(MACTEC) performed field observations of new antennas mounted on the existing telecommunications church decorative tower located at the above- referenced site. Ms. Ynocencio regularly performs this kind of work, under the supervision of Mr. Armando J. Alvarez, P.E. The purpose of these observations was to inspect the replacement of existing antennas with new antennas on the decorative tower on the church property. Ms. Ynocencio visually inspected the installed antennas from ground level with the assistance of binoculars, compared them with the approved construction drawings and found them to be in general conformance with these drawings. Based on these observations by Ms. Ynocencio, to the best of our knowledge and belief, the antenna installation was performed in general compliance with the approved construction drawings. (MACI'BC), has completed an evaluation of the effect on loading of the subject antenna support structure created by the addition of new antennas to replace existing antenna arrays. This evaluation was based on published catalog information and information provided by Bechtel. The intent of this report is to assess the effect of the proposed addition upon the supporting structural system. Our services were performed in accordance with our Subcontract No. 24782-002- HC6- A00Y-00004 and your change order dated May 18, 2005. This report summarizes the results of our findings. Our evaluation was based on the following proposed designations and configurations of existing antenna equipment to be replaced as indicated by Bechtel and as depicted in our construction drawings. Furthermore, it is our opinion that the effect of the 3.01 sq. ft. per sector increase in wind area on the structural performance of the antenna mounts and/or supporting building structure will likewise be negligible. Cingular Wireless has twelve (12) antennas installed at 80 feet on the steeple. The proposed modification requires the replacement of nine (9) antennas and the removal of three (3) antennas and associated cables. The existing framework will remain as is. The replacement antennas are manufactured by Kathrein, Inc. model numbers 742 -264; 742 -265 and 800 - 10121. The antennas will be flush mounted and painted to match the building, as before. The work, for which we are seeking approval, will be limited to the antennas alone. Once approved, the modifications will be done by our general contractor C & S Weller Corporation. The special inspection will be done by MACTEC Engineering & Consulting. A special inspection application was submitted to the Village on April 20, 2004 as requested.
