Dear Mr. Bradford,
I have inspected the remodeling and repair work at the above mentioned address
which was undertaken without a building permit. It is my opinion that the
re- framing of the damaged trusses and re- sheathing of the damaged portion of
the roof was properly installed, satisfactorily secured, and finished. The fire had
not caused any structural damage to the concrete block walls, therefore, the
area in question has been properly restored to its previously habitable condition.
The electrical damage, also repaired without a permit, is to be checked and
completed by a Dade County licensed Contractor. This work will be finally
inspected by me to ascertain proper installation.
I am sure that this effort, with your co- operation,will result in the proper
completion of the work erroneously attempted without the required building permit.
Hopefully, this will allow the owner of the property to be issued a permit and
certification after our final inspection.

1. Extension of Acceptnce may be considered after a new application has been filed and the supporting data,
test reports no older thq4 ten (10) years, have been re-evaluated.
All reports of re- testing shall bear the seal, signature and date of an engineer registered in the State of
Florida.
2. Any revision or change in the materials, use, or manufacture of the product or process shall automatically
be cause for termination; unless prior approval is granted for revisions or change.
3. Any unsatisfactory alerformance of this product or process or a change in Code provisions shall be grounds
for re- evaluation.
4. This acceptance shall not be used as an endorsement of any product for sales or advertising puiposes.
5, The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall lie done in its entirety.
6. Product approval drawings, where required for permit applications, shall be provided to the applicant by
the manufacturer or his listributors; unless otherwise noted in the Notice of Acceptance. The prints need not
be re- sealed by an engineer.

application shall be applied as set forth in the adhesive Product Control Approval. Ail tile
and accessories shall be installed in compliance with systern #3 of the Dade County Model
Tile Specification. All tile shall be presoaked before application.
Roofing Tile: The tilt system shall be installed in compliance with the Metro Dade County Model
Mortar /Adhesive -Set Tile Specification (Appendix 'X' of the South Florida Building
Code), utilizing the components listed in the Products Listings' set forth in this Product
Control Approval. Applications, as detailed in the test reports, are consistent with the
application Methods stated in the execution section of said model specification.

Velocity Pressure:
Comments: For re -roof applications, 15/32" plywood is an acceptable substrate. On slopes 4 ":12" to
6 ":12" tile shall have the first three courses of tilt nailed with not less than one nail per
tile. As an alternate, the first three courses shall be applied in mortar over a single layer of
minimum 12 ga wire mesh with square openings of not less than 3/8" which is
mechanically attached to the deck with not less than one roofing nail per every 2ft For
pitches from 6":12" to 7 ":12" every third tile of every fifth course, beginning with the
eighth course, shall be nailed. On Slopes >7 ":12" all provisions of 'System 1' of the Dade
County Model 'Horizontal Batten' Nail -on Tile Specification shall apply.

Limitations:
1. This roof system assembly may be applied to roof slopes of 3 ":12" to 6 ":12 ".
2. Horizontal battens slilalt be used on all tile applications on slopes of 7Y2":12" and greater. (All systems)
3. Nails for attachmentlof tile shall have heads larger than the pre - formed nail holes in the tile.
4. All nails shall be co#osion resistant. Exposed nails shall be hot dipped galvanized, stainless steel, or copper
only.
5. Design velocity presOures shall be calculated in compliance with the provision of Chapter 23 of South Florida
Building Code, however' the allowable height above ground shall not exceed a maximum height of 45' for
non -coastal, 30' in Coas al Area 1, and only by job approval from the Chief Product Control Compliance Officer in
Coastal Area 2.
6. Any amendments tolthese provisions shall be in compliance with Sections 203 and 204 of the South Florida
Building Code.
7. All tiles shall bear tie imprint of the manufacturer's name or logo, as noted attached, to permit identification
in the field.
8. The manufacturer s'pall retain the services of an independent testing laboratory to maintain quality control.
Testing shall be performed on a minimum of five (5) tiles every three (3) months, for strength according to Section
3404 of Ulu South Floriea Building Code; and for moisture absorption according to ASTM C -140. Test samples
shall be selected by a laboratory according to ASTM D- 3665 -82. Results shall be sent to Dade County Product
Control Section. Test ri;ports shall bear a description of the tile by material and color, i.e. cement, grey.
9. All domestic distri hors of foreign manufactured the shall have a completed distribution agreement filed with
the Metro Dade CountylOffice of Code Compliance confirming the distributor's agreement to adhere to the
conditions of this report.
10. Multiproductos Sao a Cruz Alhambrar" Tile shall be installed in strict compliance with the application
instructions noted herein as well as the manufacturer's specifications and application instructions. The Metro
Dade County Model Tide Specification shall take precedence.
11. The use of all pre -fi red trim and specialty tile tested is required as it forms an intricate part of these product
control approval tests.
42. Applications for roji fing permits must be accompanied by Section 11 of the Uniform Building Permit, clearly
indicating the extent ol'the work to be performed, along with current manufacturer's specifications and details. In
addition, a copy of this approval shall be attached to the permit application. Reference shall be made to all
appropriate data for th0 required fire rating.
