04/13/2022 (Rev. A – Issued for Approval) Submittal #051200-5.0 (Structural Steel Shop Drawings), reviewed for
compliance (stamped “Reviewed as Noted - Make Corrections”) by JSG on 05/03/2022.
• Canam Steel Corporation (CSC) steel metal deck shop drawings sheets DE1.1 thru DE2.1 (total of 3 pages) dated
04/08/2022 (Issued for Approval) Submittal #051200-6.0 (Deck Shop Drawings), reviewed for compliance (stamped
“Reviewed as Noted - Make Corrections”) by JSG on 04/15/2022.
Record of Field Observations:
1. Visual verification of the structural steel connections at the following locations indicated below:
a) 3” deep 22 gauge galvanized G80 corrugated roof metal deck profile type “N” prior roof openings (A/S ducts, ventilations,
roof hatches, etc.), connected to intermediate supports with 5/8ӯ puddle welds on a 24/4 pattern, 5/8ӯ puddle welds at
6” o.c. at deck perimeter and with #12 Type self-tapping screws @ 3” o.c. at side laps per span between grids “A” thru
“E.3” and lines “8” thru “11” at building service expansion, as per S1.0, S2.2, 2/S3.2, S4.1/details 1, 2, 3, 4 & 5, Canam
Steel Corporation deck shop drawing sheets DE1.1, DE1.3 & DE2.1 and attached clarification submitted by the steel
erector via e-mail, as we addressed on UES Structural Observation Report No. 34 (No.: 7242) dated 07/06/2023,
completed.

in progress (pending final verification for item 1c below).
c) Continuous perimeter deck bearing angle L4”x4”x1/4” anchored connections (torque verified at 100 ft−lb as per Simpson
Strong−Bolt 2 Wedge Anchor required installation torque schedule) to CMU bond beam along grid “D.6” between lines
“11” thru “12” at the oil storage, as per S1.0, S1.1/details 2 & 3, S2.2, 2/S3.1, 4/S4.1, E1000 and E1001, in progress on
07/06/23:
➢ Two (1) expansion anchors were found to be loose fitting at approx. gridlines D.6x11.3 and D.6x11.5.
The structural steel connections at the locations mentioned above was found to be in substantial compliance with the reference
documents, except for items 1b and 1c in progress. General Contractor will call for items 1b and 1c re−inspection after
completion for final compliance verification.
Mr. Jim Jenco, Project Superintendent and Mr. Sebastian Cienfuegos, Assistant Superintendent with Lebolo Construction, were
notified of the aforementioned visual verification results.

All laps on the deck are TEK screwed except one area of decking that ran on top of the edge of W27 x 84.
This is the area where the lap was puddle welded (approx. 32’ LF).
I edited the Procore RFI90 and included the aƩached marked up S2.2 Roof Framing Plan indicaƟng this area. DRAFT
