NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1 Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documents, including test - supporting data, engineering documents, are no older than eight
(8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Miami -Dade County Product Control Approved ", or as specifically stated in the
specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a. There has been a change in the South Florida Building Code affecting the evaluation of this product
and the product is not in compliance with the code changes.
b. The product is no longer the same product (identical) as the one originally approved.
c. If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product.
d. The engineer, who originally prepared, signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and /or manufacture of the product or process shall automatically
be cause for termination of this Acceptance, unless prior written approval has been requested (through the
filing of a revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a. Unsatisfactory performance of this product or process.
b. Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection at the job site
at all time. The engineer needs not reseal the copies.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.

1. DESCRIPTION
1.1 This approves a .060" aluminum, 3003-H14 alloy , storm panel designed to comply with the South
Florida Building Code, 1994 Edition for Dade County, for the locations where the pressure
requirements as determined by ASCE 7 -88 "Minimum Design Loads for Buildings and Other
Structures ", do not exceed the Design Pressure Rating values in Section 7 and within the limitations
contained in Section 3.
2. MATERIAL CHARACTERISTICS
2.1. The storm panels shall be constructed as indicated on approved drawing No. 95 -549, Sheet 1 of 4,
using .060" thick aluminum, alloy 3003-H14.
2.2 Panels are rolled formed, have a nominal width of 12 -1/2" and a total full width of 14 -1/2 ", forming
2" deep ribs
2.3 Components shall be aluminum alloys 6063 -T6, "H" headers, "U" header, 1-1/2"x 1-1/2"x 1/8" sill
angle and 2" x 5 "x 1/8 ", 2" x 5 "x 1/4" built -out angles.
2.4 Tested specimen:
Dimensions: 4 panels 52" wide x 101" long overall. 4 specimens for windload
4 panels 52" wide x 67" long overall. 1 specimen for windload
4 panels 52" wide x 48" long overall. 1 specimen for windload
4 panels 52" wide x 101" long overall. 4 specimens for impact and cyclic load
4 panels 52" wide x 104" long overall. 2 specimens for impact and cyclic load
3. LIMITATIONS
�.1 The design will be dependent on the maximum panel height of 7' -2" or 86 ", for wall mount with
bottom sill continuous wood or aluminum tube ledger as shown on drawing No. 95 -549, Sheets 2
of 4.
3.2 Minimum separation to existing glass to protect shall be 3" with stitch bolts reinforcement as
shown on Sheet 1 of 4.

3..3 Maximum clearance between top of panel and inside of header shall he 1/4"
3. t Special Requirement:
Reinforcing bar, consisting of one 1" x 1" x 16 Ga. Galy. Steel tube (Part 14) attached with I/4 " -20
x 3 -1/2" long SS bolts with wingnut assemblies spaced 12 -1/2 "" o.c at panel midspan overlaps; or
one (1) row of 1/4 " -20 x 1" long SS stitch bolts reinforcement with wingnuts assemblies spaced
12 -1/2" o.c. at panel midspan overlaps. Panel overlap spans less than 33" do not require midspan
reinforcement bars and /or stitch bolts.

4. INSTALLATION
4.1 The panels shall be installed only in the following condition:
4.1.1 Wall Mounts:
(A) (CONCRETE /MASONRY): The panels are secured to an '1 -1' Wall Header (Parts 2 or
3) at the head and attached with 1/4"-20 S.S. studs spaced 6-1/4" o.c. to a 1-1/2" x 1-1/2" x 1/8"
aluminum angle at the sill (Part 7), both the header and sill angle are anchored to the wall, using the
tension anchors as shown on Section A "Wall Mount Section "; or the panels are directly secured to
the wall using approved tension anchors as shown on Sections F "Wall Mount Section (Direct
Mount) ", as shown on Drawing No. 95 -549, Sheet 2 of 4 ; or panels attached at the head and sill to
a continuous 'F' track (Part 9) with 1/4 " -20 x 1" S.S. studs spaced 6 -1/4" o.c. , the track is then
secured to the wall using approved tension anchors as shown on Sections I "Wall Mount Detail ", as
shown on Drawing No. 95 -549, Sheet 3 of 4 bearing the Dade County Product Control approval
stamp.
(B) (WOOD FRAMED): The panels are secured to an '1 -1' Wall Header (Parts 2 or 3) at the
head and attached with 1/4"-20 S.S. studs spaced 6-1/4" o.c. to a 1-1/2" x 1-1/2" x 1/8" aluminum
angle at the sill (Part 7), both the header and sill angle are anchored to the wall, using the tension
anchors as shown on Section D "Wall Mount Section "; or at the sill , panels are attached to a
continuous 2 x 6 PT wood ledger or 1" x 4" x 1/8" or 2" x 4" x 1/8" aluminum tube ledger plate with
1/4" diameter S.S. lag screws spaced 11" o.c. and #14 SMS at midway between lag screws, the
ledger plate is then anchored to the wall with (2) - 1/4" diameter S.S. lag screws spaced 24" o.c.
maximum, as shown on Sections E "Wall Mount Section (Bottom) ", as shown on Drawing No.
95 -549, Sheet 2 of 4, or panels attached at the head and sill to a continuous 'F' track (Part 9) with
1/4 " -20 x 1" S.S. studs spaced 6 -1/4" o.c. , the track is then secured to the wall using approved
tension anchors as shown on Sections I "Wall Mount Detail ", as shown on Drawing No. 95 -549,

4.1.2 Ceiling/Inside Mounts:
(A) (CONCRETE /MASONRY): The panels are secured to an 'U' 1-leader (Part 4) at the
head and attached with 1/4"-20 S.S.studs spaced 6-1/4" o.c. to a 1-1/2" x 1-1/2" x 1/8" aluminum
angle at the sill (Part 7), both the header and sill angle are anchored to the wall, using approved
tension anchors as shown on Section B "Ceiling /Inside Mount Section ", Drawing No. 95 -549, Sheet
2 of 4, bearing the Dade County Product Control approval stamp.
(B) (WOOD FRAMED): The panels are secured to an 'U' 1- leader (Part 4) at the head and
attached with (3) #14 wood S.M.S. wood screws with minimum of 2" penetration or (3) wood
brushings with 1" penetration at each rafter or truss thru a 1 "x 5 "x 1/8" continuous channel (Part
11) or 1 "x 4 "x 1/8" continuous aluminum tube; at the sill the panels are attached with 1/4" S.S.
studs spaced 6 -1/4" o.c. to a 1 -1 /2 "x 1 -1 /2 "x 1/8" aluminum sill angle (Part 7), the sill angle in then
anchored with approved tension anchors, as shown on Section G "So it Mount Detail ", Sheet 3 of 4

4.1.3 Built -Out Condition:

Header (Part 4) at the head, which is attached to built -out angles (Parts 5, 5A, 5B or 6), with
1/4"-20 S.S bolts spaced 12" o.c., and a 1-1/2"x 1 -1 /2 "x 1/8" aluminum studded angle (Part 7) at
the sill, which is attached to built -out angles (Parts 5, 5A, 5B or 6), w ith 1/4 " -20 S.S. bolts spaced
12" o.c., both of the built -out angles are then anchored to the wall using approved tension anchors as
shown on Section C "Built -Out Mount Section ", Sheet 2 of 4; or Section H "Alt. Build -Out Mount
Section ", Sheet 3 of 4; Drawing No. 95 -549, bearing the Dade County Product Control approval
stamp; or the panels are secured to an 'H' Header (Parts 2 or 3) at the head, which is attached to
built -out channel (Part 10), with 1/4"-20 S.S bolts spaced 12" o.c., and a l -1/2"x 1-1/2"x 1/8"
aluminum studded angle (Part 7) at the sill, which is attached to built -out channel (Part 10), with
1/4 " -20 S.S. bolts spaced 12" o.c., both of the built -out channels are then anchored to the wall using
approved tension anchors as shown on Section J "Built -Out Mount Section ", Sheet 4 of 4 ,
Drawing No. 95 -549, bearing the Dade County Product Control approval stamp.
4.1.4 Storm Panel Interior Fastening:
Panel are secured at sill using component angles (Part 5A and 7) as shown on Detail J "Storm Panel
Interior Fastening (Isometric) ", Sheet 3 of 4, Drawing No. 95 -549, bearing the Dade County Product
Control approval stamp.
4.1.5 Any combination of the above,
4.2 Anchorage of the shutters shall be to concrete, masonry and /or wood framing and shall consist of
only the anchors specified on drawing No. 95 -549, Sheets 1 thru 4 of 4. Substitution of anchor type
or indicated embedment is unacceptable.
4.3 Fasteners must have their own Notice of Acceptance and must be made of stainless steel or have
adequate protection against corrosion, per DIN 50018. Aluminum contacting metals not considered
compatible shall be properly protected.
4.4 Permanent set fastener components, embedded anchor bolts, threaded cones or metal shields, not in
use, must be protected against corrosion, contamination and damage at all tinge.
5. IDENTIFICATION
5.1 Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Dade County Product Control Approved ".
5.2 Also a Warning Note shall be permanently stamped in each panel advising the home owner or
tenant that the storm panels will not offer hurricane protection unless all reinforcing bolts are
properly installed.

6. USE
6.1 Application for building permit shall be accompanied by two copies of the following:
6.1.1 This Notice of Acceptance.
6.1.2 Completely dimensioned drawing showing size and location, including height above
grade, of openings to receive shutters; mean roof height, length and width of building.
6.1.3 Duplicate prints of approved drawings No. 95 -549, Sheets I thru 4 of 4, bearing the
approval stamp, Notice of Acceptance number and date by Dade County Product Control
Section, clearly marked to show the components selected for the proposed installation.

NOTICE OF' ACCEPTANCE: STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documents, including test supporting data, engineering documents, are no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manuf'acturer's name, city, state, and the following
statement: "Dade County Product Control Approval ", or as specifically stated in the specific conditions of this
Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and the product
is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the correct
installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted, is no longer
practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be cause
for termination of this Acceptance, unless prior written approval has been requested (through the filing of a revision
application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other purposes.
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the expiration date
may be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed, then it shall be done
in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be provided to
the user by the manufacturer or its distributors and shall be available for inspection at the job site at all time. The
copies need not be resealed by the engineer.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.
9. This Acceptance contains pages 1, 2, 2(a) through 2(d) and this last page 3.

10. Unless specifically indicated in the Acceptance (approval), this unit is approved as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is required from the Product Control Section.
I I . The spacing of fasteners at window sills shall be as indicated in Section 4 of this Notice of Acceptance. The spacing of
fasteners in all other parts of the frame, shall be as indicated in Section 4 of this Notice of Acceptance, but in no case
shall exceed 24" on center. The first fastener shall be located at a maximum of 6" from each corner and mullion or
stile. Fastener shall fully penetrate the buck, which shall be the same size as the one tested with the unit. No wood or
plastic shields or pins shall be used for anchoring.
12. Hardware for all windows and doors shall conform to Security and Forced Entry Proven on ► apter 36 of the South
Florida Building Code.

reserves the right to secure this product or material at anytime from a jobsite or manufacturer's plant for
quality control testing. If this product or material fails to perform in the approved manner, the Code
Compliance Office may revoke, modify, or suspend the use of such product or material immediately. The
applicant shall re- evaluate this product or material should any ammendments to the South Florida Building
Code be enacted affecting this product or material. The Building Code Compliance Office reserves the

1. DESCRIPTION
This approves an aluminum bahama shutter designed to comply with the South Florida Building
Code, 1994 Edition for Miami -Dade County, for the locations where the pressure requirements, as
determined by SFBC chapter 23, do not exceed the Design Pressure Rating values in Section 7 and
within the limitations contained in Section 3.
2. MATERIAL CHARACTERISTICS
The bahama shutter and its components shall be constructed in strict compliance with approved
drawing No. 94 -187, with revision #4, titled Bahama Shutter; Hurst Awning Co., Inc., prepared by
Knezevich and Associates Sheets 1 and 2 of 2, bearing the Miami -Dade County Product Control
Approval stamp with the Notice of Acceptance number and approval date by the Miami -Dade
County Product Control Division. These plans shall hereinafter be referred to as the approved plans.
3. LIMITATIONS
All permanent set components, included but not limited to embedded anchor bolts, threaded cones,
metal shields, headers and sills, must be protected against corrosion, contamination and damage at
all time.
4. INSTALLATION
This hurricane protection system must be installed in strict compliance with the approved plans.
5. IDENTIFICATION
Each panel shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved ".
6. USE
6.1 Application for building permit shall be accompanied by copies of the following:
6.1.1 This Notice of Acceptance.
