This testing was performed in general accordance with Roofmg Application Standard TAS No.106 Standard procedure for field verification of the bonding of mortar or adhesive set tile system and mechanically attached, rigid, discontinuous roof systems. The total of the tested roof surface area was estimated to be less than 28,000 square feet, and the mean height of the roof is less than 30 feet above ground surface. The type of tile used for this project was reported to be Monier Flat Roof Tile. This tile was reported to have been foamed in place. At the time of this testing at least one test for every 100 square feet of roof tile in the perimeter area and one test for every 200 square feet of roof tile in the field area were performed. Walking on the roof surface or any other type of disturbance may loosen the tiles and alter the results of this test. Based on our test results, we conclude that the installation of the roof tile at the above referenced project meets the test requirement outlined in the above mentioned protocol. Building materials samples were analyzed according to the Environmental Protection Agency (EPA) Polarized Light Microscopy (PLM) Method. A copy of the laboratory analytical results and chain -of- custody documentation is provided as Attachment C. GFA was not informed of additional building materials that existed at the subject property prior to this survey. If, during renovation activities, additional materials are discovered that have not been tested or if any materials are found that were not accessible at the time of the survey, they should be considered Presumed Asbestos Containing Materials (PACM) in accordance with Chapter 1910.1001 of the Occupational Safety and Health Administration's (OSHA) Title 29, Part 1910, Subpart Z of the CFR, until laboratory testing proves otherwise. In addition, the contractor should notify the subject property owner and GFA of any such materials for further evaluation. A copy of the survey should be kept onsite during renovation activities. This report may not be used by the client to claim product endorsement by NVLAP or any agency of the U.S. Government. Installers are cautioned to refer to applicable local building codes prior to direct deck installation to ensure this is acceptable. All rolls, with the exception of Polystick TU Plus should be back -nailed in selvage edge seam as per Polyglass Back Nailing Guide. As it pertains to the section, it is the responsibility of roofing contractor to provide the owner with the required roofing permit, and to explain to the owner the content of the section. The provisions of Section R4402 govem the minimum requirements and standards of the industry for roofing system installations. If mechanical attachment to the structural deck through the lightweight insulating concrete is proposed, a field withdrawal resistance testing shall be performed to determine equivalent or enhanced fastener patterns and density. All testing and fastening design shall be in compliance with Testing Application Standard TAS 105 and Roofing Application Standard RAS 117; calculations shall be signed and sealed by a Florida Registered Engineer, Architect, or Registered Roof Consultant. 2. For steel deck application where specific deck construction is not referenced: The deck shall be a minimum 22 gage attached with 5/8" puddle welds with weld washers at every flute with maximum deck spans of 5 ft. o.c. 3. For systems where specific lightweight insulating concrete is not referenced, the minimum design mix shall be a minimum of 300 psi. GENERAL LIMITATIONS: 1. Fire classification is not part of this acceptance; refer to a current Approved Roofing Materials Directory for fire ratings of this product. 2. Insulation may be installed in multiple layers. The first layer shall be attached in compliance with Product Control Approval guidelines. All other layers shall be adhered in a full mopping of approved asphalt applied within the EVT range and at a rate of 20- 40lbs./sq., or mechanically attached using the fastening pattern of the top layer 3. All standard panel sizes are acceptable for mechanical attachment. When applied in approved asphalt, panel size shall be 4' x 4' maximum. 4. An overlay and/or recovery board insulation panel is required on all applications over closed cell foam insulations when the base sheet is fully mopped.
