on 08/22, (permit set review by the Miami-Dade County Department of Regulatory and Economic Resources on
05/16/2022).
• SUNCOR, Inc. structural steel erection/fabrication shop drawings sheets AB1 thru TP101 (total of 219 pages) dated
04/13/2022 (Rev. A – Issued for Approval) Submittal #051200-5.0 (Structural Steel Shop Drawings), reviewed for
compliance (stamped “Reviewed as Noted - Make Corrections”) by JSG on 05/03/2022.
• Canam Steel Corporation (CSC) steel metal deck shop drawings sheets DE1.1 thru DE2.1 (total of 3 pages) dated
04/08/2022 (Issued for Approval) Submittal #051200-6.0 (Deck Shop Drawings), reviewed for compliance (stamped
“Reviewed as Noted - Make Corrections”) by JSG on 04/15/2022.
Record of Field Observations:
1. Visual verification of the structural steel connections at the following locations indicated below:
a) ½” Ø x 1½” shear studs welded connections to the proposed new C10x25 steel channels approx. between grids
“C.5” thru “D.5” and lines “5.5” thru “6.5 for the precast concrete double “T’s” repair on the roof at the existing
service department, as per the attached Conemco Engineering, Inc. precast concrete double Tee’s structural repair
drawings, in progress on 07/27/23 – pending reinspection:
➢ Welding slag was not completely removed from locations that require Visual Testing (VT).
➢ Incomplete or undersize fillet welds.
➢ Missing three (3) shear studs at locations marked in the field.
The structural steel connections at the locations mentioned above were found not to be in substantial compliance with the
reference documents. General Contractor will call for item 1a re-inspection after completion for final compliance verification
and prior to the placement of concrete.
Welder qualification certificate was provided on 07/27/2023, however current continuity paperwork pending for review
(Period of Effectiveness). The welder that performed the work was not available onsite in order to verify his welder
qualification.
As per The Structural Welding Code:
6.2.3 Period of Effectiveness
6.2.3.1 Welders and Welding Operators. The welder’s or welding operator’s qualification as specified in this code shall be
considered as remaining in effect indefinitely unless:
(1) the welder is not engaged in a given process of welding for which the welder or welding operator is qualified
for a period exceeding six months, or
(2) there is some specific reason to question a welder’s or welding operator’s ability (see 6.25.1).
2. Visual verification of the reinforcing steel placement at the following locations indicated below:
a) Proposed new 6” thick interior slab on grade reinforced with 6x6 – W2.9xW2.9 W.W.F. with thickened edge over a
10 Mil vapor barrier and compacted fill between grids “D.6” thru “E.2” and lines “11” thru “13” at the Quick Lube DRAFT

(130) and between grids “G” thru “H” and lines “9.9” thru “10.2” at the Tech Training Room (121), top of slab
elevation 0’−0”, as per S1.0, S2.0, 2/S3.3 and S4.0/details 1, 5 & 7, completed. Termite treatment was performed
and vapor barrier was in placed prior to this site visit.
b) Continuous wall footing mark “WF26” (Cont. 2’−6”x1’−0”) with vertical dowels along grids “G” and “H” between lines
“8” thru “9” and along line “9” between grids “G” thru “G.3”, top of footing elevation approx. −1'−4", as per
S1.1/Foundation Schedule, S2.0, 1/S3.3, S4.0/details 1, 2, 3 & B and 1/S4.2, completed.
c) Continuous wall footing mark “WF20” (Cont. 2’−0”x1’−0”) with vertical dowels along grid “J” between lines “8” thru
“9”, top of footing elevation approx. −1'−4", as per S1.1/Foundation Schedule, S2.0, 1/S3.1, S4.0/details 1, 2, 3 &
B and 1/S4.2, completed.
d) Elevator pad with vertical dowels between grids “G.4” thru “H” and lines “8.5” thru “9” at Lobby, top of footing
elevation approx. −1'−4", as per S1.1/Foundation Schedule, S2.0, 1/S3.3 and 1/S4.2, completed.
e) Retrofit CIP concrete column at approx. gridline Dx3.3 at the existing service building, as per SK 04-25-23,
completed.
The reinforcing steel placement at the locations mentioned above was found to be in substantial compliance with the
reference documents. In place density tests were performed by a UES technician and it was found to meet or exceed the
minimum compaction requirement of 95% and ASTM D−1557.
Mr. Jim Jenco, Project Superintendent and Mr. Sebastian Cienfuegos, Assistant Superintendent with Lebolo Construction,
were notified of the aforementioned visual verification results.
