that to the best of my knowledge and belief, the pressure grouting procedure to the North and
West sides of the reinforced concrete swimming pool deck complies with the permitted
documents. I also attest to the best of my knowledge and belief, the approved permit plans
represent the as -built condition of the structural and envelope components of said structure. This
document is being prepared in accordance with Section 105.13.4.1 of the Florida Building Code
and is being submitted to the Village of Miami Shores building department at the time of final
inspection for the above referenced structure.
