1524.1 As it pet tains to this section, it is the responsibility of the roofing contractor to provide the owner
with the required roofing permit, and to explain to the owner the content of this section.. The provisions of
Chapter 15 of the Florida Building Code, Building govern the minimum requirements and standards of the
industry for roofing system installations. Additionally, the following items should be atidressed 0.3 part of the
agreement between the owner and the contractor. The owner's initial in the adjacent box indicates that the
item has been explained.

2. Insulation may be installed In multiple Iayers. The first layer shall be attached in compliance with
Product Control Approval guidelines. All, other layers &hall be adhered in a full mopping of
approved asphalt applied within the F.VT range and at a rate of 20-40 ibs./sq., or mechanically
attached using the fastening pattern of the top layer
3. Al standard panel sizes are acceptable for mechanical attachment. When applied in approved
asphalt, panel size shall be 4'x 4'nuuimum.
4. An overlay and/or recovery board insulation panel is required on all applications over closed cell
fnant insulations when the base shext is fully mopped. if no recovery board is used the IMO sheet
shall be applied using spot mopping with approved asphalt. 12" diameter circles, 24" o.c.; or strip
mopped 8" ribbons in three rows, one at each sickle's and one down the center of the sheet allowing

5. Fastener spacing for insulation attachment is based on a Minimum Characteristic Force (P') value of

6. Fastener spacing for mechanical attaclunent of anchor/base sheet or membrane attachment is based

WHEREAS, Miami Shores Presbyterian Church, a Florida non - profit corporation (the "Owner" or
"Declarant") is the owner of the following described property (the "Property") situated and being in Miami
Shores Village, Florida

running east and west through said Block 61, Miami Shores, Section 2,
according to the Plat thereof, recorded in Plat Book 10 at page 37 of the
Public Records of Miami -Dade County, Florida, and
WHEREAS, the Owner, together with Metro PCS California/Florida Inc., a Delaware Corporation.
(hereinafter "Applicant") with the consent of the Owner, has applied to Miami Shores Village, a Florida
municipal corporation ( "the Village ") for a wireless personal telecommunications services antenna permit,
pursuant to Chapter 22 of the Miami Shores Village Code, and otherwise; and
WHEREAS, the Owner and Applicant desire to provide certain assurances and certifications to the
Village;
NOW, THEREFORE, the Owner and Applicant each jointly declare and certify as follows:
1. Title. Owner and Applicant have provided evidence as to the status of title to the Property to
the Village.
2. Continuous Compliance. The Owner and Applicant hereby covenant and certify that the
wireless personal telecommunications services antenna proposed for the Property will be
made continually compliant w ith all p resent and future promulgated safety I aws, rules a nd
regulations concerning electromagnetic frequency emissions standards promulgated by
governmental authorities having jurisdiction over the Applicant and the operations of the
antenna facility, or similar safety standards for other wireless communications media
transmissions. The Owner and Applicant hereby acknowledge the right of the Village to
require immediate removal of any wireless personal telecommunications services antenna
upon the Property which does not meet such safety rules and regulations.
3. Non - Interference. The Owner and Applicant also covenant and certify that any of
Applicant's wireless personal telecommunications services antennas proposed for the
Property will not interfere with public safety communications, and further, will not
unreasonably interfere with the reception or transmission of television, radio, microwave,
telephone, digital or similar communications signals or receipt of signals of nearby residential
or business residents. The Owner and Applicant acknowledge the right of the Village to
require, if necessary, frequency relocation agreements.
4. Multiple Accommodations. The Owner and Applicant covenant and certify that the design,
structure or installation of the Applicant's wireless personal telecommunications services
antenna facility proposed for the Property will be such as to accommodate at least two (2)
wireless personal telecommunications services antenna facilities of other providers, on a non-

WHEREAS, Miami Shores Presbyterian Church, a Florida non - profit corporation (the "Owner" or
"Declarant') is the owner of the following described property (the "Property") situated and being in Miami

municipal corporation ( "the Village ") for a wireless personal telecommunications services antenna permit,
pursuant to Chapter 22 of the Miami Shores Village Code, and otherwise; and
WHEREAS, the Owner and Applicant desire to provide certain assurances and certifications to the
Village;
NOW, THEREFORE, the Owner and Applicant each jointly declare and certify as follows:
1. Title. Owner and Applicant have provided evidence as to the status of title to the Property to
the Village.
2. Continuous Compliance. The Owner and Applicant hereby covenant and certify that the
wireless personal telecommunications services antenna proposed for the Property will be
made continually compliant w ith all p resent and future promulgated safety I aws, rules a nd
regulations concerning electromagnetic frequency emissions standards promulgated by
governmental authorities having jurisdiction over the Applicant and the operations of the
antenna facility, or similar safety standards for other wireless communications media
transmissions. The Owner and Applicant hereby acknowledge the right of the Village to
require immediate removal of any wireless personal telecommunications services antenna
upon the Property which does not meet such safety rules and regulations.
3. Non - Interference. The Owner and Applicant also covenant and certify that any of
Applicant's wireless personal telecommunications services antennas proposed for the
Property will not interfere with public safety communications, and further, will not
unreasonably interfere with the reception or transmission of television, radio, microwave,
telephone, digital or similar communications signals or receipt of signals of nearby residential
or business residents. The Owner and Applicant acknowledge the right of the Village to
require, if necessary, frequency relocation agreements.
4. Multiple Accommodations. The Owner and Applicant covenant and certify that the design,
structure or installation of the Applicant's wireless personal telecommunications services
antenna facility proposed for the Property will be such as to accommodate at least two (2)
wireless personal telecommunications services antenna facilities of other providers, on a non-
discriminatory basis, after applicant's installation, in the manner depicted on Exhibit "A" which
is attached hereto and made a part hereof, to avoid duplication of the erection of wireless
personal telecommunications services antenna towers throughout the Village. Applicant and

facility at the Northeast Presbyterian Church, located at 602 NE 96 Street, Miami Shores. As
part of a Federally Mandated program, AT &T Wireless is submitting for approval to install a
GPS antenna at this location. With the addition of the GPS antenna, AT &T Wireless customers

AFL Wireless Services has performed a structural evaluation for a proposed GPS antenna mounting
attached on the wall of the Presbyterian Church on 602 NE 96 Street in Miami Shores, Florida.
The proposed GPS antenna is a Symmetricom 58532A that has an effective area of less than 2ft
and a weight of less than 101bs. The antenna will be installed at an elevation of 78' -0" above the
ground level. The wall on which the antenna is to be mounted, is a load bearing wall constructed of
masonry blocks or concrete. The proposed antenna is to be mounted directly on a L5X31/2X1 /4"
angle that is to be anchored to the steeple wall with (2) 3/8"0 Hilti Kwik Bolt II connectors. By
observation, the forces applied on the antenna under the provisions of the Florida building Code
2001 will not exceed the capacity of the proposed anchor bolts. The Code prescribes a 3- second
gust wind speed of 150 mph for the Miami area.
The building and its structural components will withstand the new antenna loads without overstress.
AFL Telecommunications recommends, upon proper installation of the antenna mounting, the
installation of (1) Symmetricom 58532A GPS antenna on the wall of the steeple of the Presbyterian
Church at 602 NE 96 Street in Miami Shores, Florida. The mount shall be installed as detailed in
the construction documents issued by AFL Wireless Services on February 6, 2003.

MACTEC Engineering and Consulting, Inc. (MACTEC), formerly known as Law Engineering and
Environmental Services, Inc. (LAW), has completed a limited evaluation of the effect of the
additional loading caused by certain proposed equipment to be located within a tenant- improved
area at the subject site. We understand that AT &T Wireless Services (AT &T) desires to install new
telecommunications equipment within the tenant - improved area as part of its new UMTS system to
be implemented in South Florida and has requested that we determine if the addition of the
proposed equipment would adversely affect the structural performance of the supporting structural
system.
It is our understanding that AT &T has retained Lucent Technologies (Lucent) to provide turnkey
services for the design and implementation of the UMTS system. We have been retained by Lucent
to provide structural engineering services for the subject site. This assessment was based on
published catalog information, information provided by Lucent, as well as information recorded
during our field survey of the subject site.
Our services were performed in accordance with your Master Service Agreement No.
AT22000176 with LAW dated October 2, 2000 and your purchase order to MACTEC dated July
17, 2003.
Since original structural drawings were not available for our review, we assumed a minimum
design live load of 50 pounds per square foot (psf), which is the minimum recommended design
load live load for normal occupancy rooms in accordance with the Florida Building Code, as well

Any conditions discovered that deviate from the information contained in this report should be
presented to us for our review.
This report has been prepared for the sole purpose of providing a structural assessment of this
tenant - improved area for the loading conditions indicated. It is intended for the exclusive use of
Lucent and AT &T. The information, assumptions and recommendations contained in this report
should not be used by others for any purpose without the express written authorization from

6. UNLESS OTHERWISE NOTED, THE WORK SHALL INCLUDE FURNISHING MATERIAL, EQUIPMENT,

11. THE SUBCONTRACTOR SHALL LEAVE THE PREMISES IN CLEAN CONDITION.
12. ALL CONCRETE WORK SHALL BE DONE IN ACCORDANCE WITH AMERICAN CONCRETE INSTITUTE
(ACI) 301.
13. ANY NEW CONCRETE NEEDED FOR CONSTRUCTION SHALL HAVE A COMPRESSIVE STRENGTH OF
3000 PSI AT 28 DAYS.
14. ALL STRUCTURAL STEEL WORK SHALL BE DONE IN ACCORDANCE WITH AISC LRFD MANUAL OF
STEEL CONSTRUCTION (SECOND EDITION).
15. THE EXISTING CELL SITE IS IN FULL COMMERCIAL OPERATION; CONSTRUCTION WORK BY THE
SUBCONTRACTOR SHALL NOT DISRUPT THE EXISTING NORMAL OPERATION WITHOUT PRIOR WRITTEN
APPROVAL FROM THE CONTRACTOR. ANY WORK ON EXISTING EQUIPMENT MUST BE COORDINATED WITH
THE CONTRACTOR. ALSO, WORK SHOULD BE SCHEDULED FOR AN APPROPRIATE MAINTENANCE

COMMISSION AND NOT INTERFERE WITH NORMAL TRANSMISSION OR RECEPTION OF BROADCAST
RADIO, TELEVISION AND TELEPHONIC SERVICES ENJOYED BY ADJACENT RESIDENTIAL AND
