Record of Field Observations: 1. Visual verification of the structural steel connections at the following locations indicated below: a) ½” Ø x 1½” shear studs welded connections to the proposed new C10x25 steel channels approx. between grids “C.5” thru “D.5” and lines “5.5” thru “6.5 for the precast concrete double “T’s” repair at the existing service department, as per Conemco Engineering, Inc. precast concrete double Tee’s structural repair drawings, as we addressed on UES Structural Observation Reports No. 38 (No.: 10040) dated 07/07/2023 and No. 41 (No.: 14712) dated 09/06/2023, re-inspection inconclusive (09/12/23) – pending current welder qualification certification (continuity paperwork- period of effectiveness): ➢ Welding slag was not completely removed from locations that require Visual Testing (VT). ➢ Incomplete or undersize fillet welds. ➢ Missing three (3) shear studs at locations marked in the field. ➢ A couple of shear studs were found to be spaced more than 8” o.c . b) New C10x25 steel channels bolted/anchored connections to the existing roof precast concrete double Tee’s/existing concrete beam approx. between grids “C.5” thru “D.5” and lines “5.5” thru “6.5 for the precast concrete double “T’s” repair at the existing service department, as per Conemco Engineering, Inc. precast co ncrete double Tee’s structural repair drawings, as we addressed on UES Structural Observation Report No. 044 (No.: 17436) dated 09/28/2023, inconclusive on 10/25/23: ➢ Pending final compliance verification for item 1a above. ➢ Missing washers/nut at random locations. ➢ Anchor bolt threads were found not protruding (negative projection) beyond the outside surface of the nut (negative stick‐out) at several locations as per RCSC (Research Council for Structural Connections) Specification for Structural Joints Using ASTM A325 or A490 Bolts. ➢ One (1) loose-fitting thru bolt. ➢ One (1) screw anchor was found not to be ¾”. c) Additional support steel angle clips anchored/welded connections (torque verified at 100 ft−lb as per Simpson Strong−Bolt 2 Wedge Anchor required installation torque schedule) to continuous perimeter roof deck bearing angle L4”x4”x1/4”CIP and to CIP concrete beam along grid “E.4” between lines “11” thru “13” at the Quik Lube, as per DRAFT Project No.: 2110.2100049 Report No.: 198687 Date: January 2, 2024 8050 NW 77th Court · Miami, FL 33166 · P: 305.249.8434 · F: 305.249.8479 STRUCTURAL OBSERVATION REPORT No. SPECIAL-060 CN: GDZ Page 2 of 6 cc: 1 − Job Site; 1 − Client; 1 The Original of this Report was Signed and Impressed Sealed by the Above Registered Engineer in Accordance with Rule 21H-18.11, Chapter 471, Florida Statute S1.0, S1.1/details 2 & 3, S2.2, S3.2, S4.1/details 1 & 4, E1000, E1001 and attached letter submitted by the SEoR, completed. d) Continuous perimeter roof deck bearing angle L4”x4”x1/4” anchored connections (torque verified at 100 ft−lb as per Simpson Strong−Bolt 2 Wedge Anchor required installation torque schedule) to CIP concrete beam along grids “G” and “H” between lines “8” thru “9” at the Lobby, as per S1.0, S1.1/details 2 & 3, S2.2, 1/S3.3, 10/S4.1, 2/S4.2, E1000, E1001 and attached letter submitted by the SEoR, completed. e) W12x14 roof steel beam welded/bolted connections to CIP concrete tie beam along grid “G.5” between lines “8” thru “8.6” at the Lobby, as per S1.0, S2.2, 2/S3.2, 10/S4.1, 2/S4.3, E1000 and E1002, completed. The structural steel connections at the locations mentioned above were found to be in accordance with the reference documents, except for items 1a & 1b inconclusive. The welder qualification certificate for item “a” was provided during our site visit on 07/27/2023, however current continuity paperwork pending for review (Period of Effectiveness). The welders that performed the work were not available onsite in order to verify their welder qualifications. As per The Structural Welding Code: 6.2.3 Period of Effectiveness 6.2.3.1 Welders and Welding Operators. The welder’s or welding operator’s qualification as specified in this code shall be considered as remaining in effect indefinitely unless: (1) the welder is not engaged in a given process of welding for which the welder or welding operator is qualified for a period exceeding six months, or (2) there is some specific reason to question a welder’s or welding operator’s ability (see 6.25.1). Mr. Jim Jenco, Senior Superintendent with Lebolo Construction, was notified of the aforementioned visual verification results. Well the tie beam will take the chord forces. The deck angle here is really more used to finish the metal deck. With the beams spaced at 10 feet on center, the deck angle will need an additional support or 2 between beams. They can leave the angle toe-up and weld to the top of the beams. For an intermediate support, they can take a piece of L3x3x1/4”x4” long and weld it to the underside of the deck angle and bolt it into the face of the beam. They could also use a 5x5 angle and then the weld would be a downward weld rather than an overhead weld.
