DURABLE POWER OF ATTORNEY
I, MARY S. BERRY, appoint NICKOLAS CHARLES BERRY my true and lawful
attorney -in -fact, (hereinafter referred to as 'agent') for me and in my name, to do any and all acts

J. SAFE DEPOSIT BOX: My agent shall have access to any safe deposit box of mine
(whether the box is held in my name alone, in my revocable trust, or jointly with another
or others) wherever located, and may remove the contents and surrender the box on my
behalf. Any institution in which a safe deposit box of mine is located is not liable to me
or my heirs or estate for permitting my agent to exercise this power.
K. POST OFFICE: My agent shall be able to request and authorize the post office to
forward my mail to whatever address my agent may deem advisable.
L. SOCIAL SECURITY: My agent may represent and act for me before the Social Security
Administration of the United States, and any similar agency of a state or local
government; collect all Social Security benefits due me; and make such arrangements in
connection with Social Security benefits including without limitation Medicaid and
Medicare as will facilitate their application to my care and support.
M. EMPLOYMENT OF AGENTS: My agent may employ and dismiss agents, attorneys,

1. SCOPE
This renews a roofing system using Santa Fe "Santafe `S" clay roofing tile, manufactured by
Santafe Tile Corporation described in Section 2 of this Notice of Acceptance, designed to
comply with the South Florida Building Code, 1994 Edition for Miami -Dade County. For
locations where the design pressure requirements, as determined by applicable building code,
does not exceed the design pressure values obtain by calculations in compliance with RAS 127
using the values listed in herein. The attachment calculations shall be done as a moment based
system.

3. LIMITATIONS
3.1 Fire classification is not part of this acceptance.

3.5 30/90 hot mopped underlayment applications may be installed perpendicular to the roof
slope unless stated otherwise by the underlayment material manufacturers published
literature.
3.6 This acceptance is for wood deck applications. Minimum deck requirements shall be in
compliance with applicable building code.
4. INSTALLATION
4.1.1 Santafe 'S' and its components chap be installed in strict compliance with Miami Dade
County Roofing Application Standard RAS 118, RAS 119, and RAS 120.

5. LABELING
5.1 All tiles shall bear the imprint or identifiable marling of the manufacturer's name or logo,
or following statement: "Miami -Dade County Product Control Approved ".
6. BUILDING PERMIT REQUIREMENTS
6.1 Application for building permit shall be accompanied by copies of the following:

order to properly evaluate the installation of this system.

NOTICE OF ACCEPTANCE STANDARD CONDITIONS
1 Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documentation, including test supporting data, engineering documents, are no older than
eight (8) years.
2 Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and
the following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3 Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product
and the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4 Any revision or change in the materials, use, and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been requested
(through the filing of a revision application with appropriate fee) and granted by this office.
5 Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6 The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7 A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the job
site at all times. The copies need not be resealed by the engineer.
8 Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.

3. LIMITATIONS
3.1 Fire classification is not part of this acceptance. Refer to the Prepared Roof Tile
Assembly for fire rating.
3.2 Polypro® AH 160 shall solely be uscd with flat, low, & high tile profiles.
3.3 Minimum underlayments shall be in compliance with the Roofing Application
Standard RAS 120.
3.4 Roof Tile manufactures acquiring acceptance for the use of Polypro® AH 160 roof
tile adhesive with their tile assemblies shall test in accordance with PA 101.
3.5 Roof Tile manufactures acquiring acceptance for two paddy placement with the
use of Polypro® AH 160 roof tile adhesive with their tile assemblies shall test in
accordance with PA 101 and with section 10.4 as modified herein.

4. INSTALLATION
4.1 Polypro® AH160 may be used with any roof tile assembly having a current NOA that
lists uplift resistance values with the use of Polypro® AH 160.
4.2 Polypro® AH 160 shall be applied in compliance with the Component Application
section and the corresponding Placement Details noted herein. The roof tile
assembly's adhesive attachment with the use of Polypro® AH 160 shall provide
sufficient attachment resistance, expressed as an uplift based system, to meet or exceed
the uplift resistance determined in compliance with Miami -Dade County Roofing
Application Standards RAS 127. The adhesive attachment data is noted in the roof
tile assembly NOA
4.3 Polypro® AH 160 roof tile adhesive and its components shall be installed in
accordance with Roofing Application Standard RAS 120, and Polyfoam Products, Inc.
Polypro® AH 160 Operating Instruction and Maintenance Booklet.
4.4 Installation must be by a Factory Trained 'Qualified Applicator' approved and licensed
by Polyfoam Products, Inc. Polyfoam Products Inc. shall supply a list of approve
applicators to the authority having jurisdiction.
4.5 Calibration of the Foampro® dispensing equipment is required before application of
any adhesive. The mix ratio between the "A" component and the "B" component shall
be maintained between 1.0 -1.15 (A) : 1.0 (B). The dispense timer shall be set to
deliver 0.0175 to 0.15 pounds per tile as determined at calibration. No other settings
shall be approved.
4.6 Polypro® AH 160 shall be applied with Foampro RTF 1000 or ProPack® 30 & 100
dispensing equipment only.
4.7 Polypro® AI.1160 shall not be exposed permanently to sunk

Polyfoam Products, Inc., produces two- component expanding polyurethane adhesive for the attachment
of clay and concrete roof tile. Polypro® AH160 is specially designed for application of concrete and
clay roof tile over a variety of roof deck substrates and underlayment materials.
Polypro® adhesive is applied with proprietary Foampro® RTFI000 dispensing equipment. Approval of
this product is specific to the two component mix and Foampro® RTF1000 dispensing equipment.
Alternative methods of dispensing will require a revision to this approval.
Polypro® adhesive shall be applied only by 'Qualified Applicators', licensed by Polyfoam Products, Inc.
Certificates and identification cards are issued to applicators upon successful completion of a application
training program. Approval of this adhesive system is contingent upon application by a 'Qualified

1. Extensions of Acceptance may be considered after a new application has been filed and the supporting data,
test reports no older than ten (10) years, have been re- evaluated.
All reports of re- testing shall bear the seal, signature and date of an engineer registered in the State of
Florida.
2. Any revision or change in the materials, use, or manufacture of the product or process shall automatically be
cause for termination, unless prior approval is granted for revisions or change.
3. Any unsatisfactory performance of this product or process or a change in Code provisions shall be grounds
for re- evaluation.
4. This acceptance shall not be used as an endorsement of any product for sales or advertising purposes.
5. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
6. Product approval drawings, where required for permit applications, shall be provided to the applicant by the

• Alfaria El Volcan's clay tile has been tested in compliance with the South Florida Building Code
requirements for clay, nail -on, mortar set and adhesive set tile applications. The minimum roof slope for
Alfaria El Volcan nail -on, clay tiles shall be 4 ":12 ". The minimum roof slope for Alfaria El Volcan
mortar or adhesive set clay tiles shall be 2 ":12 ". See the "Profile Drawing" section in this approval for
the "Volcan 'S' Statue Clay Barrel" profile drawing. The "Volcan 'S' Statue Clay Barrel" tile profile has
been tested for both wind characteristics and static uplift performance, therefore, any consideration for
installation shall be done as a 'Moment Based System'. Data for attachment calculations is noted in
Tables 1 through 4 of this approval.

127. The mortar and adhesive attachment data is noted in Table 4, attached.

12" run (4 ":12 "). The standard minimum roof pitch for Alfaria EI Voclan clay, mortar or adhrcwv'
set tile applications is 2" rise in 12" run l2 ":12 "). the maximum roof pitch for mortar or adhesive
set tile applications is 7" rise in 12" run (7 ":12 ").
2. The Alfaria El Volcan "Volcan 'S' Statue Clay Barrel" tile profile has not been tested for battened
applications, therefore, System A (Counter -Batten Application) and System C (Horizontal Batten
Application) do not form part of this approval.
3. For nail -on applications, brass or stainless steel screws for mechanical attachment of tiles shall
have a head diameter larger than that of the preformed holes in the tile.
4. System installation shall be in compliance with the system specifications outlined in this Product
Control Approval. Nail -on tile shall be attached using one of the methods of attachment noted in
Table 3 of this Approval. Mortar set or adhesive set tile shall be attached using an approved
mortar or adhesive noted in this Approval, the data of which is noted in Table 4 of this Approval.
The method of attachment utilized shall provide sufficient attachment resistance expressed as a
moment to meet or exceed the required moment of resistance determined in compliance with Dade
County Protocol PA 1 15 or PA 127.
5. For nail -on applications, an cave clip is required on all cave tilc. For mortar set applications, the
first three courses of tile shall be nailed with not less than one nail per tile. As an alternate, the first
three courses of tile shall be applied in mortar over a single layer of minimum 12 ga. wire mesh
with square openings of not less than 3 / 8 " which is mechanically attached to the deck with not less
than one roofing nail every 1 ft'. For roof pitches from 6 ":12" to 7 ":12 ", every third tile of every
fifth course, beginning at the eighth course, shall be nailed with not less than one nail per tile.
6. For mortar set or adhesive set tile applications, a field static uplift test by a Dade County
accredited testing agency, in compliance with Dade County Protocol PA 106, shall be required not
less than 30 days after application to confirm tile adhesion. The results of this testing shall be
reported to the Building Official and the installer stating that the application has passed or failed PA
106 testing. If the application fails PA 106 testing, the report shall state which portion of the test
was failed; Category 1 (examination for loose tile) or Category 2 (uplift testing of tile). Subsequent
to testing, the installer may repair not more than 5% of field area tiles and 10% of perimeter area
(:.e. ridge /rake) tile with approved tile adhesive. I he installer shall place an identifiable marking on
each repaired tile for future reference. Any PA 106 re-test shall not include any marked tile.
7. For mortar or adhesive set tile applications, 30/90 hot mopped underlayment applications may be
installed perpendicular to the roof pitch prior to June I, 1995 unless stated otherwise by the material
manufacturer. Thereafter, 30/90 underlayment systems shall be installed parallel to the roof pitch in
compliance with Appendix 'A' of Dade County Protocol PA 120.
8. All tiles shall bear the imprint or identifiable marking of the manufacturer's name or logo for
identification in the field.

10. Applications for roofing permits shall include a completed Section 11 of the Uniform Building
Permit, a copy of Alfaria El Volcan's current specifications and details and a copy of this Product
Control Approval. Reference shall be made to appropriate data for the required fire rating.
11. The applicant shall retain the services of a Dade County certified testing laboratory to maintain
quality control in compliance with the South Florida Building Code and related protocols.
12. Any amendments to these provisions shall be in compliance with Sections 203 and 204 of the South
Florida Building Bode.

All reports of rc- testing shall bear the seal, signature and date of an engineer registered in the State of
Florida.
