• 1.1 This approves a 16' wide sectional door as descialed m Sectinn 2 of this Notice of Acceptant,
designed to comply with the South FloridaBufift Code, 1994 Edition for Miami -Dade County
(SFBC) for the location where the pressure requirements as determined by SFBC Chapter 23, do not
exceed the design pressure rating value indicated on the approved drawing.
2. PRODUCT DESCRIPTION
2.1 The DAB 16' wide Sectional Door and its components shall be built in strict compliance with the
following documents: Drawing No. 98-12, titied"Sectional Residential Garage Door", prepared by Al-
Farooq Corporation, dated 04103198, with lalestrevision on 07125198, Sheet 1 through 3 of 3, bearing
the Miami -Dade County Product Control Approval stamp with the Notice of Acceptance number and
approval date by the Miami-Dade Produce Control Division. These documents shall hereinafter be
referred to as the approved drawings.
3. LIMITATIONS
3.1 Units with dimensions equal to or smaller than those shown in the approved drawings shall qualify'
under this approval.
4. INSTALLATION

NOTICE OF ACCEPTANCE STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documentation, including test supporting data, engineering documents, are no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the specific
conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if
a) There has been a change in the South Florida Huglding Code affecting the evaluation of this product and the product
is not in compliance with the code changes;
b) The product is no longer the same product (ideal) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the correct
installation of the produc
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted is no longer
practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be
cause for termination of this Acceptance, unless prior written approval has been requested (through the filing of a
revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process.
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other purpose.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed,
then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be provided to
the user by the manufacturer or its distributors and shall be available for inspection at the job site at all time. The
engineer need not reseal ti a copies.
$. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.

that under the provisions of Ordinance No. 185, dated June 15, 1948, amend-
ing paragraph (d) of Section B -368 of Ordinance No. 97 of the Building Code

that he is the owner of said property, and will be the owner of said

he will, at such times as are required by the Zoning and Building Director: -
1. File plans and specifications and obtain approval of the
Planning Eoard and the Building Inspector.
2. Apply for and secure a Permit.
4 .
3. Pay the required fees.
4. Execute the work in accordance with the provisions of this
Code.
5. Apply for inspections.
6. File with the Building Inspector certificates that provision
has been made to carry the necessary Workmen's Compensation,
Public Liability and Property Damage Insurance.
7. File with the Building Inspector as the job progresses certi-
ficates showing the payment required by the Federal Social
Security Act to the State of Florida or the United States of
America.
8. Assume the responsibility of not employing other than properly
licensed contractors by Miami Shores Village for any part or
portion of the work.
9. . $lot set himself up as a "contractor ".
10. Sign an affidavit before commencing work to the effect that
he has read this Article and will do the work personally and
observe all of the requirements of the Building, Electrical,
Plumbing and Zoning Codes of Miami Shores Village. Such
affidavit to be properly notarized upon blanks to be supplied
by the Building Inspector.
11. In order to prevent abuses and subterfuge the right of such
owner - builder as herein provided, is limited to but one
owner-builder permit each 2 years, and where an owner- builder
has once exercised the privilege herein conferred no second
application for owner- builder permit shall be granted in less
than 2 years, unless the applicant is qualified as a licensee.

Village.
