NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documents, including test supporting data, engineering documents, are no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the following
statement: "Dade County Product Control Approval", or as specifically stated in the specific conditions of this
Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and the
product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the correct
installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted, is
no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be cause
for termination of this Acceptance, unless prior written approval has been requested (through the filing of a revision
application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other purposes.
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the expiration date
may be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed, then it shall be done
in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be provided to
the user by the manufacturer or itsrdistributors and shall be available for inspection at the job site at all time. The
copies need not be resealed by the engineer.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.
9. This Acceptance contains pages 1, 2, 2(a) through 2(c) and this last page 3.
items 10. 11 & 12 listed below only apply to glazed products and doors
10. Unless specifically indicated in the Acceptance (approval), this unit is approved as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is required from the Product Control Section.
11. The spacing of fasteners at window sills shall be as indicated in Section 4 of this Notice of Acceptance. The spacing of
fasteners in all other parts of the frame, shall be as indicated in Section 4 of this Notice of Acceptance, but in no case
shall exceed 24" on center. The first fastener shall be located at a maximum of 6" from each corner and mullion or
stile. Fastener shall fully penetrate the buck, which shall be the same size as the one tested with the unit. No wood or
plastic shields or pins shall be used for anchoring.
12. Hardware for all windows and doors shall conform to Security and Forced Entry Prevention, Chapter the South
Florida Building Code.

1. DESCRIPTION
1.1 This approves a 20 ga. steel storm panel designed to comply with the South Florida Building Code,
1994 Edition for Dade County, for the locations where the pressure requirements as determined by
ASCE 7 -88 "Minimum Design Loads for Buildings and Other Structures ", do not exceed the Design
Pressure Rating values in Section 7 and within the limitations contained in Section 3.
2. MATERIAL CHARACTERISTICS
2.1. The storm panels shall be constructed as indicated on approved Drawing number 96- 20GA01, Sheet
1 of 2 , using 20 ga. steel, (min. thickness .035"), structural grade ASTM A653 Grade 37, (Fy = 37
ksi min.), Coating: Hot -Dip Zinc - Coated Galvanized ASTM A525, Reg. Spang. G60.
2.2 Panels are rolled formed, have a nominal width of 13.00" and a total width of 15.57 ", forming
1.375" deep ribs.
2.3 Components shall be aluminum alloys 6063 -T6, 2.81" x 1.475" x 0.25" special shape- header, 2.0" x
2.0" x .125" studded angle.
2.4 Tested specimen:

56.25" wide x 68" long overall - 1 specimen for impact and cyclic load

3. LIMITATIONS
3.1 The following mounting conditions shall be limited to a Maximum Design Pressure rating of
+ 71.5 & - 55.0 PSF and a Maximum Panel Span/Height of 7' -2" or (86 "); Detail A- Wall Mount
(Direct Mount System), Detail B - Header / Studded Angle Mount System, Detail C & E - Studded
Angle / Top & Bottom Mount, as shown on, ._Drawing number,96- 20GA01, Sheet 2 of 2.
3.2 Minimum separation to existing glass to protect shall be 2 5/8" as shown on Drawing number
96- 20GA01. Sheet 2 of 2.
3.3 Maximum clearance between top of panel and of header shall be 1/4" .

4. INSTALLATION
4.1 The panels shall be installed only in the following condition (all installations shall include 1/4" x 1"
sidewalk bolt w /steel spring nut at midspan to secure each lap joint):
4.1.1 Direct Wall Mounts:
(CONCRETE /MASONRY/WOOD): The panels are secured directly to the wall at the head
and sill, with 1/4" x 2" Elco Panel Mate Masonry Bolt with one 1/4" galv. washer and washered
winged nut spaced at 13" o.c. (concrete, masonry or wood) or 1/4 " -20 xl" hex head bolt with 1/4" 4)
Anchor spaced at 6.5" o.c. (concrete or masonry) QI 1/4 " -20 xl" sidewalk bolt with 1/4" 4) Anchor
spaced at 6.5" o.c. (concrete or masonry) al/4"-20 x 1" wood bushing with 1/4 " -20
x 1" hex head bolt SS spaced at 6.5" o.c. (wood) Qr 1/4 " -20 x 1" wood bushing with 1/4 " -20 x 1"
sidewalk bolt 3/4" spaced at 6.5" o.c. (wood), as detailed on Drawing number 96- 20GA01, Sheet 2
of 2, bearing the Dade County Product approval stamp.
4.1.2 Wall Mounts:
(CONCRETE /MASONRY/WOOD): The panels are secured to an 'H'- header (Part 2), at the
head and attached with any of the fasteners used in section 4.1.1 (A) or 1/4" x 2 1/4" Tap Con with
Hex head screw , 1 1/2" min. embedment spaced at 6.5" o.c. (concrete or masonry) or 1/4 "4) wood
lag with 1 1/2" min. embedment (wood) . The sill is a studded angle secured with the same anchors
as the head or 1/4" x 1 1/2" concrete drive nail with expansion anchor. spaced at 6.5" o.c. (concrete).
Studded angle may substitute the 'H'- header using all the anchors approved for the sill studded
angle, as shown in detail T", Drawing number 96- 20GA01, Sheets 2 of 2.
4.1.3 Built -Out Mounts:
(CONCRETE / MASONRY/WOOD FRAMED ): The mounting conditions of sections
4.1.1 and 4.1.2 may be installed on a 2" x 4" x 0.125" Alum. Tube or 2" x 4" P.T. wood for stud
framing which is spaced at 16" o.c. Qr 2" x 6" P.T. wood for stud framing which is spaced at 24"
- o.c:: - - Anchoring of built -out shall be with.2.1 /4_ "_.4_ x_._4.1 /2 "__Lag Screw spaced at max. 24" o.c. with
min. 1 3/4" embedment into center of studs.
4.1.4 Any combination of the above,
4.2 Anchorage of the shutters shall be to concrete, masonry and/or wood framing and shall consist of
only the anchors specified on Drawing number 96- 20GA01, Sheets 1 and 2 of 2. Substitution of
anchor type or indicated embedment is unacceptable.
4.3 Fasteners must have their own Notice of Acceptance and must be made of stainless steel or have
adequate protection against corrosion, per DIN 50018. Aluminum contacting metals not considered
compatible shall be properly protected.
4.4 Permanent set fastener components, embedded anchor bolts, threaded cones or metal shields, not in
use, must be protected against corrosion, contamination and damage at all time.

5. IDENTIFICATION
5.1 Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Dade County Product Control Approved ".
5.2 Also a Warning Note shall be permanently labeled at each opening to be protected or, in the case of
direct mounted panels with no header and sill, each panel shall have a legible permanent marking in
each panel advising the home owner or tenant that the storm panels will not offer hurricane
protection unless all reinforcing bolts and/or straps are properly installed.
6. USE
6.1 Application for building permit shall be accompanied by two copies of the following:
6.1.1 This Notice of Acceptance.
6.1.2 Completely dimensioned drawing showing size and location, including height above grade,
of openings to receive shutters; mean roof height, length and width of building.

NOTICE OF ACCEPTANCE STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documentation, including test supporting data, engineering documents, are no older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the following
statement: "Dade County Product Control Approved ", or as specifically stated in the specific conditions of this
Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and
the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation initially submitted ,
is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be cause
for termination of this Acceptance, unless prior written approval has been requested (through the filing of a revision
application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other purposes.
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the expiration date
may be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed, then it shall be done
in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be provided to the
user by the manufacturer or its distributors and shall be available for inspection at the job site at all times. The copies
need not be resealed by the engineer.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.
9. This Acceptance contains pages I, 2, 2(a) through 2(d) and this last page 3.
Items 10. 11 & 12 listed below only apply to glazed products and doors
10. Unless specifically indicated in the Acceptance (approval), this unit is approved as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is required from the Product Control Section.
1 1. The spacing of fasteners at window sills shall be as indicated in Section 4 of this Notice of Acceptance. The spacing of
fasteners in all other parts of the frame, shall be as indicated in Section 4 of this Notice of Acceptance, but in no case
shall exceed 24" on center. The first fastener shall be located at a maximum of 6" from each corner and mullion or
stile. Fastener shall fully penetrate the buck, which shall be the same size as the one tested with the unit. No wood or
plastic shields or pins shall be used for anchoring.
12. Hardware for all windows and doors shall conform to Security and Forced Ent Prevention, ' hapter the S
Florida Building Code.

NOTICE OF ACCEPTANCE STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has becn filed and the original
submitted documentation, including test supporting data, engineering documents, are no older than eight (8).ycars.
2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the follow
statement: "Dade °County Product Control Approved ", or as specifically stated in the specific conditions of this
Acceptance.
3. Renewals of Acceptance will not be considered if
a) There has been a change in the South Florida building Cote affecting the evaluation of this product and
the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as th one originally approved;
c) If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product;
d) The engineer who originally prepared, signed and scaled the required documentation initially submitted,
is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be cau-
for termination of,this Acceptance, unless prior written approval has been requested (through the tiling of a revision
application with appropriate fee) and granted by this office.

5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process; •
b) Misust; of this Acceptance an endorsement of any product, for sales, advertising or any other purposes.

6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the expiration date
niay be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed, then it shall be dot
in its entirety.
7. A copy of this Ac as well as approved drawings and other documents, where it applies, shall be provided to tl
user by the manufacturer or its distributors and shall be available for inspection at the job site at all times. The copies
need not be resealed by the engineer.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.
9. This Acceptance contains pages 1, 2, 2(a) through 2(e) and this last page 3.

10. Unless specifically indicated in the Acceptance (approval), this unit is approved as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is' required from the Product Control Section.
11. The spacing of fasteners at window sills shall be as tested. The spacing or fasteners in all other parts of the frame, shal
be as tested, but in no case shall exceed 24" on center. The first fastener shall be located at a maximum of 6" from earl
Omer and mullion or stile. Fastener shall fully penetrate the buck, which shall be the same size as the one tested with
the unit. No wood or plastic shields or pins shall be used for anchoring'. See section four (4) of this approval for sizes
fasteners used in testing.
12. Hardware for all windows and doors shall conform to Security and Forced Entry Prevention, Chapter 36 of the South
Florida Building Code.

NOTICE OF ACCEPTANCE: SPECIFIC CONDITIONS
1. DESCRIPTION OF UNIT
1.1 This approves an aluminum sliding glass door designed to comply with the South Florida
Building Code, 1994 Edition for Dade County, for the locations where the pressure requirements, as
determined by ASCE 7 - 88 "Minimum Design Loads for Building and Other Structures ", do not exceed
the Design Pressure Rating values in Section 7 and within the limitations contained in Section 3.
1.2 Model Designation: Series #100 Aluminum Sliding Glass Door
1.3 Overall Size: 12' 0 3/4" wide by 6' 7 3/4" (79 3/4 ") high x 4" deep.

1.5 No. & Size of Panels: Three: Two moving panels at interior track, 49 1/4" by 79" high and 48
5/16" x 79" high; One fixed panel, 48 1/4" by 79" high.
2. MATERIAL CHARACTERISTICS
2.1 Frame and Door Panel Aluminum alloy 6063 -T5.
2.2 Glazing
2.2:1 Glazing Material: Full lite, 3/16" tempered glass.
2.2.2 Glazing Method: Channel glazed using a flexible channel vinyl glazing bead.

2.3 Frame Construction;
2.3.1 Head: A .937" face x 4" deep x 0.62" wall thickness extruded aluminum 2 -track channel.
2.3.2 Jambs: A .688" face x 3.843" deep x 0.62" wall thickness extruded aluminum 2 -track jamb
with 2 pile groove per track and two screw splines.
2.3.3 Track: A .155" high, .375" to top of track x 4.654" wide x 2" high overall interior sill
flange extruded aluminum track is used at sill.
2.3.4 Corner construction: Corners are butt joined, frame sill corners are sealed with sealant,
two #6 x 1/2" RH SMS into jamb, per corner are employed.
2.4 Panel Construction: -
2.4.1 Stiles: Lock stiles consist of 1" deep x 1.850" face x .060" wall thickness extruded
aluminum tube with a 375" x .587" pocket for glazing; interlock stiles are 1.781" deep x 1.500"
face x .062" wall thickness aluminum tube with a .375" x .587" pocket for glazing, a double
bumper and a pile groove; female astragal stile is 2.402" face x 2.875" deep x .062 "/.093" wall
thickness aluminum tube with a .375" x .587" pocket for glazing and a pile groove.
2.4.2 Rails: A .875" deep x 2" face x .060" wall thickness extruded aluminum tube with a .375" x
1/2" pocket and one screw spline is used at top and bottom rails.
2.4.3 Corner construction; Panel corners are butt joined, top and bottom rails are secured to
stiles with one #8 x 1" SMS.

SILL : Double Row #8 x 1 1/4" Flat Head SMS located 6" from corners and 24" o.c. max.

JAMBS : Double Row #8 x 1 1/4" Flat Head SMS located 5 1/2" from corners and 19" o.c. max.

Fixed Panel: Jamb stile is anchored to frame jamb using one 3/4" x 1/2" x 3/32" x 1 1/4" long aluminum
'angle secured to frame and stile, each with one #8 x 1/2" tek screw, at 7" from each end and one at
midspan. Interlock stile is anchored to the sill using one 1 1/4" x 1" x 1/8" x 1 1/2" long aluminum angle
at top and bottom of stile, each secured with two 118 x 1 1/4" SMS into frame and two #8 x 1" tck screws
into stile. One 1" x 1" x 1/8" x 4 3/8" long aluminum retainer angle is used in frame sill at inside of
astragal stiles, secured with three #8 x 1 1/4" SMS into frame sill.
4.2 Sub -bucks shall be of pressure treated material at least 2" nominal thickness, and secured in at least 6
points on each leg with 3" long fasteners or equivalent.
4.3 For retrofit installations where the existing buck is less than the required 2" nominal; use concrete
anchors instead of the fasteners above, the same amount and diameter and an appropriate length to go
through the buck and penetrate at-least 1 1/4" into the masonry structure.
4.4 Fasteners must have their own Notice of Acceptance and must be made of stainless steel or have
adequate protection against corrosion, per DIN 50018. Aluminum contacting metals not considered
compatible shall be properly protected
5. IDENTIFICATION
5.1 Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following
statement: "Dade County Product Control Approved ".

6.1 Application for building permit shall be accompanied by two copies of the following:
6.1.1 This Notice of Acceptance
6.1.2 Completely dimensioned drawing showing size and location, including height above grade
of opening to receive door, mean roof height, length and width of building.
6.1.3 Manufacturer's installation instructions.
6.2 A copy of this approval as well as the manufacturer's installation instructions shall be provided to the
permit applicant by the manufacturer or his distributors and shall be available for inspections at the
jobsite at all time.
6.3 The Building Official shall ensure the adequacy of door to meet the pressure requirement of the
opening in which it is to be installed.

3. Renewals of Acceptance will not be considered if
a) There has been a change in the South Florida Building Cade affecting the evaluation of
this product and the product is not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
• c) lithe Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product;
d) The engineer who originally prepared, signed and sealed the required documentation
initially submitted , is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically be
cause for termination of this Acceptance, unless prior written approval has been requested (through the fling of a
revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
other purposes.
6. The Notice of Acceptance number preceded by the words Dade County, Florida, and followed by the expiration da
may be displayed in advertising literature. If any portion of the Notice of Acceptance is displayed, then it shall be
done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be provided 1.

copies need not be resealed by the engineer.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.

10. Unless specifically indicated in the Acceptance (approval), this unit is approed as a single unit installation. For
multiple installation of this unit, a separate Acceptance for mullions is required from the Product Control Section.
11. The spacing of fasteners at window sills shall be as tested. The spacing of fasteners in all other parts of the frame,
shall be as tested, but in no case shall exceed 24" on center. The first fastener shall be located at a maximum of 6"
from each corner and mullion or stile. Fastener shall fully penetrate the buck, which shall be the same size as the on
tested with the unit. No wood or plastic shields or pins shall be,uscd for anchoring. See section four (4) of this
approval for sizes of fasteners used in testing.
12. Hardware for all windows and doors shall conform to Security and Forced Entry reventio
Prevention, Chapter 36 of the Sou
