REQUIRED OWNERS NOTIFICATION FOR ROOFING CONSIDERATIONS
As it pertains to this Appendix "F ", it is the responsibility of the roofing contractor to provide the owner with the
required roofing permit, to provide the owner with this appendix and to explain to the owner the content of this
form. The provisions of Chapter 34 of the South Florida Building Code (SFBC) govern all the minimum
requirements and standards of the industry for roofing system installations. Additionally, the following items
should be addressed as part of the agreement between the owner and the contractor. The owner's initial in the
adjacent box indicates that the item has been explained.
1. Aesthetics - Workmanship: The workmanship provisions of Chapter 34 are for the purpose of
providing that the roofing system meets the wind resistance and water intrusion performance standards.
Aesthetics (appearance) issues are not a consideration with respect to workmanship provisions. Aesthetic issues
such as color or arcl:itect::ra! appearance, that are not port of a zoning code, should be addressed as part of the
agreement between the owner and die contractor.
(- ` 2. Renailing Wood Decks: When replacing roofing, the existing wood roof deck may have to be renaited
in accordance with the current provisions of Chapter 29 of the SFBC. (Tire roof deck is usually concealed prior to
removing the existing roof systetn)
t -Q 3. Common Roofs: Common, roofs are those which have no visible delineation between neighboring
units (i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing contractor and /or
owner should notify the occupants of adjacent units of roofing w ork to be performed.
L ' 4. Exposed ceilings: Exposed, open beam ceilings are where the underside of the roof decking; can be
viewed from below. The owner may wish to maintain the architectural appearance, therefore, roofing nail
penetrations of the underside of the decking may not be acceptable. The SFBC provides the option of maintaining
this appearance.
( v S. Ponding \\';ter: The current roof system and /or deck of the building may not drain well and may

cause water to pond (accumulate) in low -lying areas of the roof. Ponding can be an indication of structural
distress and may require the review of n professional structural engineer. Ponding may shorten the life
expectancy and performance of the new roofing system. Ponding conditions may not be evident until the original
roofing system is removed. Ponding conditions should be corrected.
1 0 6. Overflow Scuppers (wall outlets): It is required that rainwater flow off so that the roof is not
overloaded from a build up of water. Perimeter /edge walls or other roof extensions may block this discharge if
overflow scuppers (wall outlets) are not provided. It may be necessary to install overflow scuppers in accordance
with Chapter 23 of the SFBC.
Z . 10 7. Ventilation: Most roof structures should have some ability to vent natural airflow through the interior
of the structural assembly (the building itself). The existing amount of attic ventilation shall not be reduced. It
may be beneficial to consider additional venting which can result in extending the service life of the roof.

For nail -on applications, fasteners for mechanical attachment of tiles shall have a head diameter
larger than that of the preformed holes in the tile.
3. System installation shall be in compliance with the system specifications outlined in this Product
Control Approval. The method of attachment utilized Shall provide a minimum characteristic
resistance load greater than or equal to the required uplift resistance determined in compliance with
Metro -Dade County Protocol PA 115 or PA 127. The Ladrillera Santaf& "Santafe S" tile profile has
been tested for both wind characteristics and static uplift performance, therefore, attachment
calculations for installation in compliance with PA 115 or PA 127 shall be done as an 'Moment
Bascd System'
4. For mortar and adhesive stet tile applications. a tied static uplift test by a Metro -Dade County
accredited testing agency, in compliance with Metro -Dade County Protocol PA 106, shall be
performed.

identification in the field.
6. For mortar or adhesive set tile applications. 30/90 hot mopped underI yment applications may be
installed perpendicular to the roof pitch prior to June 1, 1995 unless stated otherwise by the material
manufacturer. Thereafter, 30190 underlayment systems shall be installed parallel to the roof pitch in
compliance with Appendix 'A' of Dade County Protocol PA 120.
The applicant shall retain the services of a Metro - Dade County certified testing laboratory to
maintain quality control in compliance with the South Florida Building Code and related protocols.

GENERAL LIMITATIONS
All asphalt shall comply with ASTM D 312, type 111 or IV requirements and approved by applicant.
'- Fire ratings are determined by a combination of slo a deck type assembly. Refer to the Underwriters Laboratories Roofing Materials Directory or other ft c data listed in the es file.
Fire ratings shall be in strict compliance with Sections 3401.5 and 3401.6 of the South Florida Building
Code.
3 Maximum slope range varies for each system. Consult the current Underwriters Laboratories Roofing
Materials Directory and manufacturer's specifications for compliance with design criteria for each
project.
4 An overlay and/or recovery.board is required on all applications over closed cell foam insulations when
the base sheet is fully mopped in hot asphalt.
5 All work shall be performed by a contractor licensed to do roofing work in Metro -Dade County.
Contractor shall be familiar with the details and specifications published by the manufacturer and the

requirements of the South Florida Building Code.
The submission of system specifications and details shall accompany the Section 11 Building
Permit. This information is available in the publications listed in the 'System Description' o ft h o s e
Product Control Approval. The submission of these documents, as well as the proper application and

installation of all materials shall be the sole responsibility of the contractor.
Perimeter and corner areas shall comply with the elevated design pressures of these roof areas, as
calculated in compliance with Chapter 23 of the South Florida Building Code. Anchor or base sheet
fastening patterns and/or insulation attachment densities shall be increased in compliance with the
provisions set forth in Metro -Dade County Rooting Application Standard PA 117 to meet these elevated
design pressures.
8 All attachment and sizing ofperimeter nailers, metal profile and/or flashing
conform to Metro -Dade Coun Roofin ,termination designs shall
Chapter 23 ty g Application Standard PA 1 1 1 and the wind load requirements
of Cha
p 3 ofthe South Florida Building Code.
9 Flashings shall be installed according to the manufacturer's standard details and may be applied in cold
application adhesive, approved asphalt or may be applied in conjunction with an approved torch applied
modified bitumen membrane. Specific details, approved by the manufacturer, shall be submitted with
the Section 11 Permit Application. All details shall comply with the provisions of the South Florida
Building Code.
10 Fastener spacing for anchor or be sheet attachment is based on a Minimum Characteristic Resistance
Force (F') as determined through field testing in compliance with Metro -Dade County Protocol PA 105
or PA 105(A). If F', as tested, is less than that listed in the System Limitations, a professional engineer
may 'submit revised fastener spacings utilizing F', taken from a Metro -Dade County Protocol PA 105 or
PA 105(A) test report, and the provisions set forth in Metro •Dade County Roofing Application Standard
PA 117.
I I Fastener density for insulation attachment is based on a Minimum Characteristic Resistance Force (F')
as determined through field testing in compliance with Metro -Dade County Protocol PA 105. 1f:F' as
tested, is less than 275 lbf., insulation attachment shall not be acceptable.
-' Asphalt moppings shall be applied with approved asphalt and shall be in compliance with the
equiviscous temperature (EVT) methods of asphalt determination. Asphalt containers or bulk tickets
shall indicate an EVT, finished blowing temperature (FBT) and flash point. Asphalt types and
temperature ranges shall be in compliance with Subsection 3403.4(A)(3) of the South Florida Building
Code.
13 Insulation may be applied in multiple layers. The first layer shall be attached in compliance with the
Product Control Approval guidelines. All other layers shall be adhered in a full mopping of a roved
asphalt applied within the EVT range and at a rate of 20-40 lbs. /sq., or mechanically attached using the
fastening pattern of the top layer. Refer to manufacturer's literature and/or the NRCA Roofing and
Waterproofing Manual for correct application procedures of insulation panels in approved asphalt or

Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documentation, including test supporting data, engineering documents, are no older than
eight (8) years.
2 Any and all approved products shall be permanently labeled '..vith the manufacturer's name, city, state, and
the following statement: "Metro -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3 Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the.product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted,
is no longer practicing the engineering profession.
4 Any revision or changein the materials, use, and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been requested
(through.theThing ofa revision application with appropriate fee) and granted by this office.
5 Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6 The Notice of Acceptance number preceded by the words Mcu u -DLJc Cuunty, Fluridu, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7 A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the job site
at all times. The copies need not be resealed by the engineer.
8 Failure to comply with any section of this Acceptance shall be cause for termination and removal of

HIGH VELOCITY HURRICANE ZONES REQUIRED OWNERS NOTIFICATION FOR ROOFING
CONSIDERATIONS
1524.1 As it pertains to this section, it is the responsibility of the roofmg contractor to provide the owner with
the required roofing permit, and to explain to the owner the content of this section.. The provisions of Chapter
15 of the Florida Building Code, Building govern the minimum requirements and standards of the industry for
roofing system installations. Additionally, the following items should be addressed as part of the agreement
between the owner and the contractor. The owner's initial in the adjacent box indicates that the item has been
- ,c0 1. Aesthetics - Workmanship: The workmanship provisions of Chapter 15 (High Velocity Hurricane
Zone) are for the purpose of providing that the roofing system meets the wind resistance and water intrusion
performance standards. Aesthetics (appearance) issues are not a consideration with respect to workmanship
provisions. Aesthetic issues such as color or architectural appearance, that are not part of a zoning code, should
be addressed as part of the agreement between the owner and the contractor.
2. Renailing Wood Decks: When replacing roofing, the existing wood roof deck may have to be
renailed in accordance with the current provisions of Chapter 16 (High Velocity Hurricane Zones) of the Florida
Building Code. (The roof deck is usually concealed prior to removing the existing roof system).
3. Common Roofs: Common roofs are those which have no visible delineation between neighboring
units (i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing contractor and/or
owner should notify the occupants of adjacent units of roofing work to be performed.
-O 4. Exposed Ceilings: Exposed, open beam ceilings are where the underside of the roof decking can be
viewed from below. The owner may wish to maintain the architectural appearance, therefore, roofing nail
penetrations of the underside of the decking may not be acceptable. The Florida Building Code provides the
option of maintaining this appearance.
Ponding Water: The current roof system and/or deck of the building may not drain well and may
cause water to pond (accumulate) in low -lying areas of the roof. Ponding can be an indication of structural
distress and may require the review of a professional structural engineer. Ponding may shorten the life
expectancy and performance of the new roofing system. Ponding' conditions may not be evident until the
original roofmg system is removed. Ponding conditions should be corrected.
6. Overflow scuppers (wall outlets): It is required that rainwater flow off so that the roof is not
overloaded from a build up of water. Perimeter /edge walls or other roof extensions may block this discharge if
overflow scuppers (wall outlets) are not provided. It may be necessary to install overflow scuppers in
accordance with the Florida Building Code, Plumbing.
\A/i. Ventilation: Most roof structures should have some ability to vent natural airflow through the
interior of the structural assembly (the building itself). The existing amount of attic ventilation shall not be
reduced. It may be beneficial to consider additional venting which can result in extending the service life of the
roof.

3. LIMITATIONS
3.11 Fire classification is not part of this acceptance. Refer to the Prepared Roof Tile
Assembly for fire rating.
3.2 Polypro® AH 160 shall solely be used with flat, low, & high tile profiles.
3.3 Minimum underlayments shall be in compliance with the Roofing Application
Standard RAS 120.
3.4 Roof Tile manufactures acquiring acceptance for the use of Polypro® AH 160 roof
tile adhesive with their tile assemblies shall test in accordance with PA 101.
3.5 Roof Tile manufactures acquiring acceptance for two paddy placement with the
use of Polypro® AH160 roof tile adhesive with their tile assemblies shall test in
accordance with PA 101 and with section 10.4 as modified herein.

4. INSTALLATION
4.1 Polypro® AH 160 may be used with any roof tile assembly having a current NOA that
lists uplift resistance values with the use of Polypro® AH 160.
4.2 Polypro® AH 160 shall be applied in compliance with the Component Application
section and the corresponding Placement Details noted herein. The roof tile
assembly's adhesive attachment with the use of Polypro® AH 160 shall provide
sufficient attachment resistance, expressed as an uplift based system, to meet or exceed
the uplift resistance determined in compliance with Miami -Dade County Roofing
Application Standards RAS 127. The adhesive attachment data is noted in the roof
tile assembly NOA
4.3 Polypro® AH 160 roof tile adhesive and its components shall be installed in
accordance with Roofing Application Standard RAS 120, and Polyfoam Products, Inc.
Polypro® AH 160 Operating Instruction and Maintenance Booklet.
4.4 Installation must be by a Factory Trained 'Qualified Applicator' approved and licensed
by Polyfoam Products, Inc. Polyfoam Products Inc. shall supply a list of approvg
applicators to the authority having jurisdiction.
4.5 Calibration of the Foampro® dispensing equipment is required before application of
any adhesive. The mix ratio between the "A" component and the "B" component shall
be maintained between 1.0 -1.15 (A) : 1.0 (B). The dispense timer shall be set to
deliver 0.0175 to 0.15 pounds per tile as determined at calibration. No other settings
shall be approved.
4.6 Polypro® .AH 160 shall be applied with Foampro RTF 1000 or ProPack® 30 & 100
dispensing equipment only.
4.7 Polypro® AH 160 shall not be exposed permanently to sunk

4.8 Tiles must be adhered in freshly applied adhesive. Tile must be set within 2 to 3
minutes after Polypro® AH 160 has been dispenscd.
4.9 Polypro® AH 160 placement and minimum patty weight shall be in accordance with
the Placement Details' herein. Each generic tile profile requires the specific. placement
noted herein.
5. LABELING
All Polypro® AH160 containers shall comply with the Standard Conditions listed herein.
6. BUILDING PERMIT REQUIREMENTS

Renewal of this Acceptance (approval) shall bc considered after a renewal application has been filed
and the original submitted documents, including test- supporting data, cngineering documents, are no
oldcr than eight (8) years.
2. Any and all approved products shall bc; permanently labeled with thc manufacturer's narnc, city,
statc, and the following statement: "Miami -Dade County Product Control Approval ", or as
specifically stated in the specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a. There has been a change in the South Florida Building Code affecting the evaluation of this
product and thc product is not in compliance with the code changes.
b. The product is no longer thc same product (identical) as the one originally approvcd.
c. If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of thc product.
d. The engineer who originally prepared, signed and scalcd the required documentation initially
submitted, is no longer practicing thc engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of thc product or process shall
automatically be cause for tcrmination of this Acceptance, unless prior written approval has been
requested (through the filing of a revision application with appropriatc fee) and granted by this
office.

a. Unsatisfactory performancc of this product or process.
b. Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
other purposcs.

by thc expiration date may be displaycd in advertising literature. If any portion of the Notice of
Acccptancc is displayed, then it shall bc done in its entirety.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by the BCCO and accepted by the Building Code and Product
Review Committee to be used in Miami Dade County and other areas where allowed by the Authority Having
Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Division (In Miami Dade County) and/or the ART (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami -Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the High Velocity Hurricane
Zone of the Florida Building Code.
DESCRIPTION: GAF Ruberoid® Modified Bitumen Roof System for Wood Decks.
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved ", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the

WOOD DECK SYSTEM LIMITATIONS:
1 A slip sheet is required with Ply 4 and Flex Ply IM 6 when used as a mechanically fastened base or
anchor sheet.
2. Minimum IA" Dens Deck or Y2 Type X gypsum board is acceptable to be installed directly over the
wood deck.
GENERAL LIMITATIONS:
1. Fire classification is not part of this acceptance, refer to a current Approved Roofing Materials
Directory for fire ratings of this product.
2. Insulation may be installed in multiple layers. The first layer shall be attached in compliance with
Product Control Approval guidelines. All other layers shall be adhered in a full mopping of
approved asphalt applied within the EVT range and at a rate of 20-40 lbs./sq., or mechanically
attached using the fastening pattern of the top layer
3. All standard panel sizes are acceptable for mechanical attachment. When applied in approved
asphalt, panel size shall be 4' x 4' maximum.
4. An overlay and/or recovery board insulation panel is required on all applications over closed cell
foam insulations when the base sheet is fully mopped. If no recovery board is used the base sheet
shall be applied using spot mopping with approved asphalt, 12" diameter circles, 24" o.c.; or strip
mopped 8" ribbons in three rows, one at each sidelap and one down the center of the sheet allowing
a continuous area of. ventilation. Encircling of the strips is not acceptable. A 6" break shall be
placed every 12' in each ribbon to allow cross ventilation. Asphalt application of either system shall
be at a minimum rate of 12 lbs. /sq. Note: Spot attached systems shall be limited to a maximum
design pressure of -45 psf.
5. Fastener spacing for insulation attachment is based on a Minimum Characteristic Force (F' value of
275 lbf., as tested in compliance with Testing Application Standard TAS 105. If the fastener value,
as field- tested, are below 275 lbf. insulation attachment shall not be acceptable.
6. Fastener spacing for mechanical attachment of anchor/base sheet or membrane attachment is based
on a minimum fastener resistance value in conjunction with the maximum design value listed within
a specific system. Should the fastener resistance be less than that required, as determined by the
Building Official, a revised fastener spacing, prepared, signed and sealed by a Florida Registered
Engineer, Architect, or Registered Roof Consultant may be submitted. Said revised fastener spacing
shall utilize the withdrawal resistance value taken from Testing Application Standards TAS 105 and
calculations in compliance with Roofing Application Standard RAS 117.
7. Perimeter and corner areas shall comply with the enhanced uplift pressure requirements of these
areas. Fastener densities shall be increased for both insulation and base sheet as calculated in
compliance with Roofing Application Standard RAS 117. (When this limitation is specifically
referred within this NOA, General Limitation #9 will not be applicable.)
8. All attachment and sizing of perimeter nailers, metal profile, and/or flashing termination designs
shall conform with Roofing Application Standard RAS 111 and applicable wind load requirements.
9. The maximum designed pressure limitation listed shall be applicable to all roof pressure zones (i.e.
field, perimeters, and corners). Neither rational analysis, nor extrapolation shall be permitted for
enhanced fastening at enhanced pressure zones (i.e. perimeters, extended corners and corners).
(When this limitation is specifically referred within this NOA, General Limitation #7 will not
be applicable.)
10. All products listed herein shall have a quality assurance audit in accordance with the Florida
Building Code and Rule 9B -72 of the Florida Administrative Code.
