This is a report regarding the above referenced individual who was electrocuted on July 22, 1997 at
the home of Edmond Kery, at 961 NE 96 Street, Miami Shores, Fl. At the time of the accident and
his subsequent death, Mr. Santana was in the employ of Metro Air Services, Inc. as an air -
conditioning mechanic.
Mr. Santana's co- worker, Jesus Daniel Marcuello, gave the following report to the Metro -Dade
Police. He and Mr. Santana were replacing an existing air - conditioning air handler with a new one
in the attic crawl space. After making the electrical connections to the new air handler, Mr. Santana
and Mr. Marcuello went from the attic into the residence to turn the electric power on. Upon
returning to the attic crawl space, Mr. Santana came in contact with a live circuit while he was
grounded to either the 1 /2 inch electric metallic tubing or the ceiling- mounted fan that is next to the
crawl space. Mr. Marcuello stated that when he saw Mr. Santana shaking, he turned the power off
and called 911.

Electrocution of Jose Raul Santana
The brother and cousin of the deceased were also present at the scene.
At this time, I inspected the electrical system to the residence starting at the FPL service drop and
ending at the air handler in the attic. The electric service terminated in a meter enclosure and six (6)
circuit Zinsco panel, located on the west side of the residence. The Zinsco panel had one 2 -pole 100
amp breaker for the lighting main, one 2 -pole 60 amp breaker for the a/c condensing unit and air
handler, one 2- pole 30 amp breaker also for the air handler. The 60 amp breaker had #8 conductors,
which are large enough for the condensing unit but not for the air handler. Two #8 and two #14
conductors for the control of the condensing unit feed from this panel to the a/c condensing unit in
a 3 /4 inch conduit. Three #3 conductors, two #8 conductors, two #10 conductors and the two #14
control circuit conductors feed from this panel to the general lighting panel inside a 1 /2 inch conduit.
I was not able to trace how the two #8, two #10 and the two #14 control conductors got from the
general lighting panel to the attic. In the attic, there were two 4 inch square junction boxes with the
two #8 and two #10 conductors feeding into the junction boxes. The two #14 control conductors
were added to the junction box farthest from the air handling unit. Each junction box also contained
a black and white #14 120 volt circuit conductor.
At the junction box closest to the air handling unit, an existing section of flexible metal conduit was
used to connect the conductors in the junction box to the air handling unit. This conduit was
approximately 2 inches short of reaching the new air handling unit. In the junction box the white #14
grounded conductor was spliced onto a black #14 conductor in the flexible metal conduit. The two
#10 conductors were cut off flush with the end of the conduit, and not capped, with the bare ends of
the conductors exposed. The two #8 conductors were connected to the line voltage terminals in the
air handling unit. The two #14 conductors were connected to the 24 volt control circuit terminals
in the air handling unit. No grounding conductor was in place.
In the air handling unit, I observed that the control circuit board showed signs of burning. It is my
opinion that the circuit board shorted out due to having 120 volts applied to it. Since there was no
grounding conductor or continuous metallic raceway system, the #14 conductor circuit did not
recognize a short circuit and trip its breaker. When Mr. Santana returned into the attic, he had to
crawl across the existing fan and E.M.T. conduits which were grounded. He may have reached out
to grab the air handling unit to help pull himself up into the attic, at which time he completed the
circuit from the air handling unit through his body to the grounded surfaces with which he was in
contact. In the confined space that he was in, he could not free himself, which would have opened
the circuit.

Metro -Dade Homicide investigators suspect the deceased person died from. electrocution.
However, an electrician that checked the area could not find the source for the electrical charge
that caused the accident. An autopsy will be performed 07 -23 -97 to establish the cause of death.
Meanwhile Metro Homicide wanted the air conditioning company to leave the work in the inside
of the house alone until the cause of death was clear. If the cause of death is from electrocution,
the investigators will decide whether to examine the area again. The homicide investigator said
the autopsy information
Miami Shores Building and Zoning Department should be notified of the accident. I told the air
conditioner contractor a Miami Shores Building and Zoning official would contact him when
work can resume. This was at the suggestion of the investigator and should be coordinated with
their office.
