Dear Mr. and Mrs. Faccidomo
Attached please find the development order adopted by the Miami Shores Planning and Zoning
Board on January 25, 2016 under file number PZ-01-06-201612 and the as built certification
provided by your design professional Mr. Victor Bruce, on January 26, 2018. Contrary to the
certification provided by Mr. Bruce, the project does not meet the conditions of the
development order nor the approved plans. You, as the property owner shall take immediate
action to obtain the require driveway permit and to complete the project as per the approved
site plan and development order requirements. Failure to obtain a permit within 10 days and
to complete the project within 30 days may result in the Building Official having to revoke or
suspend the certificate of occupancy for the property as authorized under section 111.4 of the
Florida Building Code.
2017 Florida Building Code, Building. [A] 111.4 Revocation. The building official is authorized
to, in writing, suspend or revoke a certificate of occupancy or completion issued under the
provisions of this code wherever the certificate is issued in error, or on the basis of incorrect
information supplied, or where it is determined that the building or structure or portion
thereof is in violation of any ordinance or regulation or any of the provisions of this code.
REQUIREMENTS FOR CORRECTION:
1. Obtain driveway permits by Thursday, September 6, 2018
2. Pass required inspections by Monday, September 24, 2018

hazards, you may be granted upon request an extension of time up to 30 days to correct the
