TERMS AND CONDITIONS OF THIS CONTRACT
1. R.N. Palmer's Roofing Co. Inc. shall exercise reasonable care in its execution of work however cannot be
liable in any case for damage to shrubbery, screening, sidewalks, foliage, cables or pipes, at or below
ground level. Care will be taken when inclement weather approaches during the progress of work, however
we are not to be held liable for damage to interior of buildings, including furnishings, personal belongings or
plaster. We cannot be responsible for damages due to delays from strikes, accidents, fire or other causes
beyond our control, neither for pre- existing defects at premises on which work is be done. Reasonable care
shall be taken but R.N. Palmer's Roofing Co. Inc. must be allowed access to building and will not be
responsible for damages to pools, patios, screen enclosures, lawns, fiberglass or plastic roofs, skylights and
sprinkler systems. Be advised that in areas where you might have open beam ceilings some debris may
come through - sheathing boards. Take necessary precautions and cover all areas which may need covering
such as carpeting, fish tanks, furniture, etc. that you do not want exposed during roof removal process, that
may become soiled otherwise. Loose objects should be taken down as vibrations from work may cause
damage. Any structural changes involving woodwork on other unless otherwise stated would be the
responsibility of property owner, unless otherwise stated herein. R.N. Palmer's Roofing Co. Inc. shall not be
held liable for acts of God, cracks or marks due to vibrations of work in progress, or other casualty or theft
or other causes beyond its control.
2. R.N Palmer's Roofing Co. shall not be liable for damage to building worked on or to work done thereon by
R.N. Palmer's Roofing Co. Inc. caused by settlement, distortion, warping or failure of roof deck, ceiling,
foundation sheathing, walls or partition walls of said building, nor for the failure of any materials or integral
part of said building used as a base, attachment to or for said work; nor for water damage or any other
damage to the exterior or interior of said building, or to decorations, stock, furniture, fixtures or equipment
within or such other contents thereof, due to leakage or other cause.
3. If contract is cancelled by purchaser, purchaser agrees to pay as liquidated damages a minimal amount
equal to 25% of this proposal in addition to additional cost incurred by R.N. Palmer's Roofing Co. Inc.
provided that such damages shall not exceed the entire proposal amount.
4. Owner to provide necessary electricity and water.
5. Property owner gives R.N. Palmer's Roofing Co. Inc. right of access, by vehicles and all•other means, for
removal of roofing and delivery of materials to edge of building, on or off driveways and roads during hours
of work which will include written permission from neighboring property owners that shall be used to access
workmen or machinery.
6. R.N. Palmer's Roofing Co. Inc. shall not be held responsible for damage to septic tanks, pipes or any other
underground structure unless property owner has clearly marked location of same in advance.
7. Length of warranty as stated is on a prorated basis for period indicated.

Metro Roof Tile, Inc., located in Medley, Florida, manufacturers clay roof tile for nail -on, mortar
set or adhesive set applications. This Product Control Approval relates to Metro Roof Tile's "Concrete
Flnt" flat til_ e profs, Refer to appropriatQ . oduct Control gpproyals for other tile profiles.
The Metro Roof Tile "Concrete Flat" tile profile is available in a variety of colors and has
matching trim pieces used for rake hip, ridge hip, and valley terminations. These accessories are
manufactured for all profiles and form a part of this Product Control Approval.
The Metro Roof Tile "Concrete Flat" roof tile has been tested in compliance with the South
Florida Building Code requirements for concrete, nail -on, mortar set or adhesive set tile applications.
See the "Profile Drawing" section in this Approval for the Metro Roof Tile "Concrete Flat" profile
drawing. The Metro Roof Tile "Concrete Flat" profile has been tested for both wind characteristics and
static uplift performance, therefore, any consideration for installation shall be done as a 'Moment Based
System'. Data for attachment calculations is noted in Tables I through 4 of this Approval.

SYSTEM LIMITATIONS

The standard minimum roof pitch for Metro Roof 'file's "Concrete Flat" llit profile tile shall
comply with Dade County Application Standards PA 118, PA 119 or PA 120, depending on the
method of installation.
For nail -on applications, fasteners for mechanical attachment of tiles shall have a head diameter
larger than that of the preformed holes in the tile.
System installation shall be in compliance with the system specifications outlined in his Product
Control Approval. The method of attachment utilized shall provide sufficient aitachmenuresistance
expressed as a moment to meet or exceed the required moment of resistance determined in
compliance with Dade County Protocol PA 115 or PA 127. The Metro Roof Tile "Concrete Flat"
tile profile has been tested for both wind characteristics and static uplift performance, therefore,
attachment calculations for installation in compliance with PA 115 or PA 127 shall be done as a
'Moment Based System'
For mortar or adhesive set tile applications, a field static uplift test by a Dade County accredited
testing agency, in compliance with Dade County Protocol PA 106, shall be performed.
For mortar set tile applications, 30/90 hot mopped underlayment applications may be iustallcd
perpendicular to the roof pitch unless stated otherwise by the underlayment material manufacturer's
published literature.
6 All tiles shall bear the imprint or identifiable narking of the manufacturer's name or logo for
identification in the field.
Applications for rooting permits shall include a completed Section 11 of the Uniform Building
Permit, a copy of Metro Roof Tile's current specifications and details, a copy of this Product
Control Approval and a copy of the Product Control Approval of any Roofing Component used in
the proposed tile application. Reference shall be made to appropriate data for the require] fire
rating.
The applicant shall retain the services of a Dade County certified testing laboratory to maintain
quality control in compliance with the South Florida Building Code and related protocols. Samples
taken shall be in compliance with Dade County Protocol PA 112, Appendix 'A'.
Any amendments to these provisions shall be in compliance with Sections 203 and 204 of the Soup
Florida Building Code.

a) THE MORTAR HAS TO BE FRESH • AND SHOULD BE USED
WITHIN ONE HOUR AFTER BEING MADE. IN DADE COUNTY
A PREMIX BAG HAS TO BE USED FOR MORTAR. THE
MORTAR IS APPLIED VERTICALLY WHERE THE TWO
CAVITIES OF THE TILE ARE LOCATED. A FULL TROWEL
No. 10 WILL BE SUFFICIENT FOR THE JOB. IT WILL
TAKE BETWEEN 5 AND 6 POUNDS OF MORTAR ENOUGH
AMOUNT OF MORTAR SHOULD BE APPLIED TO FILL THE
CAVITIES. AS THE TILE IS BEING PLACED ON THE
ROOF, IT SHOULD SQUEEZE SOME MORTAR OUT.
b) THE TILE SHOULD NOT BE WALKED ON AFTER IT HAS
BEEN SET IN PLACE, UP TO AT LEAST 5 DAYS AFTER
ITS INITIAL PLACEMENT.
c) AFTER THE 5 DAYS THE ROOF CAN BE CLEANED AND
CHECKED FOR ANY LOOSE TILES THAT WERE NOT IN-

A red rosin sheet shall be installed on all wood plank decks to eliminate asphalt seepag and bonding of
base sheet to wood plank deck. Red rosin sheet may also be installed on plywood decks as an option.
2 Pre - assembled 'Cap Nail' shall not be used for base sheet attachment.
3 When All Weather Empire base sheet is installed as the base sheet, a ply sheet of ASTM D 460I type I I
(type G2) must be applied in approved mopping asphalt applied within the EVT range alnd at a rate of
20 -40 lbs. /sq prior to installation of membrane.

GENERAL LIMITATIONS
All asphalt shall comply with ASTM D 312, type Ill or IV requirements and approved by applicant.
Fire ratings are determined by a combination of slope, deck type and assembly. Refer to the current
Underwriters Laboratories Roofing Materials Directory or other fire testing data listed in the testing file.
Fire ratings shall be in strict compliance with Sections 3401.5 and 3401.6 of the South Florida Building
Code.
Maximum slope range varies for each system. Consult the current Underwriters Laboratories Roofing
Materials Directory and manufacturer's specifications for compliance with design criteria for each
project.
An overlay and/or recovery board is required on all applications over closed cell foam insulations when
the base sheet is fully mopped in hot asphalt.
All work shall be performed by a contractor licensed to do roofing work in Metro- Dade'County.
Contractor shall be familiar with the details and specifications published by the manufacturer and the
requirements of the South Florida Building Code.
The submission of system specifications and details shall accompany the Section II Building Code
Permit. This information is available in the publications listed in the 'System Description' of this
Product Control Approval. The submission of these documents, as well as the proper application and
installation of all materials shall be the sole responsibility of the contractor.
Perimeter and corner areas shall comply with the elevated design pressures of these roofiareas, as
calculated in compliance with Chapter 23 of the South Florida Building Code. Anchor or base sheet
fastening patterns and/or insulation attachment densities shall be increased in compliance with the
provisions set forth in Metro -Dade County Roofing Application Standard PA 117 to meet these elevated
design pressures.
All attachment and sizing of perimeter nailers, metal profile and/or flashing termination designs shall
conform to Metro -Dade County Roofing Application Standard PA Ill and the wind load requirements
of Chapter 23 of the South Florida Building Code.
Flashings shall be installed according to the manufacturer's standard details and may be applied in cold
application adhesive, approved asphalt or may be applied in conjunction with an approved torch applied
modified bitumen membrane. Specific details, approved by the manufacturer, shall be submitted with
the Section II Permit Application. All details shall comply with the provisions of the South Florida
Building Code.
Fastener spacing for anchor or base sheet attachment is based on a Minimum Characteristic Resistance
Force (F') as determined through field testing in compliance with Metro -Dade County Protocol PA 105
or PA 105(A). If F', as tested, is less than that listed in the System Limitations, a professional engineer
may submit revised fastener spacings utilizing F', taken from a Metro -Dade County Protocol PA 105 or
PA I05(A) test report, and the provisions set forth in Metro -Dade County Roofing Application Standard
PA 117.
1 ! Fastener density for insulation attachment is based on a Minimum Characteristic Resistance Force (F')
as determined through field testing in compliance with Metro -Dade County Protocol PA I05. Iff', as
tested, is less than 275 lbf., insulation attachment shall not be acceptable.
Asphalt moppings shall be applied with approved asphalt and shall be in compliance with the
equiviscous temperature (EVT) methods of asphalt determination. Asphalt containers or bulk tickets
shall indicate an EVT, finished blowing temperature (FBT) and flash point. Asphalt types'and
temperature ranges shall be in compliance with Subsection 3403.4(A)(3) of the South Florida Building
Code.
Insulation may be applied in multiple layers. The first layer shall be attached in compliance with the
Product Control Approval guidelines. All other layers shall be adhered in a full mopping of approved
asphalt applied within the EVT range and at a rate of 20 -40 lbs. /sq., or mechanically attached using the
fastening pattern of the top layer. Refer to manufacturer's literature and/or the NRCA Roofing and
Waterproofing Manual for correct application procedures of insulation panels in approved asphalt or

applications, prior to.the application, all existing roof surfaces used as a bonding substrate
shall be tested in compliance with Metro -Dade County Protocol PA for uplift resistance. Test
pressures shall be calculated in compliance with the wind load requirements of Chapter 23 of the South
Florida Building Code for the roof in question.

3401.10(m), (n) of the South Florida Building Code.

If required, any Factory Mutual Approved vapor barrier in conjunction with proprietaryor approved
adhesives may be used prior to the application of the insulation layer.
Consult current Underwriters Laboratories Directory for the appropriate coating for each roofing
assembly to obtain the required fire rating. The assembly shall be installed in strict compliance with
Subsection 3401.5 and 3401.6 of the South Florida Building Code for maximum fire classification.

specific conditions of this Acceptance.
Renewals of Acceptance will not be considered if:
) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted,
is no longer practicing the engineering profession.
Any revision or change in the materials, use, and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been requested
t,through the filing of a revision application with appropriate fee) and granted by this office.
Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any tither
purposes.
The Notice of Acceptance number preceded by the words Metro -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the Notice of Acceptance is
