SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentatiog submitted has been reviewed by Miami -Dade County Product Control Division and accepted
by the Board of Rules and Appeals (BORA) to be used in Miami Dade County and other areas where allowed by
the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Division (In Miami Dade County) and/or the AHJ (it .areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami -Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the High Velocity Hurricane
Zone of the Florida Building Code.
DESCRIPTION: Series 6800 Aluminum Horizontal Sliding Window 1
APPROVAL DOCUMENT: Drawing No. W96 -25, titled "Series 6800 Alum Horizontal Sliding Window ",
sheets 1 through 4 of 4, dated 8/1/96, with revision C dated 09/04/02, prepared by Al Farooq Corporation, signed
and sealed by Hurnayoun Farooq, P.E., bearing the Miami -Dade County Product Control Renewal stamp with the
• Notice of Acceptance number and expiration date by the Miami -Dade County Product Control Division.
MISSILE IMPACT RATING: None
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved ", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date.or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply •
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dadc County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then if shall
be done in its entirety. .
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request ofthe Building Official.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by Miami -Dade County Product Control Division and accepted
by the Board of Rules and Appeals (BORA) to be used in Miami Dade County and other areas where allowed by
the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Division (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami -Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the High Velocity Hurricane
Zone of the Florida Building Code.
DESCRIPTION: 0.029" (min.) Galvanized Steel Storm Panels Shutter
APPROVAL DOCUMENT: Drawing No. 02 -868 -111, titled " 22 ga Galvanized Steel Storm Panels ", sheets 1
through 4 of 4, prepared by Frank L. Bennardo, P.E., dated November 18, 2002, last revision dated January 02,
2003 bearing the Miami -Dade County Product Control Approval stamp with the Notice of Acceptance number and
approval date by the Miami -Dade County Product Control Division.
MISSILE IMPACT RATING: Large and Small Missile Impact
LABELING: Each panel shall bear a permanent label with the manufacturer's name or logo, city, state and the
following statement: "Miami -Dade County Product Control Approved ", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall
be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by Miami -Dade County Product Control Division and accepted
by the Board of Rules and Appeals (BORA) to be used in Miami Dade County and other areas where allowed by
the Authority Slaving Jurisdiction (AEJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Division (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve theaight to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of.such testing and the AHJ may immediately
revoke, modify,•or suspend the use of such product or material within then jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami -Dade County Product Control Division that this .product or
material fails to meet the requirements of the applicable building code,
This product is approved as described herein, and has been designed to comply with the High Velocity Hurricane
Zone of the Florida Building Code.
DESCRIPTION: Aluminum Tube Mullion - Impact^
APPROVAL DOCUMENT: Drawing No. M95 -06A, dated 11/20/95, with revision A dated 06/24/02, titled
"Aluminum Tube Mullions ", sheets 1 through 4, prepared by AI- Parooq Corporation, signed and sealed by
Humayoun Earooq, P.E., bearing the Miami -Dade County Product Control Approval stamp with the Notice of
Acceptance number and approval date by the Miami -Dadc County Product Control Division.
MISSILE IMPACT RATING: Large and Small Missile Impact Resistant
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved ", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA. •
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. if any portion of the NOA is displayed, then it shall
be done in its entirety.
•INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.

1. SCOPE
This renews the Notice of Acceptance No. 99- 0208.01, which was approved on March 4, 1999. It
approves a 0.035" (min.) galvanized steel storm panels shutter, as described in Section 2 of this
Notice of Acceptance, designed to comply with the South Florida Building Code, 1994 Edition for
Miami -Dade County, for the locations where; the pressure requirements, as determined by SFIRC
Chapter 23, do not exceed the Design Pressure Rating values indicated in the approved drawings.
2. PRODUCT DESCRIPTION
This 0.035" (nun.) galvanized steel storm panels shutter and its components shall be constructed in
strict compliance with the following documents: Drawing No. 98 -3 1 2, titled "20 Ga. Galvanized
Steel Storm Panel ", prepared by Knezevich & Associates, Inc., dated December 16, 1998, sheets 1
through 9 of 9, signed and sealed by V. J. Knezevich, I'.E., bearing the Miami-Dade County Product
Control approval and renewal stamps with the Notice of Acceptance numbers and approval dates by
the Miami -Dade County Product Control Division. These documents shall hereinafter be referred to
as the approved drawings.
3. LIMI"I'ATIONS
All permanent set components, included but not limited to embedded anchor bolts, threaded cones,
metal shields, headers and sills, must be protected against corrosion, contamination and damai;c at
all times.
4. INSTALLATION
This 0.035" (min.) galvanized steel storm panels shutter and its components shall be installed in
strict compliance with the approved drawings.
5. LABELING
Each panel shall hear a permanent label with the manufacturer's name or logo, city, state and the
following statement: "lvliami -Dade County Product Control Approved ".
6. BUILDING PERMIT REQUIREMENTS
6.1 Application for building permit shall be accompanied by copies of the Iollowing:
6.1.1 This Notice of Acceptance.
6.1.2 Duplicate copies of the approved drawings, as identified in Section 2 of this Notice of
Acceptance, clearly marked to show the components selected for the proposed
installation.
6.1.3 Any other documents required by the Building Official or the South Florida Building
Code (Sr BC:) in order to properly evaluate the installation of this system.

NOTICE OF ACCEPTANCE:
I. Renewal of this Acceptance (approval) shall be considered alter a renewal application has been filed and the
original submitted documents, including test - supporting data, engineering documents, are no older than eight
(8) years.
Any and all approved products shall be permanently labeled with the manulacturcr's name, city, state, and the
following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a. There has been a change in the South Florida Building Code affecting the evaluation of this product
and the product is not in compliance with the code changes.
b. The product is no longer the same product (identical) as the one originally approved.
c. If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product.
d. The engineer, who originally prepared, signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and /or manufacture of the product or process shall automatically
be cause for termination of this Acceptance, unless prior written approval has been requested (through the
filing of a revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a. Unsatisfactory performance of this product or process.
b. Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the ,job site •
at all time. The engineer needs not reseal the copies.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.
9. This Notice of Acceptance consists of pages I, 2 and this last page 3.
