1. DO NOT BEGIN ANY WORK WITHOUT HAVING RECEIVED YOUR VALIDATED PERMIT AND PERMIT CARD. Applying for a permit does not grant
the right to begin construction. HOURS OF CONSTRUCTION are limited to: Monday through Friday from 7:30 a.m. to 6:00 p.m., and Saturday from 8:00 a.m.
to 5:00 p.m. No inspections will be conducted on weekends or holidays.
2. All construction of demolition areas MUST BE MAINTAINED IN A CLEAN, NEAT AND SANITARY CONDITION free from construction debris.
3. STREETS AND NEIGHBORING PROPERTIES SHALL BE KEPT FREE FROM DIRT AND DEBRIS.
4. SWALES MUST BE PROTECTED FROM BEING DAMAGED BY EQUIPMENT OR VEHICLES, AND MAY NOT BE USED FOR STORAGE. A bond is
required for work in or near the street/sidewalk.
5. CONSTRUCTION TRAILERS ARE PROHIBITED ON SINGLE FAMILY RESIDENTIAL CONSTRUCTION SITES. Other construction may have a trailer
which requires a separate permit.
6. PORTABLE TOILETS for a construction site require a separate permit.
7. DO NOT DISCHARGE WATER INTO THE RIGHT OF WAY OR STORM DRAINS without approval from the Building, Planning and Zoning Department.
8. EQUIPMENT AND MATERIALS SHALL BE STORED at least 10 feet from the edge of pavement.
9. Department of Health and Rehabilitative Services (HRS) approval is required for applications involving septic tanks. Department of Environmental Resources
Management (DERM) and/or Miami -Dade Water and Sewer Department (MDWASD) approval is required for applications involving sewers.

conform to the current code requirements of the Building Code.

Please be advised that I have researched the file on the premises located at 482
N.E. 93rd Street, Miami Shores, and found nothing in regards to roof repairs at
this location. The only reference to the roof at all was a complete re -roof which
was done in 1977 (copy enclosed). The most recent permit was issued in 1989 for
some electrical work.
I am enclosing a copy of a letter from Commercial Risk Services, Inc., dated
March 14,1995, which stipulates that I am to meet with this agency on June 15,
1995, at 1 p.m. Due to the fact that insurance rates for Miami Shores Village (and
all other government entities) will be determined by this agency, it is mandatory
that I keep this appointment.
In light of the above information, will it be necessary for me to appear at this
hearing? Please advised me soon as possible

Dear Mr. Couzzo
The Building Code Effectiveness Grading Schedule (BCEGS) was developed through the
combined efforts of the three national building code organizations and the property/casualty
insurance industry. The program evaluates a community's commitment to building code
enforcement - focusing on natural hazard mitigation. The intent is to encourage code compliant
construction through favorable insurance premium adjustments.
BCEGS was recently pilot tested in over 150 communities in North Carolina, Florida, and South
Carolina. Based on information and feedback gathered from the test communities the BCEGS
program has been revised, refined, and debugged. We are now proceeding with the program to
generate actual community classifications.
ISO Commercial Risk Services.. Inc. (ISO /CRS), a subsidiary of the Insurance Services Office,
will be administering this program.
ISO /CRS has previously visited your City or Fire District to evaluate the fire defenses. The
classification number from that evaluation is one of many factors used to determine insurance
premiums for properties in your community. That program, which has been used effectively by
the insurance industry since 1916, served as a model for the Building Code Effectiveness
Grading Schedule.
During the next two years ISO /CRS intends to visit and classify virtually every jurisdiction in
the state of Florida. We have taken the liberty of sending a copy of this letter and the current
questionnaire to your building official. We would appreciate it if they could complete the
questionnaire and gather the appropriate files for verification of their responses. We would like
to meet with Mr. Francis Libien on 6/15/95. at 1:00 pm,
If the timing of this survey conflicts with their schedule, we ask that they notify us as soon as
possible so that we can reschedule a date which is mutually convenient. During that meeting we
will review the questionnaire and responses as well as supporting documentation. We would
also like to schedule a site visit with a field inspector to see a conveniently accessible building
under construction
