Do hereby petition the Village of Miami Shores to act as my own contractor pursuant to the laws of the State of Florida, F.S 489.103(7). And I have
read and understood the following disclosure statement, which entitles me to work as my own contractor; I further understand that I as the owner
must appear in person to complete all applications.
State Law requires construction to be done by a licensed contractor. You have applied for a permit under an exception to the law. The exemption
allows you, as the owner of your property, to act as your own contractor even though you do not have a license. You must supervise the
construction yourself. You may build or improve a one -family or two-family residence. You may also build or improve a commercial building at a
cost of $25,000.00 or less (The new form states 75,000). The building must be for your own use and occupancy. It may not be built for sale or
lease. If you sell or lease a building you have built yourself within one year after the construction is complete, the law will presume that you built
for sale or lease, which is a violation of this exemption. You may not hire an unlicensed person as a contractor. It is your responsibility to make
sure the people employed by you have licenses required by state law and by county or municipal licensing ordinances. Any person working on
your building who is not licensed must work under your supervision and must be employed by you, which means that you must deduct F.I.C.A and
with -holdings tax and provide workers' compensation for that employee, all as prescribed by law. Your construction must comply with all
applicable laws, ordinances, buildings codes and zoning regulations.

Upon issuance of permits for the scope of work involving the removal, changing and/or replacement of
any type of windows, doors, sidelites, skylights or fixed glass (cladding) the permit holder or qualifier
bearing his signature on the permit application shall abide by the requirements of this department and
comply with the following statement:
Upon obtaining window and/or door permits for the installation of same, it is the responsibility of the
permit holder to request window/door framing in -progress inspection, prior to concealment of any
horizontal or vertical clip mullion, bucks, shims, etc. Inspector will also verify anchor type, edge distance,

NOTICE OF ACCEPTANCE: SPECIFIC CONDITIONS
1. SCOPE
1.1 This approves an aluminum casemeht window, as described in Section 2 of this Notice of
Acceptance, designed to comply with the South Florida Building Code, 1994 Edition for Miami -
Dade County, for the locations where the pressure requirements, as determined by SFBC Chapter
23, do not exceed the Design Pressure Rating values indicated in the approved drawings.
2. PRODUCT DESCRIPTION
2.1 The Series "C-1000" Ontswing Al iminumm Casement Window — Impact Resistant and its
components shall be constructed in strict compliance with the following documents: Drawing No.
W01--23, Sheets 1 through 4 of 4, titled "Series "1000" Outswing Aluminum Casement Window,"
prepared by Al Farooq Corporation, dated 4/30/01, with revision A dated 12/20/01, signed and
sealed by Humayoun Farooq, P.E., bearing the Miami Dade County Product Control Approval
stamp with the Notice of Acceptance number and approval date by the Miami -Dade County Product
Control Division. These documents shall hereinafter be referred to as the approved drawings.
3. LEffrATIONS
3.1 This approval applies to single unit applications of X and XX window configurations, as shown in
approved drawings.
3.2 For Design Pressure Rating vs. Window Size and Configuration, see Comparative Analysis Tables
in approved drawings.
4. INSTALLATION
4.1 The aluminum casement window and its components shall be installed in strict compliance with the
approved drawings.
4.2 Hurricane protection system (shutters): the installation of this unit will not require a hurricane
protection system.
5. LABELING
5.1 Each window unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved".
6. BUILDING PERMIT REQUIREMENTS
6.1 Application for building permit shall be accompanied by copies of the following:
6.1.1 This Notice of Acceptance.
6.1.2 Duplicate copies of the approved drawings, as identified in Section 2 of this Notice of
Acceptance, clearly marked to show the components selected for the proposed installation.

NOTICE OF ACCEPTANCE: SPECIFIC CONDITIONS
1 Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed
and the original submitted documents, including test -supporting data, engineering documents, are no
older than eight (8) years.
2. Any and all approved products shall be permanently labeled with the manuf acturces name, city,
state, and the following statement: "Miami -Dade County Product Control Approval", or as
specifically stated in the specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a. There has been a change in the South Florida Building Code affecting the evaluation of this
product and the product is not in compliance with the code changes.
b. The product is no longer the same product (identical) *as the one originally approved.
c. If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product.
d. The engineer who originally prepared, signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been
requested (through the filing of a revision application with appropriate fee) and granted by this
office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a. Unsatisfactory performance of this product or process,
b. Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any
other purposes.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the
Notice of Acceptance is displayed, then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies,
shall be provided to the user by the manufacturer or its distributors and shall be available for
inspection at the job site at all time. The engineer need not reseal the copies.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.

This NOA is being issued under the applicable Hiles and regulations governing the use of construction materials.
The docu mentatum submitted has been reviewed by Miami -Dade County Product Control Division and accepted
by Board of Rules and Appeals (BORA) to be used in Miami Dade County and other area where allowed by
the Authority Having Jurisdiction (A117).
.This NOA shall not -be valid after the expiration date stated below. The Miami -Dade County Product Control
Divisim (In Miami DadeCounty) and/or the AM (in areas other tuna Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fads to perform in
