• SUNCOR, Inc. structural steel erection/fabrication shop drawings sheets AB1 thru TP101 (total of 219 pages) dated
04/13/2022 (Rev. A – Issued for Approval) Submittal #051200-5.0 (Structural Steel Shop Drawings), reviewed for
compliance (stamped “Reviewed as Noted - Make Corrections”) by JSG on 05/03/2022.
• Canam Steel Corporation (CSC) steel metal deck shop drawings sheets DE1.1 thru DE2.1 (total of 3 pages) dated
04/08/2022 (Issued for Approval) Submittal #051200-6.0 (Deck Shop Drawings), reviewed for compliance (stamped
“Reviewed as Noted - Make Corrections”) by JSG on 04/15/2022.
Record of Field Observations:
1. Visual verification of the structural steel connections at the following locations indicated below:
a) ½” Ø x 1½” shear studs welded connections to the proposed new C10x25 steel channels approx. between grids
“C.5” thru “D.5” and lines “5.5” thru “6.5 for the precast concrete double “T’s” repair at the existing service
department, as per the attached Conemco Engineering, Inc. precast concrete double Tee’s structural repair
drawings, as we addressed on UES Structural Observation Reports No. 38 (No.: 10040) dated 07/07/2023 and No.
41 (No.: 14712) dated 09/06/2023, re-inspection inconclusive (09/12/23) – pending current welder
qualification certification (continuity paperwork- period of effectiveness):
➢ Welding slag was not completely removed from locations that require Visual Testing (VT).
➢ Incomplete or undersize fillet welds.
➢ Missing three (3) shear studs at locations marked in the field.
➢ A couple of shear studs were found to be spaced more than 8” o.c.
b) New C10x25 steel channels bolted/anchored connections to the existing roof precast concrete double
Tee’s/existing concrete beam approx. between grids “C.5” thru “D.5” and lines “5.5” thru “6.5 for the precast
concrete double “T’s” repair at the existing service department, as per the attached Conemco Engineering, Inc.
precast concrete double Tee’s structural repair drawings, failed on 09/28/23 – pending re-inspection:
➢ Missing washers at random locations.
➢ Anchor bolt thread does not protrude (negative projection) beyond the outside surface of the nut (negative
stick‐out) at several locations as per RCSC (Research Council for Structural Connections) Specification for
Structural Joints Using ASTM A325 or A490 Bolts.
➢ One (1) loose-fitting thru bolt.
➢ One (1) screw anchor was found not to be ¾”.
c) New W12x30 steel beam and MC12x50 steel channels welded/anchored connections (torque verified at 100 ft−lb
as per the anchoring adhesive HILTI HIT-HY 200 required installation torque schedule for HAS Threaded Anchor
Rods) to the existing CIP concrete columns/existing concrete CIP tie beam along grid “D.1” between lines “2.3” thru
“3.43” and between grids “D.1” thru “E” and lines “3.43” thru “3.6” for the retrofit at the existing service department,
as per S1.0R, failed on 09/28/23 – pending re-inspection:

➢ Some threaded anchor rods were found to be loose-fitting (not meeting the minimum torque of 100 ft−lb).
The structural steel connections at the locations mentioned above were found to be item 1a inconclusive and items 1b
and 1c failed. The General Contractor will call for items 1b and 1c re-inspection after completion for final compliance
verification. The welder qualification certificate was provided during our site visit on 07/27/2023, however current
continuity paperwork pending for review (Period of Effectiveness). The welder that performed the work was not available
onsite in order to verify his welder qualification.
As per The Structural Welding Code:
6.2.3 Period of Effectiveness
6.2.3.1 Welders and Welding Operators. The welder’s or welding operator’s qualification as specified in this code shall be
considered as remaining in effect indefinitely unless:
(1) the welder is not engaged in a given process of welding for which the welder or welding operator is qualified
for a period exceeding six months, or
(2) there is some specific reason to question a welder’s or welding operator’s ability (see 6.25.1).
2. Visual verification of the reinforcing steel placement at the following locations indicated below:
a) Proposed new 6” thick interior slab on grade with thickened edge reinforced with 6x6 – W2.9xW2.9 W.W.F. over a
10 Mil vapor barrier and compacted fill between grids “G” thru “J” and lines “8” thru “9” at Parts Receiving (104),
Lobby (125) and Mach. Room (126), top of slab elevation 0’−0”, as per S1.0, S2.0, 1/S3.1, 1/S3.3 and S4.0/details
1, 2, 5 & 6, completed. In place density tests were performed by a UES technician and it was found to meet or
exceed the minimum compaction requirement of 98% and ASTM D−1557. Termite treatment was performed and
vapor barrier was placed prior to this site visit.
b) 12” thick CMU load bearing walls along grids “G” and “H” between lines “8” thru “9” and along line “9” between grids
“G” thru “G.3” at Lobby (125) and Mach. Room (126), single lift from elevation approx. 0'−0" to approx. +7’−0”, as
per S1.0, S2.0, S2.2, S4.0/details 1, 3 & 6 and 7/S4.1, completed.
The reinforcing steel placement at the locations mentioned above was found to be in substantial compliance with the
reference documents.
Mr. Jim Jenco, Project Superintendent with Lebolo Construction, was notified of the aforementioned visual verification
results.
