Permittee is also responsible for the General Permit Conditions which are part of this permit. 24 hour notification shall be made to Miami Dade County Department of Transportation and Public Works, phone 305-375- 2392 when work for which this permit is granted is ready for construction. This permit is issued subject to actual work being done by a contractor properly certified by the CONSTRUCTION TRADES QUALIFYING BOARD. In accepting this permit, the permittee agrees to hold thereof harmless from payment of any compensation or damages, resulting from his exercise of the privileges granted under this permit. Construction must be in accordance with the approved plan, and/or to recorded portions of the Public Works Manual. Contractor/Permittee shall contact DTPW (305) 375-2392 48 hours in advance to meet inspector prior to final restoration of the right of way. All required Traffic Control devices must be installed prior to placement of asphalt surface. All required MOT devices shall be installed prior to beginning of construction. There shall be no interruption of traffic permitted Monday -Friday 7 to 9 AM and 4 to 6 PM unless the Traffic Maintenance and Safety Provisions indicate other restricted hours. There shall be no interruption of pedestrian, bicycle or vehicular traffic on roadways surrounding schools during school arrival/dismissal hours. Any variations from these requirements will require expressed written permission from the Public Works Director. Failure to close permits, or request the necessary inspections, will incur the Permittee/Contractor in a civil violation as per section C1.07 (Inspection) of the Public Works Manual and Chapter 8 section 2-103.1 of the Miami -Dade County Code.
