This approves overhead doors 9' -0 wide x 8' -0 thru 6' -6" high with 24 gauge steel skin,
coated with G -60 galvanizing, baked on primer and baked on polyester top coat.
The door shall be constructed and reinforced as detailed in the approved drawing
#101182.

This door is approved for a maximum design pressure of +46psf and -48psf and its use is
restricted to this pressure. The door passed the impact and cyclic test, therefore it does
not require protection devices.
The door shall meet the burglary security requirements of Section 3603.2 (c) 2 (aa) of the
South Florida Building Code.
The door operating mechanism (not included in this approval) shall be certified by
Underwriter Laboratories or other recognized agency.
The door shall be installed as shown on drawing 4101182 or other methods approved by
the professional of record for the structure. Wood in contact with concrete or
cement shall be treated with an approved preservative as set forth in paragraph 2913.2(a)1
or shall be a durable specie as defined in subparagraph 2913.2 (a) 6 (aa) of the South
Florida Building Code. Installation of 2" x 6" wood jam requires a' /," x 3'/," tapcon at
12" o/c for a 9' -0 wide door with a minimum embedment of 1 into the concrete or
reinforced block.

Application for building permit shall be accompanied by duplicate prints of the approved
drawings bearing the approval stamp, Notice of Acceptance number and date by the Dade
Product Control Section along with computations prepared by a qualified Florida
registered engineer or architect verifying the adequacy of the installation of the door at
each site. The shop drawings shall bear the signature of the architect or engineer of record
with the following statement:
"The supporting structure has been designed for the forces shown on the approved
manufacturer's drawings." The adequacy of the overhead door to resist the horizontal
load from the door shall be verified.
5. Identification:
All approved products shall be permanently labeled with the manufacturer's name, city,
state and the following statement: "Dade County Product Control Approved."

NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1. Extension of Acceptance may be considered after a new application has been filed and the
supporting data, test report no older than ten (10) years, have been re- evaluated.
All reports of re- testing shall bear the seal, signature and date of an engineer registered in the
State of Florida.
2. Any revision or change in the materials, use, or manufacture of the product or process shall
automatically be cause for termination, unless prior approval is granted for revisions or change.
3. Any unsatisfactory performance of this product or process or a change in Code provisions shall be
grounds for re- evaluation.
4. This acceptance shall not be used as an endorsement of any product for sales or advertising
purposes.
5. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the Notice of
Acceptance is displayed, then it shall be done in its entirety.
6. A copy of this approval as well as approved drawings and other documents, where it applies, shall
be provided to the applicant by the manufacturer or his distributors and shall be available for
inspection at the job site at all time. The prints need not be re- sealed by the engineer.
7. Failure to comply with Standard Conditions shall be cause for termination of Approval.

In accordance with your request and authorization, a representative of LG Engineering Services, Inc.
completed the Roof Tile Uplift Test at the above referenced project. This testing was performed in general
accordance with Roofing Application Standard RAS No.106 -- Standard procedure for field verification
of the bonding of mortar or adhesive set tile system and mechanically attached, rigid, discontinuous roof
systems.
The total of the tested roof surface area was less than 5000 square feet, and the mean height of the roof
is less than 30 feet above ground surface. The type of tile used for this project was reported to be Santa
Fe Clay "S" Roof Tile. This tile was reported to have been foamed in place.
At the time of this testing, one test for every corner, one test for every 100 square feet of roof tile in the
perimeter area and one test for every 200 square feet of roof tile in the field area were tested. Initially the
entire area of the roof was examine for loose tile. More than 10% of the roof tile located in the field area
and more than 20% of the roof tile at the roof perimeter and corner were physically examined.
Based on our test results, we conclude that the installation ofthe roof tile at the above referenced project
meets the test requirement outlined in the above mentioned protocol. Attached please find a copy of our
test report for your review.
