which requires a separate permit.
6. PORTABLE TOILETS for a construction site require a separate permit.
7. DO NOT DISCHARGE WATER INTO THE RIGHT OF WAY OR STORM DRAINS without approval from the Building, Planning and Zoning Department.
8. EQUIPMENT AND MATERIALS SHALL BE STORED at least 10 feet from the edge of pavement.
9. Department of Health and Rehabilitative Services (HRS) approval is required for applications involving septic tanks. Department of Environmental Resources
Management (DERM) and/or Miami -Dade Water and Sewer Department (MDWASD) approval is required for applications involving sewers.

This approves sectional door 9" wide x 8' -0" through 6' -6 "" high made out of steel, drawing quality
Grade 28,000 KSI (ASTM A525), galvanized coating of G60 (ASTM A525) and finished with baked
on polyester coating. The door shall be constructed and reinforced as detailed in the approved drawing

This door is approved for a maximum design pressure of +42PSF and -48PSF and its use is restricted
to this pressure. The door passed the impact and cyclic test; therefore it does not require hurricane
protection devices. The door shall meet the burglary security requirement of section 3603.2(c)2(aa) of
the South Florida Building Code. The door operating mechanism (not included in this approval) shall
be certified by Underwriters Laboratories or other recognized agency.

The door shall be installed as shown on Drawing 4101702-24. Wood in contact with concrete or
cement shall be treated with an approved preservative as set forth in paragraph 2913.2(a)1 or shall be
of a durable species as defined in subparagraph 2913.2(a)6(aa) of the South Florida Building Code.
Installation of 2" x 6" wood jamb requires a `/4" x 3 ITW tapcon and 1" 0 x 1/16" thick washer @
8" from bottom & 16" o/c with a minimum embedment of 1'/4" into 2000 PSI (minimum) concrete or
grout filled concrete block. Or 3/8" x 4" Rawl Lok/bolt and 1" 0 x 1/16" thick washer @ 8" from
bottom and 24" o/c with a minimum embedment of 1 `/," into 2000 PSI (minimum) concrete or grout
filled concrete block. Installation of track requires a 5/16" x 11/2"wood lag screw at each track and flag

Southern Yellow Pine PT 2" x 6" wood jamb.
4.0 CONDITIONS OF USE:
Application for Building permit shall be accompanied by duplicate prints of the approved Drawing
#101702-24 bearing the approval stamp, Notice of Acceptance number and date by the Dade County
Product Control Section along with computations prepared by a qualified Florida Professional
Engineer or Architect verifying the adequacy of the door at each site. The shop drawing shall bear the
signature of the Architect or Engineer with the following statement: The supporting structure has been
designed for the forces shown on the approved manufacturer's drawing ". Application for building
permit for replacement doors shall be accompanied by duplicate prints of the approved Drawing
g-101702-24 bearing the approval stamp, Notice of Acceptance number and date by the Dade County
Product Control Section. Also the building official must be satisfied that the proposed installation
meet the use of Section 2 above.

NOTICE OF ACCEPTANCE: SPECIFIC CONDITIONS
This approval requires the manufacturer to do testing of all coils used to fabricate door panels under
this Notice of Acceptance. A minimum of 2 specimens shall be cut from each coil and tensile tested
(according to ASTM E -8) by a Dade County approved laboratory selected and paid by the
manufacturer. Every 3 months, four times a year, the manufacturer shall mail to this office: a copy of
the test reports with confirmation that the specimens were selected from coils at the manufacturer's
production facilities and a notarized statement from the manufacturer that only coils with a yield
strength of 28,000 PSI or more shall be used to make door panels for Dade County under this Notice of
Acceptance.
All approved products shall be permanently labeled with the manufacturer's name, city, state and the
following statement: "Dade County Product Control,Approved ".

Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the original
submitted documentation, including test supporting data, engineering documents, are no older than eight (8) years.
2.
Any and all approved products shall be permanently labeled with the manufacturer's name, city, state and the following
statement: "Dade County Product Control Approved ", or as specifically stated in the specific conditions of this
Acceptance.
3. Renewals of Acceptance will not be considered if:
a) There has been a change in the South Florida Building Code affecting the evaluation of this product and the product is •
not in compliance with the code changes;
b) The product is no longer the same product (identical) as the one originally approved;
c1 If the Acceptance holder has not complied with all the requirements of this acceptance, including the correct
installation of the product:
d) The engineer, who originally prepared, signed and sealed the required documentation initially submitted, is no longer
practicing the engineering professions.
4. Any revision or change in the materials, use, and /or manufacture of the product or process shall automatically be cause for
termination of this Acceptance. unless prior written approval has been requested (through the filing of a revision application
with appropriate fee) and granted by this office.
Anv of the following shall also be grounds for removal of this Acceptance:
a) Unsatisfactory performance of this product or process;
b) Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other purpose.

user by the manufacturer or its distributors and shall be available for inspection at the job site at all times. The engineer
need not reseal the copies.
Failure to comply with any section of this Acceptance shall be cause for termination and removal of Acceptance.

10. Unless specifically indicated in the Acceptance (approval). this unit is approved as a single unit installation. For multiple
installation of this unit, a separate Acceptance for mullions is required from the Product Control Section
11. The spacing of fasteners at windowsills shall be as indicated in Section 4 of this Notice of Acceptance. The spacing of
fasteners in all other parts of the frame shall be as indicated in Section 4 of this Notice of Acceptance, but in no case shall
exceed 24" on center. The first fastener shall be located at a maximum of 6 from each corner and mullion or stile.
Fastener shall fully penetrate the buck, which shall be the same size as the one tested with the unit. No wood or plastic
shields or Dins shall be used for anchoring.

The charts included with this cover page help calculate the design
wind loads as determined using the American Society of Civil
Engineers Code ASCE7 -88 for use with garage doors. The charts are
divided between single car garage doors (up to 9'W x 8'H) and
double car garage doors (up to 16'2 "W x 8'H).
1. Determine the garage door size as a single car door (up to 9'W
x 8'H) or double car door (up to 16'2 "W x 8'H).
2. Measure the "Mean Roof Height" of the building. The mean roof
height is measured as the average height of the roof, not the

peak of the roof. The eave height may be used for a roof
slope of less than 10°.
3. With the door i e oo size and mean roof ei ht known, the appropriate

calculation chart can be used.
4. Determine the "Minimum Building Dimension ". For example, a
building (or house) with a 80' x 30' floor plan, has a minimum
building dimension of 30'.
5. Measure the distance from the corner of the building to the
nearest edge of the door opening. The nearer the door opening
to a corner, the higher the negative design wind load. The
corner wind zone of a building is defined as 10% of the
minimum building dimension or 40% of the mean roof height,

minimum building dimension.

6. The wind load can now be determined. For example a single car
garage door is to be installed in a house with a mean roof
height of 15' or less and a 80' x 30' floor plan. The door
opening is located 2' from a corner. The design wind loads
are (using the charts): +39.9 PSF and -43.5 PSF.

IN CONSIDERATION of the approval of this permit by the Village, the Owner agrees as follows:
1. To maintain and repair, when necessary, the above- mentioned item(s) installed within the dedicated right
of way. If it becomes necessary for Miami Shores Village or Dade County to make repairs or maintain said
items within public right of way including restoration of street by reason of the Owner's failure to do so,
such expense shall be paid by the Owner or shall constitute a lien against the above described property
until paid.
2. The owner does hereby agree to indemnify and hold Miami Shores Village or dade County harmless from
any and all liability, which may rise by virtue of permitting the installation of these items within the public
right of way.
3. The Owner does hereby agree to remove or relocate their facilities at their own expense, within 60 days
notice by the Village to do so. Failure to comply with this notice will result in the Village causing the
item(s) to be removed and a lien being placed on the property and/or assessed against the Owner for all
costs incurred in the removal and disposal of the item(s).
4. The undersigned further agrees that these conditions shall be deemed a covenant running with the land and
shall remain in full force and effect and be binding on the undersigned, their heirs and assigns, until such
time as this obligation has been canceled by an affidavit filed in the Public records of Dade County, Florida
by the Village Manager of Miami Shores Village (or his fully authorized representative).
