completed the Roof Tile Uplift Test at the above referenced project. This testing was
performed in general accordance with Roofing Application Standard RAS No.106 -- Standard
procedure for field verification of the bonding of mortar or adhesive set tile system and
mechanically attached, rigid, discontinuous roof systems.
The total of the tested roof surface area was less than 5000 square feet, and the mean height of
the roof is less than 20 feet above ground surface. The type of tile used for this project was
reported to be Flat Roof Tile. This tile was reported to have been foamed in place.
At the time of this testing, one test for every corner, one test for every 100 square feet of roof tile
in the perimeter area and one test for every 200 square feet of roof tile in the field area were
tested. Initially the entire area of the roof was examine for loose tile. More than 10% of the roof
tile located in the field area and more than 20 % of the roof tile at the roof perimeter and corner
were physically examined.

REQUIRED OWNERS NOTIFICATION FOR ROOFING CONSIDERATIONS
As it pertains to this Appendix "F ", it is the responsibility ui" uie rooting contractor to provide Litt o.‘t:•■ what the
required roofing permit, to provide the owner with this appendix and to explain to the owner the content of this
form. The provisions of Chapter 34 of the South Florida Building Codc (SFBC) govern all the minimum
requirements and standards of the industry for roofing system installations. Additionally. the- following items
should be addressed as part of the agreement between the owner and the contractor. The owner's initial in the
adjacent box indicates that the item has been explained.
1. Aesthetics WorknranshiP: The workmanship provisions of Chapter 34 are for the purpose of
roviding that the roofing system meets the wind resistance and water intrusion perform:met standards.
Aesthetics (appearance) issues are not a consideration with respect to workmanship provisions. Aesthetic issues
such as color or architectural appearance, that are not part of a zoning code, should be addressed as part of the
agreement between the owner and the contractor.
2. Renailing Wood Decks: When replacing roofing. the existing wood roof deck may have to be renailed
accordance with the current provisions of Chapter 29 oldie SFBC. (The roof deck is usually conce:.led_ prior to
removing the existing roof system)
3. Common Roofs: Common root's are those which have no visible delineation between nci�hboring
nits (i.e. townhouses, condominiums, etc.). in buildings v.ith common roofs. the roofing contractor and /or
owner should notify the occupants of adjacent units of roofing work to be performed.

System Description
Monier Roof Tile is a member of the Redland Group, a world wide producer of primary building
products and roof tile. Monier manufacturers concrete roof tile for nail -on, mortar set or adhesive set
applications in plants throughout the United States ranging from Washington State to Florida. All tile is
manufactured from extruded concrete consisting of Portland Cement, plasticizer, iron and metallic
oxides, and blended aggregates. This Produ ct_Control Aonroval s ates to Monicr's "Bermuda Flat" tile
profile. Refer to at:orop fate Product Control Approvals for other the p of leg.
Monier sells tile systems through local distribution and directly to consumers. A 11 profiles have
matching trim pieces used for rake hip, ridge hip, and valley terminations. Tile system accessories such
as underlayment, ventilation systems and flashing systems are also available. These accessories are
manufactured for all profiles and form a part of this Product Control Approval.
Monier's "Bermuda Flat" roof tile has been tested in compliance with the South Florida Building
Code requirements for concrete, mortar or adhesive set tile applications. The minimum roof slope for
Monier's "Bermuda Flat" mortar or adhesive set tiles shall be 2 ":12 ". Sec the "Profile Drawing" section
in this approval for the "Bermuda Flat" profile drawing. The Monier "Bermuda flat" tile profile has
been tested for both wind characteristics and static uplift performance, therefore, any consideration for
installation shalt be done as a 'Moment Based System'. Data for attachment calculations is noted in
Tables 1 through 3 of this approval.

Install tile in compliance with PA 120 using one of the approved mortars or
.adhesives noted in this approval. Mortar or adhesive shall be `applied in
compliance with the mortar manufacturers Roofing Component Product
Control Approval. The mortar or adhesive attachment shall provide sufficient
attachment resistance expressed as a moment to meet or exceed the required
moment of resistance determined in compliance with PA 115 or PA 127. The
mortar and adhesive attachment data is noted in Table 3, attached.
1. For re -roof applications, 15 /32" plywood is an acceptable substrate.
2. For mortar set applications, the first three courses of' tile shall be nailed with
not less than one nail per tile. As an alternate, the first three courses of tile
shall be applied in mortar over a single layer of minimum 12 ga. wire mesh
with square openings of not less than 3/8" which is mechanically attached to
the deck with not less than one roofing nail every 1 ft For roof pitches from
6 " :12" to 7 ":12 ", every third tile of every fifth course, beginning at the eighth
course, shall be nailed with not less than one nail per tile..

1. The standard minimum roof pitch for Monier "Bermuda Flat" mortar set or adhesive set tile
applications is 2" rise in 12" run (2";12"). The maximum roof pitch for mortar set or adhesive set
Tile applications is 7" rise in 12" run (7"; 12").
2. Monier's "Bermuda Flat" tile profile has not been tested for nail - on applications, therefore,
System A (Counter - Batten Nail - On Application), System B (Direct Deck Nail - On Application) and
System C (Horizontal Batten Nail - On Application) do not form part of this approval.
3. System installation shall be in compliance with the system specifications outlined in this Product
Control Approval. Mortar set or adhesive set tile shall be attached using an approved mortar or
tile adhesive noted in this approval, the data of which is noted in Table 3 of this Approval. The
method of attachment utilized shall provide sufficient attachment resistance expressed as a moment
to meet or exceed the required moment of resistance determined in compliance with Dade County
Protocol PA 115 or PA 127.
SYSTEM LIMITATIONS
4. For mortar set applications, the first three courses of tile shall be nailed with not less than one nail
per tile. As an alternate, the first three courses of tile shall be applied in mortar or adhesive over a
single layer of minimum 12 ga. wire mesh with square openings of not less than 3 16" which is
mechanically attached to the deck with not Tess than one roofing nail every 1 ft For roof pitches
from 6 ":12" to 7 ": ", every third tile of every fifth course, beginning at the eighth course, shall be
nailed with not less than one nail per tile.
5. For mortar set or adhesive set tile applications, a field static uplift test by a Dade County
accredited testing agency, in compliance with Dade County Protocol PA 106, shall be required not
less than 30 days after application to confirm tile adhesion. The results of this testing shall be
reported to the Building Official and the installer stating that the application has passed or failed PA
106 testing. l f the application fails PA 106 testing, the report shall state which portion of the test
was failed; Category 1 (examination for loose tile) or Category 2 (uplift testing of tile). Subsequent
to testing, the installer may repair not more than 5% of field area tiles and 10% of perimeter area
(i.e. ridge/rake) tilt with approved tile adhesive. The installer shall place art identifiable marking on
each repaired tile for future reference. Any PA 106 re -test shall not include any marked tile.
6. For mortar or adhesive Set tile applications, 30190 hot mopped underlayment applications may be
installed perpendicular to the roof pitch prior to June 1, 1995 unless stated otherwise by the material
manufacturer. Thereafter, 30190 underlayment systems shall be installed parallel to the roof pitch in
compliance with Appendix 'A' of Dade County Protocol PA 120.
7. All tiles shall bear the imprint or identifiable marking of the manufacturer's name or logo for
identification in the field. '
8. The Monier "Bermuda Flat" tile profile has been tested for both wind characteristics and static uplift
performance, therefore, attachment calculations for installation in compliance with Dade County
Protocol PA 115 or PA 127 shall be done as a 'Moment Based System'

9. Applications for rooting permits shall include a completed Section TI of the Uniform Building
Permit, a copy of Monier Roof Tile's current specifications and details, a copy of this Product
Control Approval and a copy of the Product Control Approval of any Roofing Component used in
the proposed tile application. Reference shall be made to appropriate data for the required fire
rating.
10. The applicant shall retain the services of a Dade County certified testing Laboratory to maintain
quality control in compliance with the South Florida Building Code and related protocols. Samples
taken shalt be in compliance with Dade County Protocol PA 112, Appendix 'A'.
11. Any amendments to these provisions shall be in compliance with Sections 203 and 204 of the South
Florida Building Code.

1 Renewal of this Acceptance (approval) shall be considered after a renewal application has been
filed and the original submitted documentation, including test supporting data, engineering
documents, are no older than eight (8) years.
2 Any and all approved products shall be permanently labeled with the manufacturers name, city,
state. and the following statement: "Metro-Dade County Product Control Approved ", or as
specifically stated in the specific conditions of this Acceptance.
3 Renewals of Acceptance will not be considered if
a) There has been a change in the South Florida Building Code affecting the evaluation of this
product and thrproduct is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially
Submitted, is no longer practicing the engineering profession.
4 Any revision or change in the materials, use, endlor manufacture of the product or process shall
automatically be cause for termination of this Acceptance. unless prior written approval has been
requested (through the filing of a revision application with appropriate fee) and granted by this
office.
5 Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
other purposes.
6 The Notice of Acceptance number preceded by the words Metro County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the
Notice of Acceptance is displayed, then it shall be done in its entirety.
7 A copy of this Acceptance as well as approved drawings and other documents, where it applies,
shall be provided to the user by the manufacturer or its distributors and shall be available for
inspection at the job site at all times. The copies need not be resealed by the engineer.
8 Failure to comply with any section of this Acceptance shall be cause for termination and removal
of Acceptance.

System Description
Polyfoam Products, inc., produces two - component expanding polyurethane adhesive for the attachment
of clay and concrete roof tile. Polypro® AH160 is specially designed for application of concrete and
clay roof tile over a variety of roof deck substrates and underlayment materials.
Polypro® adhesive is applied with proprietary Foampro® RTF 1000 dispensing equipment. Approval of
this product is specific to the two component mix and Foampro® RTFI000 dispensing equipment.
Alternative methods of dispensing will require a revision to this approval.
I'olypro® adhesive shall be applied only by 'Qualified Applicators', licensed by Polyfoam Products. Inc.
Certificates and identification cards are issued to applicators upon successful completion of a application
training program. Approval of this adhesive system is contingent upon application by a 'Qualified
Applicator.

COMPONENT APPLICATION
1. Polypro® A14160 tilt adhesive shall only be applied over an undcrlayment installed in compliance
with application instructions detailed in its NOA.. Underlayment may be any undcrlayntents or
underlayntent systems listed Miami -Dade County Roofing Application Standard 1'A 120.
2. Adhesive application shall be in accordance with the instructions provided herein. Tthe
manufacturer may provide additional instructions subsequent to this approval date which may
prevail. Installation must be by a Factory Trained 'Qualified Applicator' approved and licensed by
Polyfoam Products, Inc.
3. Calibration of the Foampro® dispensing equipment is required before application of any adhesive.
The mix ratio between the "A" component and the "B" component shall be maintained between 1.0-
1.15 (A) : 1.0 (R). The dispense timer shall be set to deliver 0.08 to 0.15 pounds per tile as
determined at calibration. No other settings shall be approved without revision of NOA.
4. Adhesive shall be dispensed in a 17 in' to 23 in' pad at time of application. The cured pad shall be
approximately 5" x 12 ". Foam placement shall be in accordance with the 'Placement Details'
herein. Each generic tile profile requires a specific placement, as noted in Table 1, below
5. Adhere tile directly in freshly applied adhesive. Tile must be set within 2 to 3 minutes after foam
has been dispensed.

NOTICE OF ACCEPTANCF, STANDARD CONDITIONS
1 Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documeritation, including test supporting data, engineering documents, are no older than
eight (8) years.
2 Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3 Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the product is not in compliance with the code changes:
b) The product is no longer the same product (identical) as the one originally approved;
c) if the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) Thc engineer who originally prepared, signed and scaled the required documentation initially submitted,
is no longer practicing the engineering profession.
4 Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically
be cause for termination of this Acceptance, unless prior written approval has been requested (through the
filing of a revision application with appropriate fee) and granted by this office.
5 Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6 The Notice of Acceptance number preceded by the words Miami -Dade County. Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7 A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the job sitc
at all times. Thc copies need not be resealed by the engineer.
8 Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.
