REQUIRED O \VNERS NOTIFICATION FOR ROOFING CONSIDERATIONS
- As it pertains to this Appendix "F ", it is the responsibility of the rooting contractor to provide the owner with the
required roofing permit, to provide the owner with this appendix and to explain to the owner the content of this
form. The provisions of Chapter 34 of the South Florida Building Codc (SFBC) govern all the minimum
requirements and standards of the industry for roofing system installations. Additionally, the following items
sho:Ild be addressed as part or the agreement between the owner and the contractor. The owner's initial in e
adjacent box indi :aces that the item has been explained.
1. Aesthetics-Workmanship: The workmanship provisions or Chapter 34 are for the purpose of
providing that the roofing system meets the wind resistance and eater intrusion performance standards.
Aesthetics (appearance) issues are not a consideration \vith respect to w'orkmanship provisions. Aesthetic issues
such as color or architectural appearance, that are not part of a zoning code, should be addressed as part or the
agreement between the owner and the contractor.
r 2. Renailing \Vootl Decks: \Vhen replacing roofing, the existing wood roof deck may have to be renai'ed
t accordance with the current provisions of Chapter 29 of the SFBC. (The roof deck is usually concealed prior to
removing the existing roof system)
3. Common Roofs: Common roofs are those which have no visible delineation between neighboring
nits (i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing contractor and /or
owner should notify the occupants of adjacent units of roofing work to be performed.
4. Exposed ceilings: Exposed, open beam ceilings are where the underside of the roof decking can be
vF ved from below. The owner may wish to maintain th�t: architectural appearance, therefore, roofing nail
penetrations of the underside of the decking may not be acceptable. The SFBC provides the option of maintainin2
this appearance.
L 5. Ponding \ Vater: The current roof system and /or deck of the buildinn, may not drain well and may
c se water to pond (accumulate) in low-lying 'areas of the roof. Ponding can be an indication of structural
distress and may require the review of a profess'ionnl structural engineer. Ponding may shorten the life
expectancy and performance of the new roofing system. Ponding conditions may not be evident until the original
roofing; system is removed. Ponding conditions should be corrected.
G. Overflow Scuppers (wall outlets): It is required that rainwater flow off so that the roof is no..
o loaded from a build up of water. Perimeter /edge \valls or other roof extensions may block this discharge i`

overflow scuppers (wall outlets) arc not provided. It may be necessary to install overflow scuppers in accordance
\vith Chapter 23 of the SFBC.
7. \'entilntion: tvlost roof structures should have some ability to vent natural through the

NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1. SCOPE
This revises a roofing system using Monier Lifetile Mission Barrel Concrete Roof Tilc, as
manufactured by Monier Lifetile LLC described in Section 2 of this Notice of Acceptance,
designed to comply with the South Florida Building Code, 1994 Edition for Mituni -Dade
County. For the locations where the pressure requirements, as determined by SF BC
Chapter 23, do not exceed the design pressure values obtain by calculations in compliance
with RAS 127 using the values listed in section 4 herein. The attachment calculations shalt
be done as a moment based system.

3.1 Fire classification is not part of this acceptance.
3.2 For mortar or adhesive set tile applications, a static field uplift test shall be
performed in accordance with RA: 106.
3.3 Applicant shall retain the services of a Miami - Dade County Certified Laboratory
to perform quarterly test in accordance with PA 112, appendix 'A'. Such testing
shall be submitted to the Building Code Compliance Office for review.
3.4 Minimum underlayments shall be in compliance with the applicable Roofing
Applications Standards listed section 4.1 herein.
3.5 30/90 hot mopped underlayment applications may be installed perpendicular to
the roof slope unless stated otherwise by the u tcrrai cturcrs

I. Renewal of this Acceptance (approval) shall be considered after a renewal application has been
filed and the original submitted documents, including test - supporting data, engineering documents.

2. Any and ail approved products shall be permanently labeled with the manufacturer's name, city,
state, and the following statement: "Miami-Dade County Product Control Approval ", or as
specifically stated in the specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a. There has been a change in the South Florida Building Code affecting the evaluation of this
product and the product is not in compliance with the code changes.
b. The product is no longer the same produ,ot (identical) as the one originally approved.
c. If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product.
d. The engineer who originally prepared, signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4. Any revision or change in the materials, use„ and /or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been
requested (through the filing of a revision application with appropriate fee) and granted by this
office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a. Unsatisfactory performance of this product or process.
b. Misuse of this Acceptance as an endorsement of any product. for sales, advertising or any
other purposes.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the
Notice of Acceptance is displayed, then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies,
shall be provided to the user by the manufacturer or its distributors and shall be available for
inspection at the job site at all time. The engineer need not reseal the copies.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal
of Acceptance,
9. This Notice of Acceptance consists of pages I through 3 and this last page 6.

base sheet to wood plank deck. Red rosin sheet may also be installed on plywood decks as an option.

1 All asphalt shall comply with ASTM D 312, type III or IV requirements and approved by applicant.
Fire ratings are determined by a combination of slope, deck type and assembly. Refer o the current
Underwriters Laboratories Roofing Materials Directory or other fire testing data listed n the testing file.
Fire ratings shall be in strict compliance with Sections 3401.5 and 3401.6 of the South Florida Building
Code.
Maximum slope range varies for each system. Consult the current Underwriters Laboratories Roofing
Materials Directory and manufacturer's specifications for compliance with design criteria for each
project.
An overlay and/or recovery board is required on all applications over closed cell foam insulations when
the base sheet is fully mopped in hot asphalt.
5 All work shall be performed by a contractor licensed to do roofing work in Metro -Dade County.
Contractor shall be familiar with the details and specifications published by the manufacturer and the
requirements of the South Florida Building Code.
6 The submission of system specifications and details shall accompany the Section 11 Building Code
Permit. This information is available in the publications listed in the 'System Description' of this
Product Control Approval. The submission of these documents, as well as the proper application and
installation of all materials shall be the sole responsibility of the contractor.
7 Perimeter and comer areas shall comply with the elevated design pressures of these rooflareas, as
calculated in compliance with Chapter 23 of the South Florida Building Code. Anchor or base sheet
fastening patterns and/or insulation attachment densities shall be increased in compliance with the
provisions set forth in Metro -Dade County Roofing Application Standard PA 117 to meet these elevated
design pressures.
S All attachment and sizing of perimeter nailers, metal profile and/or flashing termination designs shall
conform to Metro -Dade County Roofing Application Standard PA III and the wind load requirements
of Chapter 23 of the South Florida Building Code.
9 Flashings shall be installed according to the manufacturer's standard details and may be applied in cold
application adhesive, approved asphalt or may be applied in conjunction with an approved torch applied
modified bitumen membrane. Specific details, approved by the manufacturer, shall be submitted with
the Section 11 Permit Application. All details shall comply with the provisions of the South Florida
Building Code.
10 Fastener spacing for anchor or base sheet attachment is based on a Minimum Characteristic Resistance
Force (F') as determined through field testing in compliance with Metro -Dade County Protocol PA 105
or PA I05(A). If F', as tested, is Tess than that listed in the System Limitations, a professional engineer
may 'submit revised fastener spacings utilizing F', taken from a Metro -Dade County Protocol PA 105 or
PA l05(A) test report, and the provisions set forth in Metro -Dade County Roofing Application Standard
PA 117.
I 1 Fastener density for insulation attachment is based on a Minimum Characteristic Resistance Force (F')
as determined through field testing in compliance with Metro -Dade County Protocol PA 105. If•F', as
tested, is less than 275 lbf., insulation attachment shall not be acceptable.
12 Asphalt moppings shall be applied with approved asphalt and shall be in compliance with the
equiviscous temperature (EVT) methods of asphalt determination. Asphalt containers or bulk tickets
shall indicate an EVT, finished blowing temperature (FBT) and flash point. Asphalt types and
temperature ranges shall be in compliance with Subsection 3403.4(A)(3) of the South Florida Building
Code.
13 Insulation may be applied in multiple layers. The first layer shall be attached in compliance with the
Product Control Approval guidelines. All other layers shall be adhered in a full mopping of approved
asphalt applied within the EVT range and at a rate of 20 -40 lbs. /sq., or mechanically attached using the
fastening pattern of the top layer. Refer to manufacturer's literature and/or the NRCA Roofing and
Waterproofing Manual for correct application procedures of insulation panels in approved asphalt or

14 All standard insulation panels sizes are acceptable for mechanical attachment. When pplied in
approved asphalt, panel size shall be 4' x 4' maximum.
15 In re -covery applications, prior to.the application, all existing roof surfaces used as a b ndino

Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documentation, including test supporting data, engineering documents, are no older than
eight (8) years.
7 Any and all approved products shall be permanently labeled With the manufacturer's name, city, state, and
the following statement: "Metro -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3 Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation init'ally submitted,
is no longer practicing the engineering profession.
4 Any revision or change in the materials, use, and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been requested
(through the filing of a revision application with appropriate fee) and granted by this office.
5 Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
purposes.
6 The Notice of Acceptance number preceded by the words Metro -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7 A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the job site
at all times. The copies need not be resealed by the engineer.

APPENDIX "F "REQUIRED OWNERS NOTIFICATION FOR ROOFING CONSIDERATIONS
As it pertains to this Appendix "F ", it is the responsibility of the roofing contractor to provide the owner
with the required roofing permit, to provide the owner with this appendix and to explain to the owner the
content of this form. The provisions of Chapter 34 of the South Florida Building Code (SFBC) govern
the minimum requirements and standards of the industry for roofing system installations. Additionally,
the following items should be addressed as part of the agreement between the owner and the contractor.
The owner's initial in the adjacent box indicates that the item has been explained.
a 1. Aesthetics- Workmanship: The workmanship provisions of Chapter 34 are for the purpose of
providing that the roofing system meets the wind resistance and water intrusion performance standards.
Aesthetics (appearance) issues are not a consideration with respect to workmanship provisions. Aesthetic
issues such as color or architectural appearance, that are not part of a zoning code, should be addressed as
part of the agreement between the owner and the contractor.

2. Renailing Wood Decks: When replacing roofing, the existing wood roof deck may have to be
re ailed in accordance with the current provisions of Chapter 29 of the SFBC. (The roof deck is usually
concealed prior to removing the existing roof system)
' 3. Common Roofs: Common roofs are those which have no visible delineation between
ne �hboring units (i.e. townhouses, condominiums, etc.). In buildings with common roofs, the roofing
contractor and/or owner should notify the occupants of adjacent units of roofing work to be performed.
4. Exposed ceilings: Exposed, open beam ceilings are where the underside of the roof decking
ca viewed from below. The owner may wish to maintain the architectural appearance, therefore,
roofing nail penetrations of the underside of the decking may not be acceptable. The SFBC provides the
option of maintaining this appearance.

I 5. Ponding Water: The current roof system and/or deck of the building may not drain well and
m.T cause water to pond (accumulate) in low -lying areas of the roof. Ponding can be an indication of
structural distress and may require the review of a professional structural engineer. Ponding may shorten
the life expectancy and performance of the new roofing system. Ponding conditions may not be evident
until the original roofing system is removed. Ponding conditions should be corrected.
6. Overflow Scuppers (wall outlets): It is required that rainwater flow off so that the roof is not
o loaded from a build up of water. Perimeter /edge walls or other roof extensions may block this
discharge if overflow scuppers (wall outlets) are not provided. It may be necessary to install overflow
scuppers in accordance with Chapter 23 of the SFBC.
7. Ventilation: Most roof structures should have some ability to vent natural airflow through the
in of the structural assembly (the building itself). The existing amount of attic ventilation shall not
be reduced. It may be beneficial to consider additional venting which can result in extending the service
life of the roof.

As per our many conversations in regards to the Miller job site, because of their inability or refusal to pay
for service rendered thus far from Len's Roofing, Inc. according to contract, we are left with not choice
but to request that the existing permit is closed. We are also requesting the reimbursement of all bond fees
issued by Len's Roofing, Inc.
We are aware that you have personally spoke with Mr. Miller and he has also stated to you that he does
not want Len's Roofing, Inc. to complete the roof as contracted.
Your immediate attention to this issue would be greatly appreciated.
An original bond is attached for proper reimbursement.
